Outcomes of lung transplantation from organ donation after medical assistance in dying: First North American experience

Summary: This study compared lung transplantation outcomes using medical assisting in dying (MAID) donors and neurologically determined death and controlled donation after circulatory death (NDD/cDCD) donors. The results showed excellent recipient outcomes for lung transplantation using MAID donors, with similar intermediate-term outcomes compared to other types of donors. Therefore, donation after MAID should be strongly considered for lung transplantation where permitted.

Lung transplantation for acute respiratory distress syndrome

Summary: Lung transplantation is an effective therapy for chronic lung failure, but its role in acute lung failure, such as ARDS, is not well-established. With the increasing number of unweanable post-COVID-19 ARDS patients on ECMO, many lung transplant centers are evaluating the use of lung transplantation for ARDS.

Ex vivo lung evaluation of single donor lungs when the contralateral lung is rejected increases safe use

Summary: This study retrospectively reviewed single-lung transplant surgeries performed between 2000 and 2017 and compared the outcomes of patients before and after the introduction of EVLP. The study found that the use of EVLP increased the number of single-lung transplants when the contralateral lung was rejected, but there was no significant difference in survival rates.

Altered purine metabolism at reperfusion affects clinical outcome in lung transplantation

Summary: This study aims to investigate the metabolomic and transcriptomic profiles in human lungs before and after reperfusion during lung transplantation. The results showed activation of energy metabolism and reduced antioxidative biochemicals during reperfusion. Upregulation of genes related to cytokines and inflammatory mediators were identified. Purine metabolism was found to be the most significantly enriched pathway during reperfusion. Levels of inosine and uric acid were significantly associated with intensive care unit length of stay and ventilation time, indicating the potential role of oxidative stress through purine metabolism pathway in ischaemia reperfusion injury of lung grafts.

Extracorporeal life support as a bridge to lung transplantation: Where are we now?

Summary: The demand for lung transplant procedures is increasing, but there is a shortage of donor organs and high mortality among patients on waiting lists. Extracorporeal life support (ECLS) has emerged as a potential treatment option to support critically ill patients while they await a suitable organ match. This review explores the evolving landscape of ECLS as a bridge to lung transplantation, including patient selection criteria, survival predictors, pre and peri-transplant support methods, and the importance of a multidisciplinary approach to care. Innovations are being considered to improve the accessibility, safety, and effectiveness of ECLS for future lung transplant candidates.

Metabolomic fingerprinting of porcine lung tissue during pre-clinical prolonged ex vivo lung perfusion using in vivo SPME coupled with LC-HRMS

Summary: Normothermic ex vivo lung perfusion (NEVLP) is a modernized organ preservation technique that allows for detailed assessment of donor lung function prior to transplantation. This study identified potential biomarkers of lung function and/or injury during prolonged NEVLP using in vivo solid-phase microextraction (SPME) technology. The study showed that in vivo SPME fibers can be used for repeated sampling of lung tissue and monitoring biochemical changes and alterations in the metabolomic profile of the lung. Several upregulated biochemical pathways involved in pro- and anti-inflammatory responses, as well as lipid metabolism, were observed during extended lung perfusion. The feasibility of using SPME as an in vivo method in metabolomics studies was also assessed.

L-alanyl-L-glutamine modified perfusate improves human lung cell functions and extend porcine ex vivo lung perfusion

Summary: This study found that adding L-alanyl-L-glutamine during ex vivo lung perfusion (EVLP) can improve lung tissue function, provide antioxidant and cytoprotective effects, and provide additional energy support for lung transplantation. The study also developed a method to further optimize perfusates and improve EVLP outcomes.

Outcomes of lung transplantation from donors with a history of substance abuse

Summary: The study aimed to determine the impact of donor substance abuse (opioid overdose death, opioid use, cigarette or marijuana smoking) on lung acceptance and recipient outcomes. The results showed that smoking affected lung acceptance, but opioid use, opioid overdose death, or marijuana use did not. Recipient survival rates were not influenced by donor substance abuse.

Influence of anterior tumor location on survival after resection of lung cancer invading the thoracic inlet (Pancoast tumors)

Summary: This study investigated the influence of tumor location along the first rib on survival in patients with non-small cell lung carcinomas invading the thoracic inlet. The results showed that tumor location did not significantly affect the rate of R0 resection and pathological complete response to induction therapy. However, posterior tumors, complete response to therapy, posterior location, and age less than 60 years were associated with better survival. Anterior tumors had a higher risk of systemic recurrence and derived greater benefit from pathological complete response.

Determining the impact of ex-vivo lung perfusion on hospital costs for lung transplantation: A retrospective cohort study

Summary: This study compared the institutional costs of lung transplantation before and after the availability of EVLP and determined the health-economic impact of EVLP.
