Assisted reproductive techniques after fertility-sparing treatments in gynaecological cancers

BACKGROUND: The trend toward late childbearing has made fertility preservation a major issue for women who face gynecological cancer. New techniques in assisted reproductive medicine enable conception after primary treatment of these cancers. Here, we aimed to review the efficacy and safety of assisted reproductive techniques (ART) after fertility-preserving treatment of gynaecological cancers. METHODS: We conducted a systematic literature review of both prospective and retrospective studies in the PubMed, EMBASE, CENTRAL and SciSearch databases. In the retrieved studies, we evaluated live births, clinical pregnancies, overall survival and disease-free survival. RESULTS: We identified many prospective and retrospective studies on this topic, but no relevant randomized clinical trials. Fertility-sparing treatments with safe oncological outcomes are feasible in endometrial, cervical and ovarian cancer cases. After cancer treatment, ART seem safe and show variable obstetrical outcomes. CONCLUSIONS: After fertility-preserving treatment for gynaecological cancers, ART can enable pregnancy to be achieved with apparent oncological safety. The success of such procedures should directly impact clinical practice and management of those patients who require fertility-sparing treatment.

Laparoscopic single-port versus minilaparoscopic histerectomy: an International Study.

Summary: This retrospective multicentric study compared laparoendoscopic single-site surgery (LESS) and mini-laparoscopic surgery (Mini-LPS) in terms of surgical time, postoperative pain, and hospital stay for patients undergoing simple hysterectomy for a benign condition. The results showed shorter surgical time and hospital stay in the Mini-LPS group compared to the LESS group. However, the Mini-LPS group had a higher rate of conversion to standard laparoscopy due to instrumentation flaws. Both ultra-minimally invasive techniques were found to be safe for performing hysterectomy for a benign condition.

Avascular spaces in radical hysterectomy

Summary: This review aims to synthesize currently used anatomical landmarks with relation to surgical avascular spaces for the performance of radical hysterectomy. A comprehensive computer-based review of various databases and guidelines was conducted to gather relevant data. The manuscript includes a clear definition of avascular spaces, methods of dissection, and anatomical limits, and provides a better understanding of the surgical procedure.

Personalized Sentinel Node Mapping in Endometrial Cancer by the Indocyanine Green Implementation as Single Tracer: A Case Control Study

Summary: The study aimed to compare the effectiveness of indocyanine green (ICG) as a single tracer versus Technetium99 + ICG for bilateral sentinel lymph node detection in endometrial cancer. The results showed that ICG as a single tracer achieved higher rates of bilateral detection, and the oncological outcomes were not affected by the type of tracer used.

Summary: This retrospective study analyzed 683 patients with uterine sarcoma to identify and evaluate the impact of different prognostic factors on overall survival and disease-free survival. The results showed variations in 5-year overall survival and disease-free survival rates among different subtypes of uterine sarcoma. The presence of residual disease after surgery was identified as the most significant factor affecting overall survival.

Summary: The European Society of Gynaecological Oncology (ESGO) and the European SocieTy for Radiotherapy and Oncology (ESTRO) have developed a list of quality indicators for radiation therapy of cervical cancer. These indicators aim to improve the quality of care for cervical cancer patients and provide a quantitative basis for practitioners and administrators to improve clinical practice. The list includes 19 indicators based on scientific evidence and expert consensus.

Influence of telomerase activity and initial distribution on human follicular aging: Moving from a discrete to a continuum model

Summary: This paper proposes a discrete model for the temporal evolution of a population of cells sorted according to their telomeric length. The model assumes asymmetric distribution of genetic material during cell division, with one daughter cell having the same telomere length as the mother and the other having shorter telomeres. The model takes into account telomerase activity, cell death, and the Hayflick limit. A partial differential equation with boundary conditions is derived to represent the continuous model. The model is applied to study the aging rate of human follicular growth.

Influence of Clinical and Surgical Factors on Uterine Carcinosarcoma Survival

Summary: Uterine carcinosarcoma is a rare disease that accounts for less than 5% of uterine malignancies but contributes to approximately 16% of uterine cancer-related deaths. In this study, prognostic factors, such as incomplete cytoreduction, advanced disease stage, presence of residual tumor, extrauterine disease, and tumor size, were found to significantly impact disease-free survival and overall survival among patients with uterine carcinosarcoma.

Role of Adjuvant Radiotherapy in Patients with Cervical Cancer Undergoing Radical Hysterectomy

Summary: The benefit of adjuvant radiotherapy (RT) after radical hysterectomy in cervical cancer patients remains controversial. This study found that adjuvant RT did not significantly impact survival in early-stage cervical cancer patients who met Sedlis criteria. However, further studies are needed to determine if adjuvant RT is necessary for patients with intermediate risk factors.

Summary: The purpose of this study was to analyze the prognostic factors related to the recurrence rate and overall survival of patients with undifferentiated uterine sarcoma. The results showed that FIGO stage was the most important prognostic factor, adjuvant radiotherapy was associated with better disease-free and overall survival, while chemotherapy was associated with shorter disease-free survival.
