Summary: Fertility-sparing treatment is a viable option for women with gynecologic cancers, with different surgical and non-surgical approaches available depending on the type and stage of the cancer. Strategies such as progestin therapy, ovarian preservation, and assisted reproductive technology can help preserve fertility while ensuring effective cancer treatment. Ongoing research into methods such as oocyte or embryo cryopreservation provides promising possibilities for the future of fertility preservation in gynecologic oncology.

Summary: Many young cervical cancer survivors desire parenthood and have successful reproductive outcomes after fertility-sparing treatments. Vaginal Radical Trachelectomy (VRT) shows good reproductive outcomes in early-stage disease without compromising oncological safety. Fertility preservation and gestational surrogacy provide promising alternatives for those requiring gonadotoxic treatments to achieve a biological offspring.

Summary: Endometrial cancer (EC) is rare in adolescents in developed countries, with the increasing rate of obesity in children and adolescents being held responsible for the rising prevalence in younger patients. Diagnosis of EC can have devastating consequences on future fertility due to standard treatment protocols involving hysterectomy. Genetic screening should be considered in very young patients with EC, as there is a high proportion of cases with genetic backgrounds such as Cowden syndrome and Turner syndrome.

Summary: The study aims to assess the presence of sociodemographic and clinical disparities in fertility-sparing treatment and ART use among patients with a history of cervical, endometrial, or ovarian cancer. The study found that there are sociodemographic disparities related to cancer type in fertility-sparing treatment and ART use.

Summary: Young women diagnosed with endometrial cancer face unique challenges, but preserving fertility is possible in early and low-grade cases. Data suggest that conservative approaches with close monitoring do not compromise oncologic outcomes. For patients unable to preserve fertility, assisted reproductive technologies can help achieve fertility goals.

Summary: Conservative management can be considered as an option for young women with grade 2, stage IA endometrial cancer who wish to preserve their fertility. The combination of levonorgestrel-releasing intra-uterine device and systemic therapy can result in a high rate of complete response. However, fertility-sparing treatment is not the current standard of care for young women with endometrial cancer.

Summary: This study examined the oncological and obstetric outcomes of radical trachelectomy in early-stage cervical cancer patients. The results showed that radical trachelectomy is a safe and effective treatment for preserving fertility in women with early-stage cervical cancer, particularly for those with tumor size ≤ 4 cm.

Summary: Endometrial cancer is the fourth most common cancer in women in developed countries. Its incidence has increased in young women, possibly due to factors such as delayed childbearing and obesity. The new molecular classification of the disease helps personalize treatment. Current evidence suggests that oral progestin alone or in combination with other drugs is a feasible and safe option for the conservative management of early-stage endometrial cancer limited to the endometrium.

Summary: Fertility preservation appears feasible in young patients with grade 1 EC limited to the endometrium or atypical endometrial hyperplasia. Progestins are the mainstay of such management. The addition of Metformin and hysteroscopic resection seems to provide some improvements. However, fertility preservation is not the standard approach for staging and treatment, potentially worsening oncologic outcomes.

Summary: The study found that young adult women had a high satisfaction rate with ovarian tissue cryopreservation (OTC) and believed that the experience was beneficial. Most patients believed that OTC had a positive impact on their health during disease treatment. The majority of women successfully planned a pregnancy after treatment, with a natural pregnancy rate close to 53%.

Summary: This retrospective multicentric study compared laparoendoscopic single-site surgery (LESS) and mini-laparoscopic surgery (Mini-LPS) in terms of surgical time, postoperative pain, and hospital stay for patients undergoing simple hysterectomy for a benign condition. The results showed shorter surgical time and hospital stay in the Mini-LPS group compared to the LESS group. However, the Mini-LPS group had a higher rate of conversion to standard laparoscopy due to instrumentation flaws. Both ultra-minimally invasive techniques were found to be safe for performing hysterectomy for a benign condition.

Summary: The study aimed to compare the effectiveness of indocyanine green (ICG) as a single tracer versus Technetium99 + ICG for bilateral sentinel lymph node detection in endometrial cancer. The results showed that ICG as a single tracer achieved higher rates of bilateral detection, and the oncological outcomes were not affected by the type of tracer used.

Summary: This retrospective study analyzed 683 patients with uterine sarcoma to identify and evaluate the impact of different prognostic factors on overall survival and disease-free survival. The results showed variations in 5-year overall survival and disease-free survival rates among different subtypes of uterine sarcoma. The presence of residual disease after surgery was identified as the most significant factor affecting overall survival.

Summary: This paper proposes a discrete model for the temporal evolution of a population of cells sorted according to their telomeric length. The model assumes asymmetric distribution of genetic material during cell division, with one daughter cell having the same telomere length as the mother and the other having shorter telomeres. The model takes into account telomerase activity, cell death, and the Hayflick limit. A partial differential equation with boundary conditions is derived to represent the continuous model. The model is applied to study the aging rate of human follicular growth.

Summary: Uterine carcinosarcoma is a rare disease that accounts for less than 5% of uterine malignancies but contributes to approximately 16% of uterine cancer-related deaths. In this study, prognostic factors, such as incomplete cytoreduction, advanced disease stage, presence of residual tumor, extrauterine disease, and tumor size, were found to significantly impact disease-free survival and overall survival among patients with uterine carcinosarcoma.

Summary: The benefit of adjuvant radiotherapy (RT) after radical hysterectomy in cervical cancer patients remains controversial. This study found that adjuvant RT did not significantly impact survival in early-stage cervical cancer patients who met Sedlis criteria. However, further studies are needed to determine if adjuvant RT is necessary for patients with intermediate risk factors.

Summary: The purpose of this study was to analyze the prognostic factors related to the recurrence rate and overall survival of patients with undifferentiated uterine sarcoma. The results showed that FIGO stage was the most important prognostic factor, adjuvant radiotherapy was associated with better disease-free and overall survival, while chemotherapy was associated with shorter disease-free survival.
