Early Versus Late Recurrence of Hepatocellular Carcinoma After Surgical Resection Based on Post-recurrence Survival: an International Multi-institutional Analysis

Summary: The study found that setting the time point for recurrence at 8 months is beneficial for distinguishing early from late recurrence, with early recurrence patients typically having shorter survival time and post-recurrence survival, as well as less likely to receive curative treatment. Curative-intent retreatment for late recurrence patients is associated with similar overall survival compared to patients without recurrence.

Simultaneous approach for patients with synchronous colon and rectal liver metastases: Impact of site of primary on postoperative and oncological outcomes

Simone Conci, Andrea Ruzzenente, Corrado Pedrazzani, Giulia Isa, Giulia Turri, Tommaso Campagnaro, Alessandro Valdegamberi, Fabio Bagante, Ivan Marchitelli, Alfredo Guglielmi

Summary: The study found that simultaneous resection of the primary tumor and liver metastases is feasible and safe for patients with colon and rectal cancer with synchronous liver metastases. This approach led to satisfactory oncological outcomes for both colon and rectal cancer. Right-sided colon cancer had a worse prognosis compared to left-sided colon cancer and rectal cancer.

Prediction of tumor recurrence by α-fetoprotein model after curative resection for hepatocellular carcinoma

Summary: The study found that the AFP model is more accurate in identifying which HCC patients may benefit the most from surgical resection, and is superior to the BCLC system in predicting tumor recurrence and patient survival.

Surgery for Bismuth-Corlette Type 4 Perihilar Cholangiocarcinoma: Results from a Western Multicenter Collaborative Group

Summary: In this study comparing short- and long-term outcomes for patients with Bismuth-Corlette (BC) type 4 perihilar cholangiocarcinoma (pCCA) versus BC types 2 and 3 pCCA undergoing surgical resection, the results showed significant differences in clinicopathologic characteristics between the two groups. However, the type of BC was not associated with prognosis for survival, indicating that resection remains a viable curative treatment option for selected patients with BC type 4 pCCA.

Colorectal cancer with microsatellite instability: Right-sided location and signet ring cell histology are associated with nodal metastases, and extranodal extension influences disease-free survival

Florena, Michele Genna, Francesca Fior, Liang Cheng, Rita T. Lawlor, Aldo Scarpa, Corrado Pedrazzani, Claudio Luchini

Summary: By studying the risk of nodal metastasization and prognostic factors in colorectal cancer with microsatellite instability, the study revealed the histological consistency between primary tumors and matched nodal metastases, with extranodal extension identified as the strongest prognostic variable in MSI-CRC with nodal metastasis. Additionally, factors such as right-sided location, pT4 stage, and signet-ring histology were found to be commonly associated with nodal metastasization in MSI-CRC.

Role of Inflammatory and Immune-Nutritional Prognostic Markers in Patients Undergoing Surgical Resection for Biliary Tract Cancers

Simone Conci, Tommaso Campagnaro, Elisa Danese, Ezio Lombardo, Giulia Isa, Alessandro Vitali, Ivan Marchitelli, Fabio Bagante, Corrado Pedrazzani, Mario De Bellis, Andrea Ciangherotti, Alfredo Guglielmi, Giuseppe Lippi, Andrea Ruzzenente

Summary: This study evaluated the prognostic performance of different inflammatory and immune-nutritional markers in patients undergoing surgery for biliary tract cancer (BTC). The results showed that LMR is an independent prognostic factor for long-term survival, while PNI is associated with higher risk of postoperative mortality.

Molecular characterization of extrahepatic cholangiocarcinoma: perihilar and distal tumors display divergent genomic and transcriptomic profiles

Michele Simbolo, Samantha Bersani, Caterina Vicentini, Sergio Taormina, Chiara Ciaparrone, Fabio Bagante, Borislav Rusev, Giovanni Centonze, Marina Montresor, Matteo Brunelli, Serena Pedron, Andrea Mafficini, Gaetano Paolino, Paola Mattiolo, Simone Conci, Massimo Milione, Alfredo Guglielmi, Andrea Ruzzenente, Aldo Scarpa, Claudio Luchini

Summary: PHCC demonstrated more frequent KRAS mutations, while TP53 mutations were more common in DECC. Potentially actionable alterations included high tumor mutational burden and/or microsatellite instability (7.1%), PI3KCA mutations (8.1%), and MYC (10.1%) and ERBB2 amplification (5.1%). Transcriptomic profiles revealed three distinct clusters, corresponding to anatomical origin and differing in immune microenvironment. DECC appeared to consist of two distinct tumor subgroups, one enriched for druggable alterations and one lacking actionable opportunities.
