Evaluation of the Intention-to-Treat Benefit of Living Donation in Patients With Hepatocellular Carcinoma Awaiting a Liver Transplant

This study found that LDLT can reduce the risk of death in HCC patients on the waiting list for liver transplant, improving survival rates.
IMPORTANCE Living-donor liver transplant (LDLT) offers advantages over deceased-donor liver transplant (DDLT) of improved intention-to-treat outcomes and management of the shortage of deceased-donor allografts. However, conflicting data still exist on the outcomes of LDLT in patients with hepatocellular carcinoma (HCC). OBJECTIVE To investigate the potential survival benefit of an LDLT in patients with HCC from the time of waiting list inscription. DESIGN, SETTING, AND PARTICIPANTS This multicenter cohort study with an intention-to-treat design analyzed the data of patients aged 18 years or older who had an HCC diagnosis and were on a waiting list for a first transplant. Patients from 12 collaborative centers in Europe, Asia, and the US who were on a transplant waiting list between January 1, 2000, and December 31, 2017, composed the international cohort. The Toronto cohort comprised patients from 1 transplant center in Toronto, Ontario, Canada who were on a waiting list between January 1, 2000, and December 31, 2015. The international cohort centers performed either an LDLT or a DDLT, whereas the Toronto cohort center was selected for its capability to perform both LDLT and DDLT. The benefit of LDLT was tested in the 2 cohorts before and after undergoing an inverse probability of treatment weighting (IPTW) analysis. Data were analyzed from February 1 toMay 31, 2020. MAIN OUTCOMES AND MEASURES Intention-to-treat deathwas defined as a patient death that occurred for any reason and was calculated from the time of waiting list inscription for liver transplant to the last follow-up date (December 31, 2019). Four multivariable Cox proportional hazards regression models for intention-to-treat death were created. RESULTS A total of 3052 patients were analyzed in the international cohort, of whom 2447 were men (80.2%) and the median (IQR) age at first referral was 58 (53-63) years. The Toronto cohort comprised 906 patients, of whom 743 were men (82.0%) and the median (IQR) age at first referral was 59 (53-63) years. In all the settings, LDLT was an independent protective factor, reducing the risk of overall death by 49% in the pre-IPTW analysis for the international cohort (HR, 0.51; 95% CI, 0.36-0.71; P <.001), 33% in the post-IPTWanalysis for the international cohort (HR, 0.67; 95% CI, 0.53-0.85; P =.001), 43% in the pre-IPTWanalysis for the Toronto cohort (HR, 0.57; 95% CI, 0.45-0.73; P <.001), and 48% in the post-IPTWanalysis for the Toronto cohort (HR, 0.52; 95% CI, 0.42 to 0.65; P <.001). The discriminatory ability of the mathematical models further improved in all of the cases in which LDLT was incorporated. CONCLUSIONS AND RELEVANCE This study suggests that having a potential live donor could decrease the intention-to-treat risk of death in patients with HCC who are on a waiting list for a liver transplant. This benefit is associated with the elimination of the dropout risk and has been reported in centers in which both LDLT and DDLT options are equally available.

This study aimed to investigate the impact of holidays and different days of the week on the number of liver transplantations and early graft survival rates. The study found that fewer liver transplantations were performed on holidays and Mondays, but the survival rates were not affected by holidays or the day of transplantation.

Bile duct injury following cholecystectomy: delayed referral to a tertiary care center is strongly associated with malpractice litigation

Summary: The aim of this study was to evaluate the risk factors for malpractice litigation in patients with bile duct injury (BDI) referred to a tertiary care center. The results showed that patients in the late-referral group were more likely to have undergone surgical and endoscopic repair before referral. Prompt referral to experienced tertiary care centers without any attempt at repair may reduce the risk of litigation.

Summary: The individual prediction of post-liver transplantation hepatocellular carcinoma (HCC) recurrence risk is important. By analyzing the data of 4981 HCC patients undergoing transplantation, a prediction score was developed to accurately discriminate the risk of HCC recurrence. The score was externally validated in another 1160 patients, showing consistent results in risk discrimination.

Outcome of liver transplantation in elderly patients: an Italian multicenter case-control study

Summary: Despite the controversial results of liver transplantation in elderly recipients, this study found that liver transplantation in elderly patients (>= 65 years) provides favorable but inferior results compared to younger patients. Early allograft dysfunction was more frequent in elderly patients, and recipient age >= 65 years was an independent risk factor for patient death and graft loss. Cold ischemia time seems to be a crucial factor for favorable outcomes in elderly patients.

Summary: This study aimed to evaluate the magnesium content in human cirrhotic liver and its correlation with serum AST levels, expression of hepatocellular injury, and MELDNa prognostic score. The results showed that patients with cirrhosis had lower hepatic magnesium content and higher percentage of TRPM7-positive hepatocytes. The increase in hepatic magnesium content and TRPM7 expression were associated with worsening of MELDNa score. These findings suggest that severity of hepatocyte injury and prognosis in cirrhosis are closely related to magnesium depletion and overexpression of TRPM7.

Summary: This study aims to demonstrate the adverse effect of donor gamma-glutamyl transferase (GGT) on graft loss in a large European liver transplant cohort. Donor GGT is found to be an independent predictor of graft loss and its inclusion in traditional prediction models improves the accuracy. However, the exact mechanisms linking GGT and poor transplant outcomes need further investigation.

Adult 10-year survivors after liver transplantation: a single-institution experience over 40 years

Summary: This study aims to analyze the monocentric liver transplantation experience to identify the pre-operatively available parameters limiting a 10-year post-transplant survival. The study found that there were differences in initial liver function, surgical techniques, and immunosuppression between the short surviving group (< 10 years) and the long surviving group (& GE; 10 years). Age, transplantation era, and UNOS status 1-2A were identified as independent risk factors for not reaching at least 10 years of follow-up.

New Insights in the Setting of Transplant Oncology

Summary: This paper reviews the recent developments in liver transplantation (LT) for unresectable colorectal liver metastases (CRLM) and cholangiocellular carcinoma (CCA), and suggests that integration of LT with (neo)-adjuvant chemotherapy, radiotherapy, or locoregional treatments can effectively cure selected patients. However, further advances in diagnosis, treatment protocols, and donor-to-recipient matching are needed to better define the oncological indications for transplantation. Prospective, multicenter trials are still required to study these advances and their impact on outcomes.
