The study found that lower serum total testosterone levels in male COVID-19 patients were associated with poor prognosis and fatal outcomes, indicating a potential role for testosterone in the pathogenesis of the disease. This could help clinicians determine the clinical course of COVID-19.
Background A potential role of testosterone among sex hormones has been hypothesized in identifying sex-related differences in the clinical consequences of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection. Due to the high global prevalence of hypogonadism, the relationship between hypogonadism and SARS-CoV-2 infection outcomes deserves an in-depth study. Objective The present study aimed to investigate the relationship of serum testosterone with other laboratory parameters on the prognosis of coronavirus disease-19 (COVID-19) in male patients with COVID-19 diagnosis. Materials and methods This prospective cohort study included 358 male patients diagnosed with COVID-19 and 92 COVID-19 negative patients admitted to the urology outpatient clinics as a control group. The COVID-19 patients were divided into groups according to prognosis (mild-moderate and severe group), lung involvement in chest computed tomography (50%), intensive care unit needs, and survival. Results The measured serum total testosterone level of the COVID-19 patients group was found to be significantly lower than that of the control group (median, 140 ng/dl; range, 0.21-328, 322 ng/dl; range, median, 125-674, p < 0.001, respectively). The serum TT levels were statistically significantly lower in severe COVID-19 patients compared to mild-moderate COVID-19 patients (median, 85.1 ng/dl; range, 0.21-532, median, 315 ng/dl; range, 0.88-486, p < 0.001, respectively), in COVID-19 patients in need of intensive care compared to COVID-19 patients who did not need intensive care (median, 64.0 ng/dl; range, 0.21-337, median, 286 ng/dl; range, 0.88-532 p < 0.001, respectively), and in COVID-19 patients who died compared to survivors (median, 82.9 ng/dl; range, 2.63-165, median, 166 ng/dl; range, 0.21-532, p < 0.001, respectively). Discussion and conclusion Our data are compatible with low TT levels playing a role on the pathogenesis of the disease in Covid-19 patients with poor prognosis and a mortal course and may guide clinicians in determining the clinical course of the disease.

Summary: This prospective study aimed to investigate the relationship between smartphone screen time and the intravaginal ejaculation latency time (IELT) in patients with acquired PE. The study included 30 patients diagnosed with acquired PE and 30 healthy volunteers as the control group. The results showed that smartphone screen time negatively affected IELT values and PE severity in patients with acquired PE.

Summary: VEXAS syndrome, a newly defined disorder, poses challenges regarding treatment options. A case report of a patient with atypical symptoms and an intriguing treatment course is presented. The patient exhibited fatigue, recurring fever, lung infiltrates, proteinuria, anemia, leucopenia, transient skin rash, and elevated acute phase reactants. Corticosteroid tapering was not well tolerated, but rapid recovery was achieved following diagnostic splenectomy, which revealed significant findings upon pathological examination.

Visceral leishmaniasis in a patient with systemic lupus erythematosus: Dilemma in diagnosis and management

Summary: Patients with systemic lupus erythemasus (SLE) are at increased risk of infections, particularly when undergoing immunosuppressive therapy. Leishmaniasis is a parasitic disease caused by Leishmania protozoan parasites. We present a case of a patient with SLE who was also diagnosed with leishmaniasis and showed significant improvement after treatment.

Summary: This study aimed to determine the timing of contralateral testicular fixation in postpubertal patients with testicular torsion. The results showed that a patient-based approach, prioritizing the clinical characteristics of the patient, can lead to more effective and safe outcomes. This research provides valuable insights for the management of testicular torsion in postpubertal patients.

Summary: The study aimed to determine the reference interval of RDW to platelet count ratio (RPR) for premature newborns. By reviewing the medical records of preterm infants between January 2016 and December 2018, it was found that the reference interval for RPR in infants with a gestational age of 28-35 weeks was 0.038-0.126, 0.042-0.129 for infants with a gestational age of 32-35 weeks, and 0.022-0.121 for infants with a gestational age of 28-31 weeks. Conclusion: Establishing the reference interval for RPR in premature infants allows for clinical correlation of RPR alterations in this population.

Isolated inferior thyroidal artery vasculitis: A rare cause of neck pain

Summary: This is a case report of isolated vasculitis of bilateral inferior thyroidal arteries in a 49-year-old female patient. The patient presented with neck pain and was diagnosed with ultrasonography and computed tomographic angiography. The treatment with glucocorticoid induction and addition of methotrexate was successful in managing the clinical and imaging findings. Localized forms of vasculitis are rare and the small size of the affected area makes diagnostic investigations and management more complicated.

Summary: The study aimed to evaluate the type and frequency of pulmonary involvement in ANCA-associated vasculitis (AAV) and investigate the potential relationship with thorax CT signs and other clinical findings. It was found that lung involvement was detected in almost all patients with AAV, and severe pulmonary involvement was more common in MPO-ANCA positive patients and associated with severe disease. Therefore, pulmonary examination by imaging may be useful for determining vasculitis subtype and disease extent in AAV patients.

A case of Behcet syndrome presenting with acute coronary syndrome

Summary: Cardiac involvement is rare in Behcet syndrome, but it can be the first manifestation of the disease. Early diagnosis and initiation of immunosuppressive treatment are crucial for improving the prognosis. Coronary aneurysms can develop spontaneously or as a reaction to vascular intervention, and the risk of restenosis is high without immunosuppressive treatment. We present a case of a young male diagnosed with Behcet syndrome after acute coronary syndrome caused by coronary artery aneurysms and thrombosis.

Comparison of unilateral and bilateral microdissection testicular sperm extraction (MD-TESE) in patients with non-obstructive azoospermia: a prospective study

Adem Utlu, Fatih Ozkaya, Tugay Aksakalli, Ahmet Emre Cinislioglu, Saban Oguz Demirdogen, Mehmet Sefa Altay, Ibrahim Karabulut, Isa Ozbey, Sibel Guclu Utlu

Summary: This study compared the sperm retrieval rates between unilateral and bilateral microdissection testicular sperm extraction (MD-TESE) procedures in patients with non-obstructive azoospermia (NOA), and compared them with literature data. The results showed no significant difference in terms of sperm availability between the two groups of patients. Considering the operative time and complication rates of bilateral MD-TESE in patients with NOA, as well as possible subsequent MD-TESE procedures, unilateral MD-TESE is considered to be more preferable for this patient group.

Summary: This study evaluated the diagnostic performance of CT findings in the histological results of acute appendicitis. Inflammatory rheumatic diseases developed after COVID-19 vaccination: presentation of a case series and review of the literature

G. Akkuzu, C. Bes, D. S. Ozgur, B. Karaaliolu, M. Y. Mutlu, F. Yildirim, P. Atagunduz, A. Gunduz, M. Soy

Summary: This study investigated the clinical characteristics and laboratory parameters of newly developed autoimmune-inflammatory rheumatic diseases (AIRD) in individuals recently vaccinated for COVID-19. Among the 22 cases vaccinated, 14 cases developed AIRD, with 54.5% of them diagnosed with rheumatoid arthritis. The study suggests that COVID-19 vaccination may lead to the development of AIRD and should be carefully considered in newly diagnosed AIRD patients.
