Summary: The aim of this study was to determine the real-world effectiveness and survival of methotrexate (MTX) and adalimumab (ADA) in patients with moderate-to-severe psoriasis registered in BADBIR. The study found that patients on ADA were twice as likely to be clear or nearly clear of psoriasis and were less likely to discontinue their medication than patients on MTX.

Summary: This study investigated psoriasis patients with poor response to multiple biologic agents and identified characteristics associated with multiple biologic failure (MBF). The results showed that about 6% of patients met the criteria for MBF, and certain characteristics such as female sex, shorter psoriasis duration, and prior nonbiologic systemic therapy use were associated with MBF. The findings provide valuable insights for identifying patients who may require more frequent follow-up.

Summary: Psoriasis affects approximately 3% of the population in the USA and costs over $110 billion annually. The development of targeted biologics has revolutionized psoriasis management, but trial-and-error approach leads to frequent biologic switching, causing unnecessary wasted spending. Dermatologists believe that using a dermal biomarker patch could reduce trial and error, improve patient outcomes, and help with drug selection. Many dermatologists expressed willingness to utilize this new predictive method.

Summary: Psoriasis is a chronic inflammatory disease affecting 2-3% of the global population due to genetic and environmental triggers. Understanding its pathophysiology has led to the development of novel therapeutic options such as biologic therapy and small-molecule inhibitors. Psoriasis is also associated with comorbid conditions like psoriatic arthritis, cardiovascular disease, psychiatric disorders, and inflammatory bowel disease. Close monitoring is required for psoriasis patients with inflammatory bowel disease who are treated with IL-17 inhibitors.

Summary: Biologic and nonbiologic immunomodulators used to treat immune-mediated inflammatory diseases (IMIDs) can affect the immune response to COVID-19 vaccines and their effectiveness. Patients with IMIDs receiving these therapies showed impaired seroconversion after COVID-19 vaccination, especially those on nonbiologic therapies like methotrexate.

Summary: This study investigated the nonadherence to immune-modifying therapy in individuals with psoriasis during the COVID-19 pandemic and its association with mental health. The most common reason for nonadherence was concerns about immunity. After adjusting for potential confounders, the association between anxiety and nonadherence to immune-modifying therapy became weaker and no longer significant. This study highlights the importance of clear and adequate communication with patients during a pandemic.

Summary: The skin is equipped with immune cells like mast cells and CD8 T cells which play a role in recognizing and responding to environmental challenges. These cells are essential for maintaining tissue balance and regulating the immune response in skin diseases. Studies have shown that mast cells can influence the migration and function of CD8 T cells, while CD8 T cells can also activate mast cells. The close proximity between mast cells and CD8 T cells has been observed in dermatoses such as alopecia areata. This review summarizes the bidirectional interactions between human mast cells and CD8 T cells, focusing on their involvement in psoriasis, atopic dermatitis, and vitiligo, and discusses unresolved questions.

In order to restore or maintain health, healthcare should focus on the patient's overall well-being, including life satisfaction, quality of life, and mental well-being. This expert review emphasizes the importance of incorporating well-being into the holistic therapeutic approach for psoriasis patients, and provides recommendations for clinical practice to implement well-being assessment and treatment.
