Summary: This study synthesized current evidence on prognostic factors for predicting postoperative pain after knee or hip replacement. The findings suggest that various preoperative factors, including race, opioid use, preoperative function, neuropathic pain symptoms, and anxiety, are associated with postoperative chronic pain. Additionally, certain comorbidities, such as heart disease and lung disease, are not associated with postoperative chronic pain.

Summary: The study aimed to compare the effectiveness of physical activity on prescription with a one-hour advice session in increasing physical activity levels. Despite a significant reduction in pain after walking in the prescription group, there were no significant differences between the two groups in other measures, indicating that individually tailored physical activity prescriptions did not significantly improve physical activity levels more than simple advice.

Summary: This study aimed to determine whether specific gait outcomes previously linked to symptomatic medial compartment knee OA are unique to knee OA. The results showed that there were no differences in knee joint kinematics and moments between the knee OA and asymptomatic groups, but the knee adduction moments were greater in the knee OA group compared to the hip OA group. The knee OA group also exhibited a higher activation ratio of the lateral to medial hamstrings compared to the hip OA and asymptomatic groups. This suggests that medial and lateral hamstring muscle activation levels may provide utility as a knee OA gait biomarker.

Summary: This study compared the baseline characteristics and treatment outcomes of patients with symptomatic knee or hip osteoarthritis participating in patient education and exercise therapy. The results showed clinically relevant improvements in both knee and hip patients at 3 and 12 months after treatment. Therefore, patients with knee and hip osteoarthritis should be prioritized for treatment with patient education and supervised exercise therapy.

Given its excellent safety profile, exercise should be given more prominence in clinical care, especially in older people with comorbidity or at higher risk of adverse events related to NSAIDs and paracetamol.

Summary: The objective of this study was to identify subgroups of patients with MRI-confirmed degenerative meniscus tears who may benefit from APM compared to non-surgical or sham treatment. The results showed a small improvement in knee pain for the APM group compared to the non-surgical or sham group at 24 months follow-up. However, there were no significant differences in overall knee function and health-related quality of life between the two groups. No relevant subgroup of patients who benefitted from APM was identified.

Summary: Exercise therapy is effective in treating patients with diabetic foot ulcers (DFUs), and this study describes the development of a 12-week exercise therapy program for DFU patients. The program consists of aerobic and resistance training exercises and was developed with input from healthcare professionals and patients. The preliminary feasibility of the program was assessed, and although recruitment rates were low, patients found the intervention relevant and expressed a desire for further guidance and a more convenient location for the intervention. However, further studies are needed to assess the feasibility of the program.

Summary: This study aims to compare the effects of early surgery and exercise and education on mechanical symptoms in patients aged 18-40 with a meniscal tear and self-reported mechanical knee symptoms. The results suggest that early surgery is more effective for relieving self-reported mechanical knee symptoms compared to exercise and education, but it does not improve pain, function, and quality of life.

Summary: The increasing number of people living with multimorbidity due to ageing populations and improved survival has raised concerns about polypharmacy, treatment burden, competing priorities, and poor coordination of care. Self-management programs are commonly included in interventions to address these issues, but there is currently a lack of overview on interventions supporting self-management in patients with multimorbidity.

Summary: The aim of this study was to investigate the utilization patterns of prescribed analgesics before, during, and after an exercise therapy and patient education program among patients with knee or hip osteoarthritis. The study found that the majority of patients had an increase in analgesic use in the year before the intervention, but a decrease during and after the intervention. A small proportion of patients accounted for more than half of the total analgesic dispensed during the study period.
