Summary: Endoscopic submucosal dissection is a common method for treating gastrointestinal tumors, but there is still no defined approach to reduce complications after the procedure. Clips are commonly used for mucosal defect closure, but their grasping force and size are limited. New clips and suture devices have been developed in recent years for defect closure.

Summary: This study assessed the safety and feasibility of using the OverStitch Sx system for the closure of large defects after endoscopic submucosal dissection (ESD). The results showed that the OverStitch Sx system achieved good technical and clinical success in closing large defects, with a same-day discharge rate of 77.4%. However, further trials are needed to determine its superiority for closure of defects in challenging locations.

Summary: A novel endoscopic multi-firing clip applicator (EMFCA) system was developed for closing large perforations by deploying clips through a tendon-sheath actuation system (TSAS). A pushing transmission model of the TSAS was proposed and tested through simulation and ex vivo experiments, showing a promising potential for accurate control of the EMFCA system and improving safety in endoscopic surgery.

Summary: Endoscopic full-thickness resection (EFTR) without laparoscopic assistance is a minimally invasive technique for treating gastric submucosal tumors originating from the muscularis propria. The recent development of flexible endoscopic suturing tools may enhance the efficacy and safety of EFTR procedures.

Summary: This study aimed to demonstrate the feasibility and safety of a novel twin-grasper assisted mucosal inverted closure (TAMIC) technique for large perforations after gastric endoscopic full-thickness resection (EFTR) in a porcine model. The TAMIC technique was successfully performed in all 12 pigs, with complete healing achieved in 11 pigs. Histologic analyses showed appropriate connection of the epithelium and muscularis mucosae layers under inverted healing.

Summary: This study presents the authors' experience in managing iatrogenic colonic perforations through endoscopic closure. Among 964 advanced endoscopic resections, 22 cases (2.3%) experienced colonic perforation, with 18 cases treated with endoscopic clipping. No complications were observed postoperatively, and there was no recurrence during the follow-up period.

Summary: This study found that among EOC patients aged 65 years or older, those treated with CRS/HIPEC had comparable outcomes to patients under 65 years old, while there were slight differences in survival for those treated with NACT + CRS/HIPEC and salvage CRS/HIPEC. Age and feasibility of complete cytoreduction should be considered when selecting treatment methods for elderly patients.

Summary: CRS/HIPEC in a non-academic setting is a safe option for treating PSM with favorable surgical and oncological outcomes, despite the presence of major complications and low mortality rates. Incomplete cytoreduction and major complication rates decrease over time, while mortality remains low and constant.

Results showed that DR did not increase morbidity and mortality, but was associated with worse survival in patients with appendiceal and colorectal tumors, particularly after adjusting for tumor burden.

Summary: This study analyzed the recurrence of mucinous carcinomatosis peritonei (MCP) after the first cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) for appendiceal tumors (AT) and compared the outcomes between patients who underwent iterative CRS/HIPEC and those who did not. The study found that selected low- and high-grade appendiceal cancer patients with MCP recurrence who were able to undergo iterative CRS/HIPEC procedures showed favorable outcomes. However, there was no evidence supporting a survival benefit with iterative CRS/HIPEC in recurrent MCP with signet ring cell morphology.

Summary: This study evaluated the association between adjuvant chemotherapy (ACT) and survival in stage IVA/B high-grade mucinous appendiceal cancer patients treated with CRS/HIPEC. The results showed that ACT was not associated with better overall survival (OS) compared to no chemotherapy (NoCT) in this cohort. The findings may be attributed to differences in tumor biology or the small sample size.
