Bartonella and Coxiella infections presenting as systemic vasculitis: case series and review of literature

Through a case series and literature review, it was found that Bartonella infection can mimic PR3-ANCA-associated vasculitis with involvement of the kidneys and endocarditis, while Coxiella infection may induce vasculitis affecting vessels of all sizes, with 75% presenting as cryoglobulinaemia vasculitis.
Objectives Coxiella and Bartonella spp. display particular tropism for endothelial or endocardial tissues and an abnormal host response to infections with induced autoimmunity. We aimed, through a case series combined with a comprehensive literature review, to outline characteristics of Coxiella and Bartonella infections presenting as systemic vasculitis. Methods We retrospectively included cases of definite Coxiella and Bartonella infections presenting with vasculitis features and performed a comprehensive literature review. Results Six cases of Bartonella infections were added to 18 cases from literature review. Causative pathogens were mainly B. henselae. Bartonella infection mimicked ANCA-associated vasculitis in 83% with PR3-ANCA and presented as cryoglobulinaemic vasculitis in 8%. GN was present in 92%, and 88% had endocarditis. Complement fractions were low in 82% and rheumatoid factor positive in 85%. Kidney biopsies showed cell proliferation, mostly crescentic, with pauci-immune GN in 29%. Outcome was favourable, with the use of antibiotics alone in one-third. Five cases of Coxiella infections were added to 16 from literature review. Sixteen had small-vessel vasculitides, mainly cryoglobulinaemia vasculitis in 75%. One patient had polyarteritis nodosa-like vasculitis and four large-vessel vasculitis. Outcome was good except for one death. A highly sensitive next generation sequencing analysis on three Coxiella- and two Bartonella-related vasculitides biopsies did not find any bacterial DNA. Conclusion Coxiella and Bartonella are both able to induce vasculitis but display distinct vasculitis features. Bartonella mimics PR3-ANCA-associated vasculitis in the setting of endocarditis, whereas Coxiella may induce vasculitis involving all vessel sizes.

Summary: This study investigated the phenotypic characteristics of patients with ANCA-associated vasculitis (AAV) associated with aneurysms. It found that aneurysms were more frequently observed in granulomatosis with polyangiitis (GPA) patients and predominantly affected medium-sized vessels, especially the renal arteries. Aneurysmal rupture occurred in 43% of patients, with those experiencing rupture having higher baseline pulmonary infiltrates and creatinine levels compared to those without rupture.

Clinico-radiological correlation and prognostic value of baseline chest computed tomography in eosinophilic granulomatosis with polyangiitis

Summary: The objective of this study is to investigate the prognostic value of baseline HRCT in EGPA patients. Abnormal parenchymal findings were observed in a high proportion of patients. Patients were divided into three groups based on HRCT characteristics, with group 1 having the worst prognosis and group 2 having the best prognosis. It is concluded that chest HRCT at diagnosis of EGPA may have prognostic value and aid in the management of these patients.

Summary: In this study, we report the first emergence of dual resistance to levofloxacin and rifampin in C. avidum in vivo, in a patient who received both antibiotics orally during salvage debridement and implant retention of an ODRI. This study highlights potential limitations of the combination of oral rifampin and levofloxacin in patients undergoing these surgical procedures and the potential need to evaluate specific optimal therapy for emerging ODRI pathogens.

Impact of patient ancestry on heterogeneity of Sjogren's disease

Summary: This study compared disease characteristics between primary Sjogren's syndrome (pSS) patients of African ancestry and Caucasian ancestry. The results showed that patients of African ancestry had younger age at diagnosis, higher titre of gammaglobulins, higher rates of positive anti-SSA and anti-RNP antibodies, and more frequent systemic complications. Factors associated with disease activity were sub-Saharan African ancestry, rheumatoid factor, and anti-RNP positivity. In conclusion, patients of African ancestry displayed higher disease activity with increased B-cell activation.

Summary: This retrospective study investigated the use of benralizumab in the treatment of eosinophilic granulomatosis with polyangiitis (EGPA), and found that benralizumab is effective for refractory asthma and ENT manifestations, and reduces the use of glucocorticoids. However, its efficacy is lower in patients who have previously failed mepolizumab treatment.

Summary: Epidemiological studies have shown a link between certain infections and an increased risk of developing psychiatric disorders. These infections can activate retro-transposable genetic elements (HERV), which in turn impact immune receptors and synaptic plasticity. Recent research has found an association between the HERV-W ENV protein and pro-inflammatory cytokines in patients with schizophrenia or bipolar disorder. The COVID-19 pandemic's influence on patients with psychosis spectrum disorders (PSD) was investigated, revealing a high prevalence of SARS-CoV-2 antibodies in PSD patients, with HERV-W ENV detected only in seropositive individuals. SARS-CoV-2 infection and HERV-W ENV positivity were found to co-cluster with high levels of pro-inflammatory cytokines in psychotic patients. These findings suggest a dominant influence of the virus on HERV-W ENV and cytokine expression, and the need for further investigation into the interplay between viral infection and the clinical evolution of PSD patients.
