Summary: In this study, it was discovered that human T cells express GSDME, which is associated with durable viability and the release of IL-1 alpha. This property is only present in a subset of human helper type 17 T cells with specificity for Candida albicans, regulated by the T cell-intrinsic NLRP3 inflammasome. The release of IL-1 alpha is mediated by a proteolytic cascade involving caspase-8, caspase-3, and GSDME cleavage, as well as T cell receptor stimulation and calcium-licensed calpain maturation of pro-IL-1 alpha form. This finding expands our understanding of the functional diversity and mechanism of T cells and has implications for antifungal immunity.

Epigenetic changes in inflammatory arthritis monocytes contribute to disease and can be targeted by JAK inhibition

Summary: This study investigates the epigenetic changes in monocytes derived from inflamed joints of JIA patients and reveals the role of the local inflammatory environment in regulating these changes. The activation phenotype of synovial-derived monocytes is found to be regulated on the epigenetic level, with increased expression and epigenetic alterations in IFN signaling-associated genes. Treatment with the JAK inhibitor ruxolitinib transforms the activated enhancer landscape and reduces disease-associated gene expression, thus inhibiting the inflammatory phenotype.

Summary: This study indicates that human dendritic cells (DCs) have specific functional programming in chronic inflammatory conditions. Compared to other cell types, cDC1 remains relatively quiescent and unchanged at the site of inflammation, while cDC2 and monocytes show stronger inflammatory features and T cell activation ability.

The clinical and experimental treatment of Juvenile Idiopathic Arthritis

Summary: This review discusses the unresolved clinical challenges in juvenile idiopathic arthritis (JIA) and explores how an increased understanding of the disease's biology can lead to future treatment strategies at various stages of the disease. The most relevant disease subtypes, nonsystemic (OA and PA) JIA and systemic (SJ) JIA, are examined, along with their proposed mechanisms and current therapeutic modalities. Current treatment strategies in JIA, such as the "Step-up" and "Treat to Target" approaches, are discussed, and the potential for more targeted strategies in the future is explored.

Summary: The care for JIA patients has changed in the biologics era, however, biologics have important risks and are expensive. Flares after biologic withdrawal are common, but there is little clinical guidance on which patients can safely discontinue their biologics. We conducted a survey to identify the characteristics that are important to pediatric rheumatologists when considering withdrawal of biologics.

Summary: Over 1,000 cases of unexplained paediatric hepatitis have been reported worldwide, with 278 cases in the UK. A study investigated 38 cases and found high levels of adeno-associated virus 2 (AAV2) in the liver, blood, plasma or stool of 27 out of 28 cases. The study suggests that abnormal AAV2 replication aided by other viruses may trigger immune-mediated liver disease in predisposed children.

Summary: This study investigated the immunogenicity of the meningococcal serogroup ACWY conjugate vaccine in minors with juvenile idiopathic arthritis (JIA) and inflammatory bowel disease (IBD). The results showed that the concentrations of antibodies for serotypes A and W were lower in patients compared to healthy controls, and patients using anti-TNF therapy had lower immune response. Therefore, an extra booster vaccination should be considered for these patients.

The protective effect of COVID-19 vaccines on developing multisystem inflammatory syndrome in children (MIS-C): a systematic literature review and meta-analysis

Summary: This study conducted a systematic review and meta-analysis to investigate the effectiveness of current COVID-19 vaccines in preventing multisystem inflammatory syndrome in children (MIS-C) and adolescents. The findings suggest that mRNA COVID-19 vaccines in children aged 12-18 years are safe and significantly reduce the development of MIS-C. The risk of MIS-C post-vaccination is much lower compared to the risk of MIS-C post-infection.

Withdrawing biologics in non-systemic JIA: what matters to pediatric rheumatologists?

Summary: This study surveyed pediatric rheumatologists in Canada and the Netherlands to explore their views on withdrawing biologic therapy in children with clinically inactive non-systemic JIA and when they decide to postpone the withdrawal. The results showed that the strongest driver of the decision to postpone biologic therapy withdrawal was the preference of the child and/or parents, followed by flare in the current treatment period and uveitis in the current treatment period. On average, biologic therapy withdrawal was initiated 6.7 months later when the child or parent preferred to continue treatment.

Summary: This study aimed to explore the management experience of youth with Juvenile Idiopathic Arthritis (JIA) and their parents. Through semi-structured interviews with youth receiving biological medication for JIA and their parents, four themes were identified: aspects of life affected by JIA and its management, lived experience with JIA management, medication decision-making, and involvement in decision-making. The findings highlight the importance of person- and family-centred care in pediatric rheumatology.
