This article gathers 9 thought leaders and their team members to discuss their ideas about the future of in vitro fertilization and the andrology laboratory. Despite the progress and innovation in the laboratory over the years, there is still important work to be done in technology and processes for future development.
The aim of this article is to gather 9 thought leaders and their team members to present their ideas about the future of in vitro fertilization and the andrology laboratory. Although we have seen much progress and innovation in the laboratory over the years, there is still much to come, and this article looks at what these leaders think will be important in the future development of technology and processes in the laboratory. Summary: This study aimed to explore the impact of different gonadotrophin selection for ovarian stimulation on oocyte competence. The results showed that the choice of recombinant FSH or human menopausal gonadotrophin for ovarian stimulation was associated with the euploid blastocyst rate. However, there was no association between the type of gonadotrophin used and live birth rate or blastocyst quality.

Summary: Human embryos are often affected by maternally inherited aneuploidies, leading to developmental failure at early stages. Recent evidence shows that aberrant patterns at the cellular or molecular level can impact multiple stages of embryo development. Fertilization is a delicate phase where centrosomes are assembled and nuclei undergo crucial changes. Errors in this process can result in cellular or genomic integrity loss and fatal consequences for embryonic development.

These indicators can be used to assess the quality of each center and improve the efficacy and efficiency of IVF.

Summary: During pre-implantation stages, stored material in mammalian development promotes epigenetic profile erasure and genome activation. Through single-cell methylome and transcriptome sequencing, we analyzed the expression and methylation in oocytes and human embryos, identifying key events in epigenetic reprogramming and genome activation. Failure to achieve these milestones negatively impacts the developmental potential of embryos and may affect the success of assisted reproductive cycles.

Successful cryptozoospermia management with multiple semen specimen collection

Summary: Providing fresh ejaculated semen samples allows approximately 10% of men with azoospermia to avoid surgery and achieve satisfactory intracytoplasmic sperm injection (ICSI) outcomes. Additionally, providing multiple ejaculated samples over a short period of time does not have a negative impact on sperm concentration and may enhance sperm motility in men with cryptozoospermia.

Sperm centriolar factors and genetic defects that can predict pregnancy

Summary: The sperm centrosome, an important organelle in fertilization and embryonic development, plays a crucial role in mitotic spindle formation, chromosome segregation, and syngamy. Abnormalities of the sperm centrosome can lead to abnormal embryonic development, pregnancy loss, and chromosomal instability. Understanding the molecular composition, regulation, and function of the sperm centrosome is vital for elucidating the mechanisms of successful fertilization and embryonic development, as well as addressing infertility and developmental disorders associated with centrosomal defects.

Summary: Pregnancy is established during the periconceptional period from blastocyst attachment to placenta formation, significantly impacting the health of both the child and the mother. Advances in understanding the periconceptional space, including the preimplantation human embryo and maternal endometrium, offer opportunities for preventative interventions. This review discusses the role of maternal decidua, the maternal-embryonic interface, the endometrial microbiome, and the myometrium in pregnancy health during the periconceptional period.
