Summary: Home parenteral nutrition (HPN) may improve survival in patients with malignant bowel obstruction, but certain clinical and laboratory markers need to be considered to accurately select patients who would benefit from HPN. Discontinuation of chemotherapy, anemia, severe hypoalbuminemia, and water retention may be correlated with shorter survival and in these cases, routine initiation of HPN may not be beneficial. The decision to initiate HPN should be made in conjunction with continuing or beginning chemotherapy.

Summary: Maintaining adequate nutrition is a challenge for small bowel neuroendocrine tumour (NETs) patients. Surgical resection may cause short bowel syndrome (SBS), while without surgical resection there is a risk of ischemia or inoperable malignant bowel obstruction (IMBO). This systematic review shows the feasibility of HPN in NETs patients with intestinal failure (IF), with a reasonable 1-year survival rate and low complication rate.

Summary: A study of 66 pediatric CIPO patients revealed a higher proportion of early-onset CIPO and patients with urologic symptoms, as well as a majority of cases with generalized involvement. Factors such as duration of PN, PN use, and home management were found to impact outcomes, while mortality was associated with pathologic type.

Summary: ESPEN has published guidelines for Chronic Intestinal Failure (CIF) in adults and has now updated the guidelines specifically for CIF caused by benign diseases. The new guidelines include new evidence and provide recommendations on various topics such as IF centers, chronic enterocutaneous fistulas, caring for CIF patients during pregnancy, and transition of patients from pediatric to adult centers.

Summary: This study investigated the differences in outcomes after weaning off intravenous support in adult patients with chronic intestinal failure. Nutritional and functional markers were assessed, and it was found that malnutrition and reduced muscle mass were common in these patients. Treatment with teduglutide was effective in preventing these complications.

Summary: In France, the number of children enrolled in a HPN program continuously increased over a 6 years period. Short bowel syndrome (SBS) is the leading cause of CIF requiring HPN. The rate of catheter-related bloodstream infections (CRBSIs) dropped dramatically as the use of Taurolidine lock procedure (TLP) increased. Mortality rate was low and mainly in relation to the underlying disease (cancer, immune deficiency). Cholestasis and intestinal transplantation remained very rare.

Summary: This study compares outcomes and surgical decision-making in patients with short bowel syndrome (SBS) and fistulas. The authors found that patients who underwent reconstructive surgery had higher rates of nutritional autonomy, while older patients with higher comorbidity index were less likely to achieve cessation of home parenteral nutrition (HPN). There were no differences in long-term nutritional and survival outcomes or surgical decisions between patients with SBS and fistulas, or between enterocutaneous and enteroatmospheric fistulas.

Summary: This study reported the experiences of an intestinal rehabilitation unit (IRU) from a country without home parenteral nutrition (HPN). Among 349 patients admitted, 100 were diagnosed with intestinal failure (IF). The most common causes of IF were enterocutaneous fistulas, short-bowel syndrome, and chronic intestinal pseudo-obstruction.

Summary: Short bowel syndrome (SBS) is a rare gastrointestinal condition caused by extensive surgical resection, resulting in less than 200 cm of remaining small intestine. This condition is associated with a high risk for intestinal failure (IF) and requires parenteral support (PS). This review focuses on the role of the colon in SBS-IF, including intestinal adaptation, energy salvage, and the impact of the microbiota. Additionally, the effects of glucagon-like peptide-2 (GLP-2) analogs, a disease-modifying treatment, in SBS-IF patients with and without a colon are discussed. Overall, this review emphasizes the importance of preserving the colon in SBS-IF.

Summary: This survey assesses current attitudes and practices in managing acute mesenteric ischemia (AMI) worldwide. Results show that diagnosis of AMI is often delayed but rarely missed. Abdominopelvic CT-scan with intravenous contrast was suggested as the most useful diagnostic test. Treatment approaches vary greatly, indicating the need for updated guidelines.

Summary: Short bowel syndrome (SBS) is a rare gastrointestinal disorder associated with intestinal failure (SBS-IF) and poor health-related outcomes. Patients with SBS-IF require long-term intravenous supplementation (IVS) and the goal of treatment is to reduce or eliminate the need for IVS. Teduglutide, a glucagon-like peptide 2 analog, has been shown to be effective in reducing IVS dependence and improving the quality of life for SBS-IF patients. The management of SBS-IF patients is complex and requires close monitoring.

Summary: Short bowel syndrome (SBS) is a clinical condition characterized by malabsorption, diarrhea, fatty stools, malnutrition, and dehydration due to a small bowel length of less than 200 cm from the ligament of Treitz. SBS is the main cause of chronic intestinal failure (CIF), which requires intravenous supplementation (IVS) to maintain the absorption of nutrients and water. SBS can be classified anatomically, evolutionally, pathophysiologically, clinically, and by the severity of CIF. Proper patient categorization is crucial for effective communication in clinical practice and research.

Summary: This study investigated the incidence and severity of COVID-19 infection in patients receiving home parenteral nutrition (HPN) for chronic intestinal failure (CIF) from a European database. The results showed a significant variation in infection rates among countries, with the majority of cases being asymptomatic or mild but a significant proportion of infected patients experiencing severe or fatal outcomes. Lack of vaccination was associated with a higher risk of death.
