This practical guideline, based on the ESPEN Guidelines, offers simplified recommendations for the management and treatment of patients with chronic intestinal failure, providing guidance and support to healthcare professionals.
Background: This practical guideline is based on the ESPEN Guidelines on Chronic Intestinal Failure in Adults. Methodology: ESPEN guidelines have been shortened and transformed into flow charts for easier use in clinical practice. The practical guideline is dedicated to all professionals including physicians, dieticians, nutritionists, and nurses working with patients with chronic intestinal failure. Results: This practical guideline consists of 112 recommendations with short commentaries for the management and treatment of benign chronic intestinal failure, including home parenteral nutrition and its complications, intestinal rehabilitation, and intestinal transplantation. Conclusion: This practical guideline gives guidance to health care providers involved in the management of patients with chronic intestinal failure. It is based on previous guidelines and includes 71 recommendations covering various aspects of HPN. The evidence was evaluated and clinical recommendations were developed using the Scottish Intercollegiate Guidelines Network methodology. The guideline was commissioned and supported by ESPEN.

The Role of a Colon-in-Continuity in Short Bowel Syndrome

Summary: Short bowel syndrome (SBS) is a rare gastrointestinal condition caused by extensive surgical resection, resulting in less than 200 cm of remaining small intestine. This condition is associated with a high risk for intestinal failure (IF) and requires parenteral support (PS). This review focuses on the role of the colon in SBS-IF, including intestinal adaptation, energy salvage, and the impact of the microbiota. Additionally, the effects of glucagon-like peptide-2 (GLP-2) analogs, a disease-modifying treatment, in SBS-IF patients with and without a colon are discussed. Overall, this review emphasizes the importance of preserving the colon in SBS-IF.

Use of teduglutide in adults with short bowel syndrome-associated intestinal failure

Summary: Short bowel syndrome (SBS) is a rare gastrointestinal disorder associated with intestinal failure (SBS-IF) and poor health-related outcomes. Patients with SBS-IF require long-term intravenous supplementation (IVS) and the goal of treatment is to reduce or eliminate the need for IVS. Teduglutide, a glucagon-like peptide 2 analog, has been shown to be effective in reducing IVS dependence and improving the quality of life for SBS-IF patients. The management of SBS-IF patients is complex and requires close monitoring.

Definition, classification, and causes of short bowel syndrome

Summary: Short bowel syndrome (SBS) is a clinical condition characterized by malabsorption, diarrhea, fatty stools, malnutrition, and dehydration due to a small bowel length of less than 200 cm from the ligament of Treitz. SBS is the main cause of chronic intestinal failure (CIF), which requires intravenous supplementation (IVS) to maintain the absorption of nutrients and water. SBS can be classified anatomically, evolutionally, pathophysiologically, clinically, and by the severity of CIF. Proper patient categorization is crucial for effective communication in clinical practice and research.

Summary: This study investigated the incidence and severity of COVID-19 infection in patients receiving home parenteral nutrition (HPN) for chronic intestinal failure (CIF) from a European database. The results showed a significant variation in infection rates among countries, with the majority of cases being asymptomatic or mild but a significant proportion of infected patients experiencing severe or fatal outcomes. Lack of vaccination was associated with a higher risk of death.

Summary: This study aimed to evaluate the association between metabolic dysfunction-associated steatotic liver disease (MASLD) and eating disorders. The results showed that binge eating disorder (BED) was present in approximately 17.8% of MASLD patients screened in a specialized metabolic disorders unit. This study highlights the importance of addressing eating disorders in the treatment of MASLD.
