Expanding the variability of the ADPKD-GANAB clinical phenotype in a family of Italian ancestry

Summary: Causative mutations in the GANAB gene have been described in only 14 families, with 9 diagnosed with late-onset ADPKD and 5 with ADPLD. A new family with late-onset ADPKD due to the p.R839W GANAB variant was reported, previously reported in a severe ADPLD patient requiring liver transplantation.

Summary: Mepolizumab at both 100 mg every 4 weeks and 300 mg every 4 weeks proved effective in treating EGPA, with no significant difference between the two doses. Complete response rates reached 30.4% at 12 months and 35.7% at 24 months, with some patients experiencing exacerbations in asthma and ENT.

Therapies for Membranous Nephropathy: A Tale From the Old and New Millennia

Summary: Primary Membranous Nephropathy (PMN) is the most common cause of nephrotic syndrome in adults and can lead to end-stage renal disease if left untreated. This review discusses the evidence for different immunosuppressive regimens used for PMN and provides recommendations based on current evidence. The association of PMN with HLA-DQA1 and the presence of autoantibodies support the autoimmune nature of the disease.

Summary: This study aims to assess factors associated with hypogammaglobulinemia in AAV patients treated with rituximab, and the results showed that hypogammaglobulinemia is common in AAV patients treated with rituximab, and lower IgG levels are associated with severe infections. Age and higher glucocorticoid doses increase the risk of hypogammaglobulinemia in these patients.

Summary: In this study, a new in situ proteomic technique called matrix-assisted laser desorption/ionisation mass spectrometry imaging (MALDI-MSI) was investigated as an alternative to conventional laser capture microdissection MS for the characterisation of amyloids. The results showed that MALDI-MSI correctly identified and typed known amyloid types and identified amyloidosis in challenging cases.

Summary: In recent years, there has been significant progress in clinical research and treatments for ANCA-associated vasculitis. However, there are still unanswered questions and unmet needs regarding AAV patients, such as predicting relapse and improving quality of life. Identifying predictors of relapse can optimize therapies and reduce side effects, while recognizing the long-term consequences of the disease and its treatment is crucial. This review aims to highlight achievements, challenges, and the perspectives of physicians and patients.

The place of cyclical therapy for the treatment of membranous nephropathy in the era of rituximab

Summary: Primary membranous nephropathy (MN) is the most common cause of nephrotic syndrome in adults and is often caused by autoantibodies against PLA2R. Immunosuppression therapy, such as cyclical therapy, is effective in inducing remission and reducing the risk of end-stage renal disease. Rituximab has emerged as a key player in MN management, with comparable effectiveness and likely better safety compared to the cyclical regimen. However, there are still uncertainties, especially for patients resistant to Rituximab or with severe disease. This article aims to illustrate the role of cyclical therapy in the era of Rituximab.

Summary: There is still uncertainty about the optimal dosing of rituximab in AAV patients. This study found that a subset of patients experienced sustained B-cell depletion after rituximab treatment, which was associated with reduced risk of flare and increased risk of serious infection in the long term. Preserved renal function and female sex were associated with faster B-cell repopulation.
