Impact of Seasonal Coronavirus Antibodies on Severe Acute Respiratory Syndrome Coronavirus 2 Vaccine Responses in Solid Organ Transplant Recipients

Andrew H. Karaba, Weiqiang Zhou, Shuai Li, Tihitina Y. Aytenfisu, Trevor S. Johnston, Olivia Akinde, Yolanda Eby, Aura T. Abedon, Jennifer L. Alejo, Caroline X. Qin, Elizabeth A. Thompson, Jacqueline M. Garonzik-Wang, Joel N. Blankson, Andrea L. Cox, Justin R. Bailey, Sabra L. Klein, Andrew Pekosz, Dorry L. Segev, Aaron A. R. Tobian, William A. Werbel

Summary: Antibody response to SARS-CoV-2 vaccination is reduced in solid organ transplant recipients (SOTRs), possibly due to increased levels of preexisting antibodies to seasonal coronaviruses, suggesting the influence of antigenic imprinting on vaccine responses in SOTRs.

PASC in Solid Organ Transplant Recipients With Self-reported SARS-CoV-2 Infection

Sami Alasfar, Teresa Po-Yu Chiang, Andrew J. Snyder, Michael T. Ou, Brian J. Boyarsky, Aura T. Abedon, Jennifer L. Alejo, Sydney Cook, Willa Cochran, Emily Brigham, Ann M. Parker, Jacqueline Garonzik-Wang, Allan B. Massie, Daniel C. Brennan, Tracy Vannorsdall, Dorry L. Segev, Robin K. Avery

Summary: This study aimed to estimate the frequency of postacute sequelae of SARS-CoV-2 infection (PASC) symptoms in solid organ transplant (SOT) recipients and compared their frequency between those with hospitalized and non-hospitalized cases. The results showed a high frequency of PASC symptoms in SOT recipients, indicating the need for multidisciplinary care beyond the acute phase.

Neutralizing activity and 3-month durability of tixagevimab and cilgavimab prophylaxis against Omicron sublineages in transplant recipients

Abedon, Yolanda Eby, Trevor Scott Johnston, Maggie Li, Tihitina Ayten, Casey Hussey, Alexa Jefferis, Nicole Fortune, Rivka Abedon, Letitia Thomas, Feben Habtehyimer, Jessica Ruff, Daniel S. Summary: Neutralizing antibody (nAb) responses are weakened in solid organ transplant recipients (SOTRs) despite being fully vaccinated. Preexposure prophylaxis (PrEP) with the antibody combination tixagevimab and cilgavimab (T+C) may enhance immune protection, but its activity and durability against Omicron sublineages BA.4/5 in fully vaccinated SOTRs have not been determined. Most fully vaccinated SOTRs receiving T+C PrEP achieved neutralization against BA.4/5, but nAb activity commonly declined after 3 months. It is crucial to evaluate the optimal dose and interval of T+C PrEP to maximize protection against emerging variants.

Patient-reported outcomes after Tixagevimab and Cilgavimab pre-exposure prophylaxis among solid organ transplant recipients: Safety, effectiveness, and perceptions of risk

Summary: This study investigated the patient-reported outcomes of solid organ transplant recipients receiving T+C for COVID-19 prophylaxis. It found that T+C was well tolerated with few serious events. It also revealed changes in risk tolerance and reported breakthrough COVID-19 cases among recipients. These results contribute to counseling SOTRs on the real-world safety and effectiveness of T+C.

Coronavirus Disease 2019-Associated Pulmonary Aspergillosis: A Noninvasive Screening Model for Additional Diagnostics

Nitipong Permpalung, Teresa Po-Yu Chiang, Robin K. Avery, Darin Ostrander, Kausik Datta, Dorry L. Segev, Christine M. Durand, Sean X. Zhang, Allan B. Massie, Kieren A. Marr

Summary: A retrospective study was conducted to investigate COVID-19-associated pulmonary aspergillosis (CAPA), which is likely underdiagnosed and lacks sensitive diagnostic tools. A CAPA prediction model was built using multivariable logistic regression analysis, and the performance of the model was assessed using the area under the receiver operating characteristic curve (AUC). The model identified several factors, such as age, intubation time, dexamethasone treatment, and underlying diseases, that were significantly associated with CAPA, resulting in an AUC of 0.75. Screening at a cutoff of =0.085, the model showed a sensitivity of 82%, specificity of 51%, positive predictive value of 18.6%, and negative predictive value of 95.3%.

Heterologous versus homologous boosting elicits qualitatively distinct, BA.5-cross-reactive T cells in transplant recipients

Summary: The SARS-CoV-2 Omicron BA.5 subvariant evades vaccination-induced neutralizing antibodies due to mutations in the spike protein. Solid organ transplant recipients (SOTRs) have poor recognition of the Omicron variant after COVID-19 vaccination. T cell responses are important for defense against the virus.
