Listing at non-local transplant centers is associated with increased access to deceased donor kidney transplantation

Katherine Ross-Driscoll, Jonathan Gunasti, Raymond J. Lynch, Allan Massie, Dorry L. Segev, Jon Snyder, David Axelrod, Rachel E. Patzer

Summary: The ability of kidney transplant candidates to travel outside of their usual place of care is influenced by sociodemographic factors, which may worsen access disparities. A certain proportion of candidates choose to receive treatment in neighboring or areas beyond neighboring regions, which is associated with their demographic characteristics. Seeking medical care in a neighbor region increases the likelihood of receiving a deceased donor transplant, while seeking care beyond a neighbor region further increases the possibility. Travelers and non-travelers have similar rates of living donor transplant and waitlist mortality, but patients who travel beyond a neighbor region have slightly lower posttransplant mortality.

Quantifying excess deaths among solid organ transplant recipients in the COVID-19 era

Allan B. Massie, William A. Werbel, Robin K. Avery, Teresa Po-Yu Chiang, Jon J. Snyder, Dorry L. Segev

Summary: Estimating the mortality burden of COVID-19 on solid organ transplant recipients is crucial for understanding the impact of the disease. A study found that COVID-19 likely caused over 5000 excess deaths among SOTRs, representing 1 in 75 SOTRs and a substantial proportion of all deaths among SOTRs during the study period.

Subsets of Idiopathic Inflammatory Myositis Enriched for Contemporaneous Cancer Relative to the General Population

Christopher A. Mecoli, Tak Igusa, Mengkun Chen, XingYao Wang, Jemima Albayda, Julie J. Paik, Eleni Tiniakou, Brittany Adler, Carrie Richardson, Will Kelly, Sonye Danoff, Andrew L. Mammen, Elizabeth A. Platz, Antony Rosen, Lisa Christopher-Stine, Livia Casciola-Rosen, Ami A. Shah

Summary: This study investigated the cancer risk in patients with idiopathic inflammatory myopathy (IIM) compared to the general population. The results showed that IIM patients had a 1.43-fold increased risk of developing cancer. Specific autoantibodies, such as anti-transcription intermediary factor 1 gamma (anti-TIF1 gamma), were associated with an increased risk of ovarian and breast cancer.

Patient-reported outcomes after Tixagevimab and Cilgavimab pre-exposure prophylaxis among solid organ transplant recipients: Safety, effectiveness, and perceptions of risk

Summary: This study investigated the patient-reported outcomes of solid organ transplant recipients receiving T+C for COVID-19 prophylaxis. It found that T+C was well tolerated with few serious events. It also revealed changes in risk tolerance and reported breakthrough COVID-19 cases among recipients. These results contribute to counseling SOTRs on the real-world safety and effectiveness of T+C.

Cognitive Dysfunction in Liver Disease and Its Implications for Transplant Candidates

Jessica M. Ruck, Elizabeth A. King, Nadia M. M. Chu, Dorry L. Segev, Mara McAdams-DeMarco

Summary: Recent studies have shown that besides hepatic encephalopathy, many liver diseases have cognitive impairments of varying reversibilities. It is important to conduct further research to determine the optimal cognitive assessment tools, identify high-risk patients, determine reversible elements of cognitive impairment, and find effective treatments for patients with liver disease.

Post-kidney transplant body mass index trajectories are associated with graft loss and mortality

Yi Liu, Victoria A. Bendersky, Xiaomeng Chen, Nidhi Ghildayal, Meera N. Harhay, Dorry L. Segev, Mara McAdams-DeMarco

Summary: Post-kidney transplantation BMI changes are associated with graft loss and mortality. BMI increases in the first 3 years post-KT and then decreases in years 3-5. BMI gain in individuals with obesity is linked to increased risks of all-cause mortality and graft loss, while BMI gain in non-obese individuals is associated with decreased graft loss risks.

Summary: This study evaluated the diagnostic yield of computed tomography (CT) imaging for cancer screening/surveillance in patients with idiopathic inflammatory myopathy (IIM). The results showed that dermatomyositis patients had the highest diagnostic yields on CT scans of the chest and abdomen/pelvis, while patients under 40 years old had lower diagnostic yields.

Coronavirus Disease 2019-Associated Pulmonary Aspergillosis: A Noninvasive Screening Model for Additional Diagnostics

Nitipong Permpalung, Teresa Po-Yu Chiang, Robin K. Avery, Darin Ostrander, Kausik Datta, Dorry L. Segev, Christine M. Durand, Sean X. Zhang, Allan B. Massie, Kieren A. Marr

Summary: A retrospective study was conducted to investigate COVID-19-associated pulmonary aspergillosis (CAPA), which is likely underdiagnosed and lacks sensitive diagnostic tools. A CAPA prediction model was built using multivariable logistic regression analysis, and the performance of the model was assessed using the area under the receiver operating characteristic curve (AUC). The model identified several factors, such as age, intubation time, dexamethasone treatment, and underlying diseases, that were significantly associated with CAPA, resulting in an AUC of 0.75. Screening at a cutoff of =0.085, the model showed a sensitivity of 82%, specificity of 51%, positive predictive value of 18.6%, and negative predictive value of 95.3%.
