Summary: This review provides an updated assessment of the crucial role played by phosphodiesterase 5 (PDE5) inhibitors in the nitric oxide/cGMP pathway and their potential in treating various diseases. It discusses the medicinal chemistry and drug discovery aspects of PDE5 inhibitors, as well as the breakthrough development of allosteric inhibitors. Furthermore, the concept of multi-targeted ligands in relation to PDE5 inhibitors is critically evaluated.

Summary: Erectile dysfunction (ED) is a common condition where men are unable to achieve or maintain a satisfactory erection for sexual performance. There are various treatment options available, with phosphodiesterase 5 (PDE5) inhibitors being the first-line treatment. This review examines the safety, efficacy, and pharmacokinetic properties of new ED drugs such as udenafil, mirodenafil, youkenafil, lodenafil, and SLx-2101.

Summary: This study aimed to determine the effect of phosphodiesterase type 5 inhibitors (PDE-5is) on major adverse cardiovascular events (MACE) and overall mortality. The results showed that men who received PDE-5is treatment had a 13% lower incidence of MACE and a 25% lower overall mortality. This suggests that PDE-5is may have cardioprotective effects.

Summary: This study investigated the impact of familial occurrence of CAD on the presence of ED and classic risk factors for ED in men with CAD. The findings showed that while familial CAD increased the incidence of certain risk factors for ED, it did not increase the incidence of ED itself.

Summary: Low-intensity shockwave therapy (LiSWT) is gaining popularity for the treatment of erectile dysfunction (ED). Current studies show that LiSWT has low risk and positive improvements in erectile function. However, there is still debate over its clinical utility and optimal treatment protocol.

Summary: This study evaluated the impact of phosphodiesterase-5 inhibitors (PDE5-Is) on cardiovascular disease (CVD) and mortality risk in patients with diabetes mellitus (DM) and erectile dysfunction (ED). The results showed that ED was associated with an increased risk of composite CVD/MACE, all-cause mortality, coronary heart disease (CHD), and stroke in the general population. However, in the DM population, ED did not confer an excess risk of CVD and mortality compared to those without DM. Limited data from randomized controlled trials (RCTs) indicated no significant differences in the risk of major adverse cardiac events (MACE), CHD, and all-cause mortality between PDE5-I use and non-use in patients with ED and DM.

Summary: Through systematic literature search and network meta-analysis, it was found that on-demand use of Avanafil 200 mg may be the best treatment option among PDE5 inhibitors for erectile dysfunction following nerve-sparing radical prostatectomy (NSRP). Additional randomized controlled trials are needed to further validate this finding.

Summary: This paper reviewed 97 medicinal plants with PDE5 inhibitory effect, of which 77 were selected based on their traditional use as aphrodisiacs. Isoflavones and biflavones were identified as the major active constituents responsible for action, while Icariin and Icariside II were found to be the most effective inhibitors from natural sources. Traditional medicines containing extracts of Ginkgo biloba, Kaempferia parviflora, Clerodendrum colebrookianum, Eurycoma longifolia, and Vitis vinifera were also found to inhibit the PDE5 enzyme.

Summary: Research on new small molecules targeting PDEs, particularly PDE5, is a current and interesting topic. The designed aminothiazole compounds showed potential PDE5 inhibitory activity, with in vivo testing demonstrating similar effects to sildenafil without its known drawbacks, indicating they may be beneficial drug candidates for the treatment of erectile dysfunction.

Summary: The increased population of LVAD-supported patients and ongoing clinical challenges necessitate improvement in LVAD technology and innovative medical management. The use of post LVAD implant phosphodiesterase-5 inhibitors (PDE-5i) has shown a potential association with reduced thrombotic events and improved survival. However, further randomized control trials are needed to determine the efficacy and safety of PDE-5i in patients with centrifugal flow LVAD.

Summary: Although total testosterone levels increased over time after COVID-19, more than 50% of men who recovered from the disease still had circulating testosterone levels suggestive for a condition of hypogonadism at 7-month follow-up. In as many as 10% of cases, testosterone levels even further decreased. Of clinical relevance, the higher the burden of comorbid conditions at presentation, the lower the probability of testosterone levels recovery over time.

Summary: This narrative review aimed to investigate the role of diabetes in the pathophysiology, diagnosis, and treatment of Peyronie's disease (PD). Diabetes is one of the most common comorbidities in PD patients and is strongly associated with erectile dysfunction. Metabolic alterations typical of diabetes play a pivotal role in the development of PD and resistance to its medical treatment.

Summary: Obesity has a negative impact on sex hormone metabolism, leading to reduced testosterone levels and potentially affecting male fertility. This study conducted a meta-analysis of observational studies and found that overweight and obese men had lower sperm count and motility compared to normal weight men. The results suggest that increased body weight is a risk factor for male infertility.
