Prevalence of Obstructive Sleep Apnea Among Saudi Pilots

This study found that obstructive sleep apnea (OSA) is quite common among Saudi-based airline pilots and first officers, with 69% of the study population being diagnosed with OSA through home sleep testing. The majority had mild OSA (64%), while 5% had moderate and severe OSA (2.5% each). Additionally, one-third of the participants had insomnia, 33.3% had severe fatigue, 23.1% had excessive daytime sleepiness, 10.3% had moderate depression, and 25.6% had mild depression symptoms. These findings highlight the importance of screening for OSA in high-risk occupational groups like airline pilots and first officers, as well as the need to assess and treat secondary consequences like fatigue, depression, and insomnia early on.
Purpose: To evaluate the presence of obstructive sleep apnea (OSA), using home sleep testing in pilots and first officers in Saudi-based airlines. This will allow for proper diagnosis and management of OSA in that group which can help in minimizing any possible aviation accidents related to sleep disturbances secondary to OSA. Materials and Methods: This is a cross-sectional study conducted through Saudi-based aviation among pilots and first officers who were in service, active, and flying short- to medium-haul flights. Forty-one participants met the inclusion criteria. However, 39 out of 41 completed the study questionnaires (Berlin questionnaire, Epworth sleepiness scale, Pittsburgh sleep quality index (PSQI), Fatigue severity scale (FSS), Visual analog fatigue scale (VAFS), and Patient health questionnaire (PHQ9), along with home sleep testing. Results: Sixty-nine percent of the study population had OSA by home sleep testing. Majority of which had mild OSA (64%) and 5% had moderate and severe OSA (2.5% each). Results of different questionnaires showed one-third of the participants had insomnia, 33.3% of the participants had severe fatigue, excessive daytime sleepiness was found in 23.1% of the participants, and 10.3% had moderate depression, while 25.6% were classified as having mild depression symptoms. Conclusion: Sleep apnea is prevalent among the studied Saudi-based airline pilots and first officers. Screening for workers of this high-risk occupation needs to be considered. Fatigue, depression, and insomnia can be secondary consequences of sleep apnea and should be assessed and treated early.

Summary: Sleep and the circadian clock are interconnected and vital to human health and well-being. The suprachiasmatic nucleus controls the sleep-wake cycle and homeostatic activities. Melatonin, as a part of the endogenous circadian clock, plays a critical role in sleep regulation. Light acts as a signal for sleep/wake control by affecting melatonin secretion. Clock genes serve as molecular timekeepers for maintaining the 24-hour cycle. The significance of these processes has been officially recognized in modern sleep and behavioral disorder diagnostic manuals. Timing in the brain is crucial for optimal brain physiology.

Summary: This study aimed to determine the optimal positive airway pressure (PAP) requirement in the Saudi population and investigate the differences across different OSA severity groups. The results showed that male participants and those with severe OSA require higher PAP. Additionally, the diagnostic polysomnography measurements of OSA severity can be used to predict the optimal PAP requirement. These findings provide guidance for the treatment of OSA patients.

This review aimed to determine the prevalence of nomophobia symptoms in youth and young adults, including severity, country, culture, population, measurement tool, and year of data collection. The study found that approximately 20% of individuals showed mild symptoms, 50% showed moderate symptoms, and 20% showed severe symptoms of nomophobia. University students from non-Western cultures were most likely to suffer from severe symptoms. The findings have important implications for timely and effective therapeutic management.

Summary: The current development and applications of artificial intelligence have advantages and disadvantages in the digital age. Stakeholders, including all of us, need to assess the use of AI in the field of mental health. We must understand the trends, challenges, opportunities, and shortcomings of this new technology. Rules, guidelines, and policies will evolve as the field advances. To promote progress, mental health professionals must be prepared to face obstacles and leverage innovative technologies like AI. Therefore, a collaborative strategy involving various stakeholders is necessary for the development of AI in mental health.

Summary: This study assessed the sleep habits of pilots and copilots flying short-haul flights, and found that they are at risk of irregular sleep patterns, poor sleep efficiency, poor sleep quality, daytime sleepiness, and fatigue. Measures should be taken to minimize these risks.

Summary: This study examined the prevalence and characteristics of positional obstructive sleep apnoea (POSA) using different definitions. The results showed that POSA is common and its prevalence varies depending on the definition used. Low body mass index (BMI) was identified as a significant predictor of POSA.

Summary: In this study, we examined the immediate impact of a social media outage on nomophobia and associated symptoms using a longitudinal cohort design. The results showed that symptoms of nomophobia increased significantly during the social media outage, and baseline insomnia scores predicted a surge in the global scores of nomophobia symptoms.

The results showed that SomnoscreenTM plus was sensitive in identifying proximal airway obstructions but had limitations in detecting distal obstructions.
