Summary: Monitoring disease progression is crucial for determining treatment strategies in liver disease patients, especially those with reversible conditions. However, there is a lack of suitable tools for monitoring liver function, calling for the development of such tools, given the expected increase in these diseases in the future.

Summary: This study evaluated the prognostic performance of the ALBI score as an alternative to CTP classification for assessing liver function. The ALBI score showed independent predictive value for hepatotoxicity and outperformed the CTP score. Assessment of liver function is crucial for predicting treatment outcomes in hepatocellular carcinoma.

Summary: The impact of albumin-bilirubin (ALBI) score at post-hepatectomy hepatocellular carcinoma (HCC) recurrence on survival after recurrence (SAR) was investigated in this study. It was found that higher ALBI scores at recurrence were associated with poorer SAR. Factors such as ALBI scores at hepatectomy and post-hepatectomy liver failure (PHLF) & grade B were also found to be related to the ALBI score at recurrence.

Summary: This study investigated the association between preoperative albumin-bilirubin (ALBI) grades and the achievement of textbook outcomes (TO) in patients with hepatocellular carcinoma undergoing laparoscopic hepatectomy. The results showed that preoperative ALBI grade 2/3 was significantly associated with non-achievement of TO.

Summary: This study aimed to construct and validate a combined pre- and intra-operative nomogram based on the degrees of cirrhosis in predicting posthepatectomy liver failure (PHLF) in hepatocellular carcinoma (HCC) patients. The results showed that the combined pre- and intra-operative nomogram was more accurate in predicting PHLF compared to the pre-operative nomogram.

Summary: This study investigated the feasibility and performance of predicting hepatocellular carcinoma (HCC) using a combined albumin-bilirubin (ALBI) and fibrosis-4 (FIB-4)-based model in patients with compensated cirrhosis and chronic hepatitis B (CHB) receiving nucleos(t)ide analog therapy. The combination of ALBI and FIB-4, along with diabetes mellitus and alpha-fetoprotein (AFDA), was found to be independent risk factors for HCC. The combined ALBI and FIB-4-based prediction model stratified the risk of HCC and showed a higher area under the receiver operating characteristic curve compared to other prediction models.

Summary: This study examined the short-term and long-term outcomes of robotic liver resection (RLR) for hepatocellular carcinoma (HCC) in Western high-volume centers. The study found that RLR had shorter hospital stays and lower incidence of postoperative liver failure compared to open liver resection (OLR). Furthermore, there were no significant differences in tumor recurrence and overall survival rates between the two groups.

Summary: The severity of liver functional reserve is an important prognostic predictor in hepatocellular carcinoma (HCC). Among the five liver reserve models, the ALBI grade is the best surrogate marker to represent liver functional reserve in terms of outcome prediction. These models serve as feasible prognostic markers to indicate liver injury, specifically in HCC patients with renal insufficiency.

Summary: For patients with Child-Pugh B cirrhosis undergoing liver resection for HCC, laparoscopic surgery is associated with lower blood loss, lower morbidity, and shorter hospital stay compared to open surgery. However, the 5-year overall and disease-free survival rates do not significantly differ between the two approaches. Patients without preoperative portal hypertension and Child-Pugh B7 cirrhosis may benefit most from laparoscopic liver surgery.

Summary: Patients with hepatocellular carcinoma (HCC) who receive dialysis have decreased overall survival compared to those who do not. Factors associated with poor prognosis in dialysis patients include longer duration of dialysis, non-diabetes related dialysis, presence of ascites, and non-curative treatments. The ALBI grade is a feasible prognostic model for evaluating liver functional reserve, and the CLIP model is the most optimal cancer staging system for dialysis patients with HCC.

Summary: This study aimed to evaluate the prevalence of gastrointestinal symptoms and post-COVID-19 disorders of gut-brain interaction among hospitalized patients with SARS-CoV-2 infection. The results showed that patients with COVID-19 had higher rates of gastrointestinal symptoms on hospital admission and lower rates of constipation and hard stools at 12 months post-infection compared to the control group. Additionally, the rate of irritable bowel syndrome was significantly higher in patients with COVID-19 than in controls.

Summary: Splenomegaly is a common complication in myelofibrosis patients and can negatively impact outcomes of allogeneic hematopoietic cell transplantation (HCT). This Position Paper provides a shared position statement on the management of splenomegaly before HCT. The assessment, prevalence, and clinical significance of splenomegaly are discussed, along with the need for therapeutic intervention. Specific scenarios, such as splanchnic vein thrombosis and COVID-19, are also addressed.

Summary: The study examined the characteristics of patients with chronic hepatitis B virus in Italy and found that a significant proportion of patients were non-Italian natives, and there was a decline in HBeAg prevalence and stable anti-HDV prevalence. Cirrhosis was common in both cohorts, with a higher proportion in the PITER cohort, and variables such as age, sex, BMI, and anti-HDV positivity were associated with cirrhosis. The study also observed an increase in adherence to antiviral treatment recommendations over time. In conclusion, chronic hepatitis B virus infection in Italy appears to be under control, but HDV infection remains a health concern.

Summary: This study aims to describe a case series of patients with decompensated liver cirrhosis and Pneumocystis jirovecii pneumonia (PCP). The results show that liver cirrhosis can be an independent risk factor for PCP, especially in patients with severe alcoholic hepatitis who are on corticosteroid treatment for decompensated cirrhosis. Primary prophylaxis for PCP should be considered in these patients.

Summary: Chronic liver disease (CLD), including non-alcoholic fatty liver disease (NAFLD) and non-alcoholic steatohepatitis (NASH), is a significant global health concern. NAFLD is characterized by fat accumulation in the liver, while NASH is associated with inflammation and liver damage. Osteosarcopenia, a combination of muscle and bone loss, is an emerging clinical issue in CLD, with insulin resistance and chronic inflammation playing crucial roles. This article explores the relationship between osteosarcopenia and NAFLD/MAFLD, focusing on diagnosis, prevention, and treatment in CLD patients.

Summary: Chronic constipation (CC) is a common gastrointestinal condition characterized by infrequent bowel movements and/or hard stools. It can affect people of all races, ages, and sexes, but is more common in women and the elderly. CC significantly impairs patients' quality of life. Diagnosis is based on symptoms, and treatment options include lifestyle changes, fiber supplementation, laxatives, and other medications. This review provides a comprehensive overview of the epidemiology, classification, pathophysiology, clinical evaluation, and management of CC.
