Prognostic Value of the Albumin-Bilirubin Grade for the Prediction of Post-Hepatectomy Liver Failure: A Systematic Review and Meta-Analysis

The ALBI grade can accurately predict the risk of PHLF in HCC patients undergoing LR, with individuals classified as ALBI grade 2 and 3 showing higher rates of PHLF compared to those classified as ALBI grade 1.
(1) Introduction: Liver resection (LR) for hepatocellular carcinoma (HCC) is often burdened by life-threatening complications, such as post-hepatectomy liver failure (PHLF). The albumin-bilirubin (ALBI) score can accurately evaluate liver function and the long-term prognosis of HCC patients, including PHLF. We aimed to evaluate the diagnostic value of the ALBI grade in predicting PHLF in HCC patients undergoing LR. (2) Methods: MEDLINE, Embase, and Scopus were searched through January 17th, 2021. Studies reporting the ALBI grade and PHLF occurrence in HCC patients undergoing LR were included. The Odds Ratio (OR) prevalence with 95% confidence intervals (CI) was pooled, and the heterogeneity was expressed as I-2. The quality of the studies was assessed using QUADAS-2 (Quality Assessment of Diagnostic Accuracy Studies). (3) Results: Seven studies met the inclusion criteria and were included in the analysis. A total of 5377 patients who underwent LR for HCC were considered, of whom 718 (13.4%) developed PHLF. Patients with ALBI grades 2 and 3 before LR showed increased rates of PHLF compared to ALBI grade 1 patients. The pooled OR was 2.572 (95% CI, 1.825 to 3.626, p < 0.001), with substantial heterogeneity between the studies (I-2 = 69.6%) and no publication bias (Begg's p = 0.764 and Egger's p = 0.851 tests). All studies were at a 'low risk' or 'unclear risk' of bias. Univariate meta-regression analysis showed that heterogeneity was not dependent on the country of study, the age and sex of the participants, the definition of PHLF used, the rate of patients in Child-Pugh class A or undergoing major hepatectomy. (4) Conclusions: In this meta-analysis of published studies, individuals with ALBI grades of 2 and 3 showed increased rates of PHLF compared to ALBI grade 1 patients.

Effectiveness and safety of 1-L PEG-ASC versus other bowel preparations for colonoscopy: A meta-analysis of nine randomized clinical trials

Summary: This meta-analysis evaluated the effectiveness and safety of 1-L PEG-ASC compared to other bowel preparations for colonoscopy. The results showed that 1-L PEG-ASC had higher cleansing success and right-colon high-quality cleansing rates compared to other preparations, with similar adenoma detection rates. However, the incidence of adverse events was higher with 1-L PEG-ASC, although no serious adverse events or deaths occurred.

Summary: This study aimed to evaluate the prevalence of gastrointestinal symptoms and post-COVID-19 disorders of gut-brain interaction among hospitalized patients with SARS-CoV-2 infection. The results showed that patients with COVID-19 had higher rates of gastrointestinal symptoms on hospital admission and lower rates of constipation and hard stools at 12 months post-infection compared to the control group. Additionally, the rate of irritable bowel syndrome was significantly higher in patients with COVID-19 than in controls.

Summary: The study examined the characteristics of patients with chronic hepatitis B virus in Italy and found that a significant proportion of patients were non-Italian natives, and there was a decline in HBeAg prevalence and stable anti-HDV prevalence. Cirrhosis was common in both cohorts, with a higher proportion in the PITER cohort, and variables such as age, sex, BMI, and anti-HDV positivity were associated with cirrhosis. The study also observed an increase in adherence to antiviral treatment recommendations over time. In conclusion, chronic hepatitis B virus infection in Italy appears to be under control, but HDV infection remains a health concern.

Summary: This study aims to describe a case series of patients with decompensated liver cirrhosis and Pneumocystis jirovecii pneumonia (PCP). The results show that liver cirrhosis can be an independent risk factor for PCP, especially in patients with severe alcoholic hepatitis who are on corticosteroid treatment for decompensated cirrhosis. Primary prophylaxis for PCP should be considered in these patients.

Chronic constipation: from pathophysiology to management

Summary: Chronic constipation (CC) is a common gastrointestinal condition characterized by infrequent bowel movements and/or hard stools. It can affect people of all races, ages, and sexes, but is more common in women and the elderly. CC significantly impairs patients' quality of life. Diagnosis is based on symptoms, and treatment options include lifestyle changes, fiber supplementation, laxatives, and other medications. This review provides a comprehensive overview of the epidemiology, classification, pathophysiology, clinical evaluation, and management of CC.
