Summary: The study aimed to investigate the naturalistic clinical course in adolescent and young adult patients with developmental delays and catatonia, when maintenance electroconvulsive therapy (M-ECT) frequency was reduced due to COVID-19 restrictions. Participants showed significant decline in symptoms, particularly motor symptoms, leading to rehospitalization for some patients.

Summary: Neuropsychiatric systemic lupus erythematous (NPSLE) refers to various psychiatric and neurological symptoms that develop in patients with systemic lupus erythematous (SLE) due to the involvement of the central nervous system (CNS). Catatonia, although less commonly reported in SLE patients, can be managed with benzodiazepines (BZDs), immunosuppression, therapeutic plasma exchange (TPE), and electroconvulsive therapy (ECT). TPE has shown significant improvement in catatonic symptoms associated with NPSLE.

Summary: Patients with autoimmune encephalitis (AE) may present with severe psychiatric features, including psychosis and/or catatonia, with the latter potentially indicating a need for more aggressive treatment. Comparing patients with and without catatonia, those with catatonia showed more psychotic features and movement disorders, and responded better to first and second line therapies. However, they also experienced more relapses, suggesting that autoimmune catatonia may be an indicator of severity and the need for early, aggressive treatment.

Summary: Despite the inclusion of catatonia as a specifier of autism spectrum disorder in DSM-5, we have identified issues with the diagnosis and clinical management of catatonia in neurodevelopmental disorders. In this Personal View, we summarize the literature, describe our concerns, and offer a novel approach to addressing these issues. We propose a measure considering previous symptoms and suggest two concepts for understanding catatonia. We hope this measure will benefit clinicians and researchers working with this underserved population.
