ObjectiveTo write clinical guidelines for the use of psychotropic drugs during pregnancy and breast-feeding for daily practice in psychiatry, obstetrics and paediatrics. MethodAs we wanted a guideline with a high degree of consensus among health professionals treating pregnant women with a psychiatric disease, we asked the Danish Psychiatric Society, the Danish Society of Obstetrics and Gynecology, the Danish Paediatric Society and the Danish Society of Clinical Pharmacology to appoint members for the working group. A comprehensive review of the literature was hereafter conducted. ResultsSertraline and citalopram are first-line treatment among selective serotonin reuptake inhibitor for depression. It is recommended to use lithium for bipolar disorders if an overall assessment finds an indication for mood-stabilizing treatment during pregnancy. Lamotrigine can be used. Valproate and carbamazepin are contraindicated. Olanzapine, risperidone, quetiapine and clozapine can be used for bipolar disorders and schizophrenia. ConclusionIt is important that health professionals treating fertile women with a psychiatric disease discuss whether psychotropic drugs are needed during pregnancy and how it has to be administered.

Time trends of coronary procedures, guideline-based drugs and all-cause mortality following acute coronary syndrome in patients with bipolar disorder

Line Philipsen, Nanna Wurtz, Christoffer Polcwiartek, Kristian Hay Kragholm, Christian Torp-Pedersen, Rene Ernst Nielsen, Svend Eggert Jensen, Rubina Attar

Summary: This study analyzed the time trends in coronary procedures, guideline-based drugs, and mortality following acute coronary syndrome (ACS) in patients with and without bipolar disorder (BD). The results showed that patients with BD and ACS received fewer coronary angiography and redeemed fewer prescriptions of guideline-based drugs compared to the control group. The treatment gap remained unchanged for PCI and CABG, and the mortality risk ratio for patients with BD experiencing ACS remained constant over the study period.

Characteristics prior to and at time of diagnosis in pediatric bipolar disorder

Mathilde Frahm Laursen, Christoph U. Correll, Rasmus W. Licht, Maria Rodrigo-Domingo, Anne Katrine Pagsberg, Rene Ernst Nielsen

Summary: This study aims to describe the symptoms before and at the time of register-diagnosis in children and adolescents. A random sample from a Danish nationwide cohort of patients was selected for chart-review. The results showed that frequently reported symptoms before diagnosis include anhedonia, irritability, hyperactivity, decreased energy, and psychotic symptoms. In addition, symptoms of ADHD, anxiety, suicide attempts, cutting, and criminal activity were also reported before incident BD diagnosis.

Sudden Sensorineural Hearing Loss Following Immunization With BNT162b2 or mRNA-1273: A Danish Population-Based Cohort Study

Summary: This study compared the occurrence of sudden sensorineural hearing loss after immunization with BNT162b2 (Comirnaty; Pfizer BioNTech) or mRNA-1273 (Spikevax; Moderna) vaccines to unvaccinated individuals. The results showed that there was no association between these vaccines and the occurrence of sudden sensorineural hearing loss, but there was a slightly increased risk of receiving moderate to high-dose prednisolone treatment after visiting an ear-nose-throat specialist.

An analysis of the level of evidence behind treatments recommended by the Danish Medicines Council

C. L. Petersen, M. R. Hansen, T. Ohlenschlaeger, P. Damkier

Summary: This study investigated the quality of evidence and expected clinical value of treatments recommended by the Danish Medicines Council (DMC). The results showed that most recommendations by the DMC were based on low or very low-quality evidence according to GRADE ratings, and no recommendations were based on high-quality evidence. The added clinical value of the treatments was often not documented and rarely large.

Re-constructing parental identity after parents face their offspring's suicidal behaviour: An interview study

A. Juel, A. Erlangsen, L. L. Berring, E. R. Larsen, N. Buus

Summary: This study explores how parents reconstruct and negotiate their parental identity after realizing that their offspring is suicidal. Through interviews with 21 Danish parents, it is found that parents' perspectives on their parental identity can be conceptualized as a moral career consisting of three distinct stages. These stages are negotiated through social interaction and play a crucial role in the re-construction of parental identity and sense of agency.

Implementation and clinical benefit of DPYD genotyping in a Danish cancer population

N. H. Paulsen, P. Pfeiffer, M. Ewertz, S. Feddersen, H. S. Holm, T. K. Bergmann, C. Qvortrup, P. Damkier

Summary: The study aimed to compare the frequency of grade >3 FP-TOX before and after the implementation of DPYD genotyping. The results showed that the overall frequency of grade >3 FP-TOX was 27% in the DPYD genotype-guided group compared to 24% in the historical cohort. DPYD genotyping reduced the frequency of FP-related hospitalization from 19% to 0% in DPYD variant carriers. In wild-type patients, those with uracil >16 ng/mL had a higher frequency of FP-TOX than those with uracil <16 ng/mL.

Sense of psychological ownership in co-design processes: A case study

Anette Juel, Lene L. Berring, Annette Erlangsen, Erik R. Larsen, Niels Buus

Summary: This study explores how participants develop and display feelings of ownership during a collaborative process to design a website. The findings reveal that participants' sense of ownership goes through two distinct and overlapping phases. In the first phase, only researchers and the website designer display a sense of ownership, facilitated by the research context. In the second phase, service users gradually develop a sense of ownership through workshop design activities, enabling them to control the process and gain intimate knowledge of the outcome. The sense of ownership is displayed in their statements about the website.

Summary: This study examined the treatment and collaboration among 334 Danish children and adolescents diagnosed with functional seizures. The most commonly used treatment modalities were psychoeducation and outpatient follow-up. Only a few patients received the recommended combination of psychoeducation, outpatient follow-up, and psychotherapy. Implementing a multidisciplinary treatment approach should be explored to improve the treatment for this group.

Paternal preconception exposure to non-steroid anti-inflammatory drugs or opioids and adverse birth outcomes: A nationwide registry-based cohort study

Ken Lund, Olav Sivertsen Garvik, Signe Marie Aagaard, Line Riis Jolving, Michael Due Larsen, Per Damkier, Bente Mertz Norgard

Summary: The study investigated the association between paternal use of non-steroid anti-inflammatory drugs and opioids before conception and adverse birth outcomes. Weak associations were found for paternal exposure to these medications and preterm birth/small for gestational age, but not for low Apgar score or major congenital malformations. The effect sizes were small and less likely to be clinically relevant.
