Study of stability and biophysical characterization of ranibizumab and aflibercept

The anti-vascular endothelial growth factor (VEGF) agents such as ranibizumab (Lucentis (R)) and aflibercept (EyLea (R)) are currently used as monthly or bimonthly intravitreal injections to treat potentially retinal diseases such as wet age-related macular degeneration (AMD) or diabetic macular edema (DME). Because of the complications associated with repeated intra-vitreal injections, there is considerable interest in developing a sustained delivery system. The purpose of this study was to examine the stability of both therapeutic proteins under physiological conditions as well as when incorporated into drug delivery systems (DDS). First, thermotropic properties in physiological conditions and at different pH values were evaluated by differential scanning calorimetry (DSC) to determine the protein denaturation temperature. Second, the effects of pH and incubation time on conformational changes and aggregation were evaluated by circular dichroism (CD), steady-state tryptophan fluorescence spectroscopy, and size-exclusion chromatography (SEC). Also, the ability of both proteins to bind to VEGF was tested in the aforementioned experimental conditions for up to 30 days. Finally, we investigated the stability of both proteins after a rapid screening method that simulates the first homogenizing step during the protein microencapsulation process. This method allowed the development of stable ranibizumab and aflibercept formulations that may be useful to entrap these proteins into microparticles selecting the most convenient organic solvent and protein stabilizers. Injectablein-situgel depot system for targeted delivery of biologics to the retina

Summary: Delivering stable and clinically relevant biologics concentration to the back of the eye remains a challenge in ocular therapeutics, with injectable in-situ gelling depot systems emerging as an effective system for this purpose. This review discusses various biologics used in ocular therapeutics, challenges associated with their delivery, different routes of delivery, tissue barriers, and types of injectable in-situ gelling depot systems developed for biologics delivery to the back of the eye. Various critical parameters related to formulation development process and injectable depot systems highlighted in this study require careful consideration and further investigations.

Meanwhile, alternative methods such as home remedies and phytotherapies are being explored for prevention and therapy in the absence of specific treatments for COVID-19.

Liposome interaction with macrophages and foam cells for atherosclerosis treatment: effects of size, surface charge and lipid composition

Summary: The study showed that the size, charge, and lipid composition of liposomes affect the uptake by macrophages and foam cells, with cationic and DSPC liposomes being internalized more. Phagocytosis and clathrin-mediated endocytosis were identified as dominant mechanisms of uptake, and anionic and DSPC liposomes induced more cholesterol efflux capacity in foam cells.

Summary: This study investigated the use of nanocomposite materials to enhance the properties of bioresorbable coronary scaffolds, finding that functionalized nanofillers improved mechanical properties. Finite element analysis showed that nanocomposite BRS with thinner struts had increased radial strength and stiffness, which could potentially mitigate scaffold thrombosis and promote re-endothelialisation of the vessel.

Progress in drug-delivery systems in cardiovascular applications: stents, balloons and nanoencapsulation

Summary: Drug delivery systems in cardiovascular applications include drug-eluting stents and drug-coated balloons to ensure effective treatment. Besides antiproliferative drugs, growth factors, genetic materials, hormones, and signaling molecules are also used. Nanoencapsulation techniques are developed for targeted drug delivery. This review discusses drug delivery and coating mechanisms in current drug-eluting stents and drug-coated balloons, as well as innovations in drug-eluting stent technologies and drug encapsulation in nanocarriers for vascular diseases.

Interaction of systemic drugs causing ocular toxicity with organic cation transporter: an artificial intelligence prediction

Summary: Patients with chronic diseases undergo long-term systemic drug treatment, but ocular barriers can falsely recognize these drugs and allow them to enter the eye from the bloodstream, leading to toxicity. Understanding the role of organic cation transporter (OCT1) in ocular barriers is important for the entry of systemic drugs into the eye. Machine learning and computer simulation models were used to predict potential OCT1 substrates from various systemic drugs causing ocular toxicity. The accuracy of the machine learning model was around 80%, and it identified previously unknown potential OCT1 substrates.
