Mirror peripheral neuropathy and unilateral chronic neuropathic pain: insights from asymmetric neurological patterns in leprosy

Summary: This study investigated the differences between leprosy patients with and without neuropathic pain (NeP). The results showed that patients with NeP exhibited signs of C-fiber dysfunction and hyperesthesia, while patients without NeP did not. It was also found that the duration of leprosy treatment was associated with the intensity of neuropathy.

Cytokine expression profiles in white blood cells of patients with small fiber neuropathy

Summary: This study found higher gene expression of the anti-inflammatory cytokines IL-2, IL-8, and TNF in white blood cells of patients with SFN compared to healthy controls. The combination of these three cytokines had high diagnostic accuracy. Furthermore, female patients showed higher gene expression levels of IL-8 and TNF. IL-2 gene expression was negatively correlated with current pain intensity, and IL-10 gene expression differed between different types of pain.

Summary: This study investigated 101 patients with suspected autoimmune neurological syndromes (AINS) associated with glutamic acid decarboxylase (GAD) antibodies. The results showed that immunotherapy had moderate effectiveness in these patients, and serum GAD antibody levels and intrathecal GAD antibody synthesis did not predict clinical characteristics or treatment outcomes.

Local Inflammatory Mediators Involved in Neuropathic Pain

Summary: Polyneuropathy (PNP) refers to diseases of the peripheral nervous system, with 50% of cases presenting with neuropathic pain. Certain types of PNP show pain restricted to the distal leg skin, indicating a local regulatory process leading to pain. This study suggests that a pro-inflammatory pathway mediated by NF-κB may contribute to pain development in patients with painful PNP. Our findings support the hypothesis of local inflammatory processes involved in pain in PNP, and suggest disrupted anti-inflammatory pathways in inflammatory PNP involving TRPV1 and SIRT1.

Neurofilament light chain levels indicate acute axonal damage under bortezomib treatment

Summary: This study aimed to evaluate the relationship between serum levels of neurofilament light chain (NfL) and the characteristics of BTZ-induced peripheral neuropathy (BIPN). The study found that both patients with ongoing BTZ treatment and patients with previous BTZ treatment had higher serum NfL levels compared to controls, and patients with ongoing BTZ treatment had higher NfL levels than patients with past treatment. Serum NfL levels correlated with electrophysiological measures of axonal damage in the group with ongoing BTZ treatment.

Summary: The study assessed the use of RT-QuIC method to detect alpha-synuclein aggregation in skin biopsy samples of iRBD patients. It was found that 97.4% of iRBD patients, 87.2% of PD patients, and 13% of controls showed aggregation of alpha-synuclein. RT-QuIC was more sensitive but less specific compared to immunohistochemistry.

Super-resolution imaging pinpoints the periodic ultrastructure at the human node of Ranvier and its disruption in patients with polyneuropathy

Summary: This study used super-resolution fluorescence microscopy and deep learning analysis to study human nerve biopsies. The results revealed periodic protein arrangement in the node of Ranvier in healthy and diseased human peripheral nerves, with a weakened periodicity in disease conditions associated with cytoskeletal damage. The study demonstrates the potential of super-resolution imaging for clinical applications.
