Three year functional changes and long-term mortality hazard in community-dwelling older men

Background: Low levels of physical function have been associated with higher mortality hazard in older persons. However, few studies have investigated the association between functional changes and subsequent mortality. This study aimed to examine whether 3-year functional changes independently predict subsequent all-cause mortality. Methods: This population-based cohort study included 171 community-dwelling men aged >= 71 years at wave 2 (baseline of the present analysis), living in the semi-rural community of Merelbeke (Belgium). Physical function assessments included the Short Form-36 (SF-36) Physical Function Index, Grip strength, Chair rising, and Timed Up and Go. Changes over a 3-year time were calculated using data obtained at four annual visits. Results: After a 15-year follow-up, 149 men (87%) died. Median survival time was 8.2 (4.2-12.4) years. Physical function assessed at a single time point (at wave 2 or wave 5) was significantly associated with subsequent mortality hazard, independently from future or preceding 3-year changes. Greater functional declines during the 3-year follow-up were associated with higher mortality hazards. These associations were 1) more pronounced within the first seven years, 2) independent frombaseline age, polypharmacy, depression, disability, and physical function, and 3) no longer significant when closure physical function was taken into account. Conclusion: Physical function assessed at a single time point is a robust predictor of all-cause long-termmortality in community-dwelling older men. Yet, repeated assessments of physical function can provide prognostic information beyond that available from single initial assessment. However, with repeated assessments, most prognostic information can be found in the final assessment of physical function. (C) 2016 European Federation of Internal Medicine. The study found that sex steroids, IGF-1, and bone metabolism markers are related to skeletal development in peri-pubertal boys, but the relationship is nonlinear and dependent on the maturation stage and skeletal site.

Summary: In clinical practice in Belgium, LT4 tablets are the preferred treatment for hypothyroidism. A minority of doctors would try combined LT4 + LT3 treatment in symptomatic but biochemically euthyroid patients. The use of thyroid hormones in infertile women with thyroid autoimmunity and patients with non-toxic goiter is also common in other European countries, despite the lack of evidence supporting this practice.

Arterial stiffness in patients with type 1 diabetes and its comparison to cardiovascular risk evaluation tools

Summary: This study evaluated arterial stiffness in patients with type 1 diabetes (T1D) and without known cardiovascular disease (CVD), and compared it to other cardiovascular risk evaluation tools used in T1D. The findings showed a substantial proportion of T1D patients without CVD displayed premature arterial stiffening, and arterial stiffness was strongly associated with the risk of future cardiovascular events as well as other cardiovascular factors. However, there were heterogeneity in cardiovascular risk and differences in risk categorization between different evaluation tools. Further research is needed to refine cardiovascular risk classification and determine the clinical usefulness of assessing arterial stiffness in T1D.

Summary: This study compared the effects of single drug and combination treatment as therapeutic strategies against non-alcoholic steatohepatitis (NASH). The results showed that CCR2/5 inhibition and FGF21 agonism both had favorable effects on improving NASH and fibrosis. Combination treatment amplified the beneficial effects and provided better outcomes in both short-term and long-term applications.

Osteoporosis and Bone Health in Transgender Individuals

Summary: This review discusses the impact of gender-affirming hormonal treatment (GAHT) on bone mass, structure, and metabolism in transgender adults and adolescents. It suggests that GAHT in transgender adults does not result in significant bone loss, while pubertal suppression in transgender adolescents affects bone development, with partial recovery observed after GAHT initiation. However, there is still a research gap in understanding fracture risk and other determinants of bone strength. Attention to bone health is especially important for adult and adolescent trans women, who have a relatively high prevalence of low bone mass before and after long-term GAHT. Strategies to optimize bone health include monitoring treatment compliance, ensuring adequate exposure to sex steroids, physical activity, sufficient vitamin D and calcium intake, and a healthy lifestyle. When risk factors for osteoporosis are present, DXA screening should be conducted with a low threshold, and treatment decisions should follow the same guidelines as the general population.

Muscle Strength, Muscle Mass and Physical Impairment in Women with hypermobile Ehlers-Danlos syndrome and Hypermobility Spectrum Disorder

Summary: This study evaluated the differences in physical impairment, muscle strength, muscle mass, and muscle density between patients with hypermobile Ehlers Danlos Syndrome (hEDS), hypermobile spectrum disorder (HSD), and healthy controls. The results showed no significant differences in physical functioning and muscle strength between individuals with hEDS and HSD. However, compared to the healthy controls, both patient groups had significantly lower physical functioning and muscle strength. Therefore, strength training is necessary for individuals with hEDS and HSD.
