Proprotein convertase subtilisin kexin 9 (PCSK9) inhibitors to treat hypercholesterolemia: Effect on stroke risk

Background/purpose: A reduction of cardiovascular events has been reported in phase 2 and 3 trials of the proprotein convertase subtilisin kexin 9 (PCSK9) inhibitors alirocumab and evolocumab. We aimed to investigate the effect PCSK9 inhibition on stroke risk in a meta-analysis involving data from randomized studies with alirocumab and evolocumab. Methods: Data from pre-specified combined analysis of 4465 patients who completed phase 2 or 3 studies of evolocumab over a period of 1 year and a randomized trial on alirocumab including 2341 patients with hyperlipidemia on maximally tolerated statin who were at high risk for coronary heart disease over a period of 1.5 years were used. Results: The number of patients having an ischemic stroke was small in both trials. PCSK9 inhibition showed no significant effect on stroke rate (risk ratio 1.43; 95% CI, 0.45-4.57, p = 0.55). No significant differences in stroke risk were evident when transient ischemic attacks were included in the analysis (risk ratio 0.65; 95% CI, 0.25-1.68, p = 0.37). No hemorrhagic strokes were reported in either study. Conclusion: Although a benefit towards reduction of cardiovascular events in the overall has been documented, longer exposure is warranted to be able to evaluate the effect on stroke risk. Atrial cardiopathy and non-stenotic intracranial complicated atherosclerotic plaque in patients with embolic stroke of undetermined source

Summary: The study found that the presence of atrial cardiopathy (AC) was associated with embolic stroke of undetermined source (ESUS) in patients without ipsilateral non-stenotic intracranial complicated atherosclerotic plaque (NICAP). Biomarkers related to AC, such as P-wave terminal force in lead V1 (PTFV1) and N-terminal probrain natriuretic peptide (NT-proBNP), were found to be informative in distinguishing between ESUS and small-vessel disease (SVD). The distinction between ESUS and SVD by AC and related biomarkers was significantly improved after excluding ipsilateral NICAP. Similarly, the discrimination by ipsilateral NICAP was notably augmented after excluding AC, PTFV1 and NT-proBNP.

Estimation of the economic burden of atrial fibrillation-related stroke in Greece

Summary: This study estimated the total annual socioeconomic burden of AF-related stroke in Greece at 175 million euros in 2018. Major cost contributors included informal care, patient productivity losses, and hospitalizations. The results suggest that reallocating resources efficiently could reduce the burden of AF-related stroke and improve healthcare system efficiency.

Interleukin-6 inhibitors reduce mortality in coronavirus disease-2019: An individual patient data meta-analysis from randomized controlled trials

Summary: The use of oral anticoagulation therapy in patients with heart failure and sinus rhythm can reduce the occurrence of stroke, but it also increases the risk of major bleeding. Rivaroxaban has a better effect in preventing stroke, but it is also associated with a higher risk of bleeding. There were no significant differences between the use of oral anticoagulants and the control group in terms of myocardial infarction, all-cause mortality, and hospitalization for heart failure.

Risk of myocardial infarction and ischemic stroke in individuals with first-diagnosed paroxysmal vs. non-paroxysmal atrial fibrillation under anticoagulation

Summary: This study aimed to investigate whether there is a difference in the risk of cardiovascular events, including acute myocardial infarction and ischemic stroke, between individuals with first-diagnosed paroxysmal AF treated with anticoagulants and those with non-paroxysmal AF. The results showed that individuals with paroxysmal AF had a higher risk of acute myocardial infarction, while there was no significant association between the type of AF and risk of ischemic stroke.

Characteristics of intracranial plaque in patients with non-cardioembolic stroke and intracranial large vessel occlusion

Summary: This study used 3.0 T high-resolution MRI to investigate the characteristics of intracranial plaque proximal to large vessel occlusion (LVO) in stroke patients without major-risk cardioembolic source. The results showed that intracranial plaque proximal to LVO was more common on the ipsilateral side to stroke and had larger plaque burden, higher remodelling index, higher percentage of lipid-rich necrotic core, higher prevalence of discontinuity of plaque surface, fibrous cap rupture, intraplaque haemorrhage and complicated plaque. Logistic analysis revealed that remodelling index and plaque burden were positively associated with ischaemic stroke. In the subgroup with <50% stenotic plaque, higher plaque burden, remodelling index, percentage of lipid-rich necrotic core and presence of complicated plaque were more closely related to stroke.

Summary: This study investigated the clinical characteristics and outcomes of hospitalized T2D patients with COVID-19 and found that the use of DPP4 inhibitors for chronic T2D management was associated with increased risks of in-hospital death, ICU admission, progression to ARDS, and thromboembolic events. These findings highlight the importance of considering the potential impact of chronic T2D treatment regiments on COVID-19.

Summary: The aim of this study was to assess the association between the presence of a right-to-left shunt (RLS) and neurological decompression sickness (NDCS) and asymptomatic brain lesions among otherwise healthy divers. The results showed that high-grade RLS is associated with a greater risk of NDCS, but the association with asymptomatic brain lesions is not statistically significant.
