CRISPR/Cas12-Based Ultra-Sensitive and Specific Point-of-Care Detection of HBV

The study developed a rapid and accurate point-of-care assay for HBV based on LAMP-Cas12a, addressing the challenge of nucleic acid extraction and providing visualization of the assay results. The method showed high sensitivity and specificity, with results comparable to qPCR.
Hepatitis B remains a major global public health challenge, with particularly high prevalence in medically disadvantaged western Pacific and African regions. Although clinically available technologies for the qPCR detection of HBV are well established, research on point-of-care testing has not progressed substantially. The development of a rapid, accurate point-of-care test is essential for the prevention and control of hepatitis B in medically disadvantaged rural areas. The development of the CRISPR/Cas system in nucleic acid detection has allowed for pathogen point-of-care detection. Here, we developed a rapid and accurate point-of-care assay for HBV based on LAMP-Cas12a. It innovatively solves the problem of point-of-care testing in 10 min, particularly the problem of sample nucleic acid extraction. Based on LAMP-Cas12a, visualization of the assay results is presented by both a fluorescent readout and by lateral flow test strips. The lateral flow test strip technology can achieve results visible to the naked eye, while fluorescence readout can achieve real-time high-sensitivity detection. The fluorescent readout-based Cas12a assay can achieve HBV detection with a limit of detection of 1 copy/mu L within 13 min, while the lateral flow test strip technique only takes 20 min. In the evaluation of 73 clinical samples, the sensitivity and specificity of both the fluorescence readout and lateral flow test strip method were 100%, and the results of the assay were fully comparable to qPCR. The LAMP-Cas12a-based HBV assay relies on minimal equipment to provide rapid, accurate test results and low costs, providing significant practical value for point-of-care HBV detection.

Local immune dysregulation and subsequent inflammatory response contribute to pulmonary edema caused by Enterovirus infection in mice

Summary: Sudden onset of pulmonary edema is the leading cause of mortality in hand-foot-and-mouth disease (HFMD) patients, but its pathogenesis remains unclear. Research suggests that immunopathogenesis plays a role in the occurrence of pulmonary edema in severe HFMD patients. In mouse infection models, enterovirus (EV) infection was found to induce immune system disorder, reduce the numbers of pulmonary and circulatory immune cells, increase the numbers of lung neutrophils, myeloid-derived suppressor cells (MDSCs), and activated T cells, and lead to apoptosis of lung cells and degradation of tight junction proteins. These findings suggest that EV infection triggers a complex immune defense mechanism, causing dysregulation of innate and adaptive immune cells, release of cytokines and inflammatory factors, and lung barrier damage leading to pulmonary edema.

Involvement of RNA chaperone hfq in the regulation of antibiotic resistance and virulence in Shigella sonnei

Summary: The study investigated the functional roles of Hfq in Shigella sonnei by constructing an hfq deletion mutant. The results showed that the hfq deletion mutant was more sensitive to antibiotics and had impaired virulence. Transcriptome analyses supported the phenotype of the hfq mutant and predicted eleven novel Hfq-dependent sRNAs potentially involved in the regulation of antibiotic resistance and/or virulence in S. sonnei.

Arising Concerns of Atypical Manifestations in Patients with Hand, Foot, and Mouth Disease

Summary: Hand, foot, and mouth disease (HFMD) is a mild viral infection that poses a threat to global public health. Typical HFMD is characterized by a short febrile illness with a distinctive rash on the hands, feet, and mouth. However, atypical HFMD caused by a specific virus can present with different skin manifestations, making it challenging to diagnose and distinguish from other severe skin diseases. Recognizing the distinguishing features of atypical HFMD is crucial for accurate diagnosis and prompt treatment. A comprehensive overview of atypical HFMD is necessary to improve clinical strategies. This review aims to summarize clinical investigations and differential diagnostics to provide a scientific guide for timely diagnosis and prevention of serious complications.

Abundant Neutrophil-Initiated Acute Myocardial Injury Following Coxsackievirus A6 Infection

Summary: Coxsackievirus A6 (CVA6) infection is associated with myocardial injury in hand, foot, and mouth disease (HFMD). Neutrophil infiltration is linked to the occurrence of myocardial injury. Depletion of neutrophils reduces cardiac enzymes, pathology in the heart, and inflammatory cytokine production. Clinical observations also support the role of neutrophils in myocardial injury.

Progress and bioapplication of CRISPR-based one-step, quantitative and multiplexed infectious disease diagnostics

Summary: In vitro diagnosis (IVD) technology, such as CRISPR-Cas system, has become an important tool for disease diagnosis due to its high sensitivity and specificity in detecting pathogens or biomarkers at an early stage of disease. Scientists have been focused on improving CRISPR-based detection and on-site point-of-care testing (POCT) by developing extraction-free, amplification-free, quantitative, one-pot, and multiplexed methods. This review discusses the potential roles of these novel approaches and platforms in quantification, molecular diagnostics, and multiplexed detection, and highlights their significance in addressing real-world challenges like the ongoing COVID-19 pandemic.

Current status of hand-foot-and-mouth disease

A meta-analysis of immunogenicity and safety of two versus single-doses of influenza A (H1N1) vaccine in person living with HIV

Summary: This study evaluated the immunogenicity and safety of two doses of influenza vaccine compared to a single dose in people living with HIV. The analysis of 14 studies with a total of 2436 participants showed that two doses of the vaccine resulted in higher seroprotection and seroconversion rates, as well as a better immunogenicity based on geometric mean titers. However, the risk of fever was increased by 3.42 times in HIV-positive individuals receiving two doses of the vaccine, while the risk of myalgia was reduced by a quarter. No serious vaccine-related adverse events were reported.

Analysis of the features of 105 confirmed CRISPR loci in 487 Klebsiella variicola

Summary: This study analyzed the characteristics of CRISPR/Cas systems using data from 487 K. variicola genomes. The results showed that approximately 21.56% of the genomes contained at least one confirmed CRISPR array. Additionally, three types of CRISPR/Cas systems were identified and spacer analysis revealed the significance of CRISPR/Cas systems in controlling horizontal gene transfer.
