Summary: Liver transplantation (LT) is a complex surgical procedure that requires careful planning and care to ensure the nutritional status of the patient. This review examines the assessment and management of nutritional status before, during, and after LT, specifically for patients who have undergone bariatric surgery. Key factors influencing nutritional status in liver transplant patients, such as pre-existing malnutrition, liver disease severity, comorbidities, and immunosuppressive medications, are identified. The review emphasizes the importance of pre-operative nutritional assessment and intervention, close monitoring of nutritional status, individualized nutrition care plans, and ongoing support after LT. It also evaluates the impact of bariatric surgery on the nutritional status of liver transplant recipients, providing valuable insights for optimizing nutritional status during the transplant process.

Summary: Biliary atresia (BA) is the leading indication for pediatric liver transplantation worldwide, but it is uncommon for older liver transplant recipients. The impact of recent donor allocation changes, organ shortage, and evolving transplantation practices on the BA liver transplant population is unknown. The study found that liver transplant outcomes in BA patients are excellent, and optimal graft outcomes are associated with living-donor liver transplantation (LDLT) and higher transplant center volume.

Summary: Living donor liver transplantation (LDLT) is an effective solution for the organ shortage in the US, but there is limited understanding of the medical evaluation practices for living liver donors. A survey was conducted on all 53 active LDLT transplant programs in the US, revealing consensus on certain criteria such as minimum age and exclusion of obese donors, while variations existed in areas like acceptance of donors with diabetes mellitus. The findings highlight the need for data generation to develop evidence-based guidelines for the evaluation and risk assessment of living liver donors.

Summary: With the improvement in post-transplant patient and graft survival, there is a need to focus on patient experience and health-related quality of life (HRQOL). Liver transplantation can save lives but may come with morbidity and complications. Patient HRQOL improves after transplantation but may not reach the level of age-matched cohorts. Understanding factors such as physical and psychological health, immunosuppression, medication adherence, employment or school, financial burden, and expectations can help in developing interventions to improve HRQOL.

Summary: This study analyzed the frequency of cost/value statements in clinical guidance documents on chronic liver disease (CLD). Results showed that out of 1334 guidance statements, only 80 (6%) were related to cost/value. Only 1.1% reported patient-level costs and none reported out-of-pocket costs. Despite the growing importance of considering cost and value in care, there is a low proportion of cost/value statements in CLD-related clinical guidance documents.
