Randomized clinical trials of COVID-19 vaccines: Do adenovirus-vector vaccines have beneficial non-specific effects?

Summary: We reviewed the randomized control trials of mRNA and adenovirus-vector COVID-19 vaccines to examine their possible non-specific effects. The mortality risk ratios (RRs) for mRNA COVID-19 vaccines vs. placebo recipients and adenovirus-vector COVID-19 vaccine recipients vs. controls were calculated. The overall mortality RR for mRNA vaccines vs. placebo was 1.03 (95% confidence interval [CI]: 0.63-1.71), while for adenovirus-vector vaccines, it was 0.37 (0.19-0.70). There was a significant difference between the two vaccine types in terms of overall mortality impact (p = 0.015). The unblinded and vaccinated control groups in the COVID-19 vaccine RCTs may not represent long-term effects. Randomized control trials comparing long-term effects on overall mortality between mRNA and adenovirus-vector vaccines are warranted.

Adjuvants for COVID-19 Vaccines

Summary: In recent decades, improvements in traditional vaccines have led to the development of more processed vaccines with better tolerability profiles but lower immunogenicity. Adjuvants provide a good solution to enhance the immunogenicity of these vaccines with improved tolerability and fewer side effects. The approval of protein-based vaccines in 2022 and 2023 further expanded the options. Adjuvanted vaccines can induce potent immune responses in populations with weak or insufficient immune systems, such as the elderly, and could play a crucial role in completing global COVID-19 vaccination efforts in the present and future.

Summary: This study investigated the adverse effects associated with three different COVID-19 vaccines and found that local reactions were more common with Pfizer and AstraZeneca vaccines, while systemic events were more prevalent with AstraZeneca and Pfizer vaccines. There was no correlation between age or gender and the duration of adverse effects, and some participants reported unusual symptoms like swelling of eyelids and severe allergic reactions. Overall, Pfizer, AstraZeneca, and Sinopharm vaccines were deemed safe, with Sinopharm showing lower prevalence of adverse effects compared to the others.

Summary: This retrospective study assessed the effectiveness and safety of tildrakizumab in the real-life clinical practice for treating plaque psoriasis. The results showed that a significant proportion of patients achieved PASI 75, PASI 90, and PASI 100 at 52 weeks, with higher percentages achieving complete skin clearance. No significant safety concerns were observed, and no patients had to discontinue treatment due to adverse events.

Consensus was reached on six main areas that cover pre-screening exams, dosing and administration, optimal strategy for patients in remission, use of folic acid, safety, and predictors of toxicity and efficacy. Specific recommendations are provided for all 23 statements.

The findings of this study can help healthcare professionals in planning comprehensive patient management and lead to more frequent early diagnosis and treatment, benefiting patients on their medical journey.
