The study established the reference intervals (RI) of glycated albumin (GA) in Caucasian healthy pregnant women and found that GA levels decrease throughout pregnancy.
Background and aims: Glycated albumin (GA) could represent a useful biomarker in pregnant women for diagnosing and monitoring gestational diabetes mellitus (GDM). The establishment of reference intervals (RI) is mandatory before assessing its clinical usefulness. The RIs of GA in healthy pregnant women are not well defined. The aim of the current study was to establish the RI in a cohort consisting of Caucasian pregnant women without overt diabetes mellitus or gestational diabetes mellitus. Methods: The study included 183 healthy pregnant women. GA was measured on plasma by an enzymatic method (quantILab Glycated Albumin, IL Werfen, Germany). The RI was calculated by the non-parametric and robust methods. Results: The RI of GA in the whole population was 10.16% (90%CI 9.60-10.70) and 15.44% (90%CI 14.90-16.90). GA levels decreased during pregnancy, with lower levels in the third trimester: 10.11 (90%CI 9.48-10.79) and 15.72 (90%CI 15.15-16.27) in the first trimester, 10.49 (90%CI 10.05-10.96) and 15.49 (90% CI 15.05-15.92) in the second trimester, 9.84 (90%CI 9.50-10.22) and 14.57 (90%CI 14.11-15.01) in the third trimester. Finally, a weak negative correlation was found between GA levels and body mass index. Conclusion: This is the first study establishing the RIs of GA in Caucasian healthy pregnant women.

Monocyte Distribution Width (MDW) as a biomarker of sepsis: An evidenced-based laboratory medicine approach

Anna Maria Ciaccio, Luisa Agnello, Bruna Lo Sasso, Rosaria Vincenza Giglio, Alessandro Iacona, Caterina Maria Gambino, Concetta Scazzone, Antonino Tuttolomondo, Marcello Ciaccio

Summary: Monocyte Distribution Width (MDW) is a new cell blood count parameter that measures the anisocytosis of monocytes. It has emerged as a reliable biomarker for sepsis in emergency departments and intensive care units. MDW has advantages such as low-cost and quick measurement compared to other sepsis biomarkers.

Multiple Sclerosis Pathogenesis: Possible Interplay between Vitamin D Status and Epstein Barr Virus Infection

Giulia Bivona, Concetta Scazzone, Bruna Lo Sasso, Rosaria Vincenza Giglio, Caterina Maria Gambino, Luisa Agnello, Marcello Ciaccio

Summary: Recent literature has discussed the role of vitamin D, Epstein Barr Virus (EBV), and endogenous retrovirus (ERV) in the pathogenesis of multiple sclerosis (MS). Some authors suggest a synergistic interplay between these factors that could contribute to the onset and progression of the disease. However, further research is needed to better understand this important topic.

Summary: Pre-obesity is a condition that increases the risk of obesity, cardiovascular diseases, and diabetes. A study found that Altilix(R), a nutraceutical containing chlorogenic acid and luteolin extracts, can improve hepatic and cardio-metabolic parameters. In a post-hoc analysis, Altilix(R) was shown to effectively improve anthropometry, glucose metabolism, lipid profile, hepatic functionality, carotid-media thickness, and endothelial function in pre-obesity subjects.

Summary: A systematic review and meta-analysis were conducted to evaluate the diagnostic performance of TDP-43 as a biomarker for ALS. The results showed that CSF TDP-43 levels are higher in ALS patients compared with control groups. CSF TDP-43 could potentially serve as a biomarker for ALS, but more research is needed before definitive conclusions can be drawn.

Summary: Diabetic nephropathy is the leading cause of end-stage renal disease globally, and its pathogenesis involves increased pressure, activity of the renin-angiotensin system, and oxidative stress. Current treatment options focus on optimizing blood pressure and blood glucose targets, but new drug classes such as GLP-1 RAs, SGLT-2is, and inhibitors of mineralocorticoid receptor-mediated sodium reabsorption show positive effects on kidney function in individuals with type 2 diabetes mellitus. This review aims to provide an update on therapeutic options for preventing or slowing the onset and progression of diabetic nephropathy.

Monocyte distribution width alterations and cytokine storm are modulated by circulating histones

Daniela Ligi, Bruna Lo Sasso, Chiara Della Franca, Rosaria Vincenza Giglio, Luisa Agnello, Marcello Ciaccio, Ferdinando Mannello

Summary: The study investigated the role of extracellular histones on monocyte distribution width (MDW) and their effect on the release of cytokines by blood cells. The results showed that extracellular histones can induce changes in MDW and promote heterogeneity in monocytes. They also lead to the release of cytokines, especially G-CSF, IL-1 beta, IL-6, MIP-1 beta, and IL-8. These findings may have significant implications for predicting the risk of severe outcomes.

Genetics of congenital heart disease

Summary: The cardiovascular system and the central nervous system exhibit a coordinated developmental process during embryonic development. Congenital heart disease (CHD) is the most common congenital disorder, and neurodevelopmental disorders (NDD) are common complications in CHD patients. Both genetic and non-genetic factors contribute to the co-occurrence of CHD and NDD. Further research should focus on identifying common molecular mechanisms underlying this co-occurrence and promoting the research and treatment of developmental disorders related to the cardiovascular and central nervous systems.
