Living-Donor Lung Transplantation for Dyskeratosis Congenita

Two cases of dyskeratosis congenita in patients with progressive pulmonary diseases underwent urgent living-donor lung transplantations and recovered uneventfully. The surgeries were successful and resulted in good outcomes.
This report describes 2 cases of dyskeratosis congenita in patients with progressive pulmonary diseases who required urgent living-donor lung transplantations. In the first case, a 13-year-old boy underwent bone marrow transplantation for aplastic anemia at age of 6 years. Severe pulmonary right-to-left shunting and fibrosis developed, and his condition deteriorated with acute fibrosis exacerbation and increased intrapulmonary shunt. He underwent lung transplantation and recovered uneventfully. In the second case, a 3-year-old girl with Revesz syndrome underwent bone marrow transplantation for refractory cytopenia. At 6 years of age, she had progressive hypoxia and a brain abscess. Her respiratory condition worsened, and she recovered uneventfully after urgent lung transplantation. Summary: This report describes a case of COVID-19-associated respiratory failure that was successfully treated with urgent living-donor lobar lung transplantation. The patient's respiratory function improved after ECMO support and LDLLTx surgery. LDLLTx is a viable option for patients with endstage respiratory failure following COVID-19.

Summary: This study evaluated the pathological findings associated with radiological PPFE-like lesions and the clinical and morphological features of patients with pathological PPFE. The results showed that pathological PPFE was related to previous pneumothorax, volume loss in the upper lobes, and a flattened chest. Patients with idiopathic disease and those with secondary disease with pathological PPFE had similar clinical, radiological, and pathological findings. However, fibroblastic foci were more common in idiopathic cases and alveolar septal thickening with elastosis or fibrosis were more common in secondary cases. The post-transplantation survival of patients with pathological PPFE was similar to those with idiopathic pulmonary fibrosis (IPF).

Summary: This study reviewed the clinicopathological characteristics and PD-L1 expression in 46 patients with pulmonary pleomorphic carcinoma, and found that high PD-L1 expression in the carcinomatous components was significantly associated with a poor prognosis after surgery.

Summary: This article reports successful cases of lung transplantation using ex vivo lung perfusion (EVLP) to evaluate the function of injured donor lungs. After being declined by other transplant centers, the lungs were offered to patients at the authors' hospital. The outcomes of the two cases suggest that EVLP may expand the pool of acceptable donor lungs for transplantation.

Living-donor segmental lung transplantation for pediatric patients

Summary: This study evaluated the outcome of living-donor segmental lung transplantation in pediatric patients. The results showed that the procedure was technically difficult but feasible, with acceptable outcomes. The transplantation was successful in most cases, with only a few complications.

Assessment of listing criteria for lung transplant candidates with interstitial lung disease

Summary: By analyzing registered lung transplant patients, the study found that a decline of more than 10% in lung function within 6 months and a 6-minute walking distance of less than 250m were associated with a worse prognosis (death or living-donor lobar LT). The ISHLT criteria could appropriately identify ILD patients with potentially poor prognosis.

Reduction of donor mononuclear phagocytes with clodronate-liposome during ex vivo lung perfusion attenuates ischemia-reperfusion injury

Summary: The objective of this study was to investigate the depletion of mononuclear phagocytes and the attenuation of ischemia-reperfusion injury by clodronate-liposome during ex vivo lung perfusion. The experiments determined the number of mononuclear phagocytes in flushed grafts using flow cytometry and administered clodronate-liposome into the perfusate during ex vivo lung perfusion. The results showed a significant reduction in the number of mononuclear phagocytes in the clodronate group, improved lung function, and reduced lung injury.

Outcome and growth of lobar graft after pediatric living-donor lobar lung transplantation

Summary: This study retrospectively analyzed the clinical data of 25 living-donor lung transplantations performed in 24 pediatric recipients aged <= 17 years, evaluating the outcomes of pediatric living-donor lobar lung transplantation (LDLLT) and post-transplant graft growth. The study shows that pediatric LDLLT offers satisfactory long-term survival, with mature adult lobes transplanted into growing children.

Electromagnetic navigation bronchoscopy-guided radiofrequency identification marking in wedge resection for fluoroscopically invisible small lung lesions

Summary: This study developed a wireless localization technique for fluoroscopically invisible small lung lesions and compared two marking approaches with or without cone-beam computed tomography (CBCT). The results showed that electromagnetic navigation bronchoscopy-guided radiofrequency identification marking provided a high tumor resection rate with sufficient surgical margins. The use of CBCT for marking resulted in a shorter distance from the marker to the lesion.
