Enteral immunonutrition versus enteral nutrition for patients undergoing esophagectomy: a randomized controlled trial

This study aimed to investigate the efficacy of perioperative enteral immunonutrition (EIN) on clinical and immunological outcomes of patients undergoing esophageal resection. The results showed significant differences in immune indicators between the EIN and EN groups, while no significant differences were found in clinical outcomes and survival rates. Immunonutrition may have a positive impact on the immune function of patients undergoing esophagectomy.
Background: In recent years, immunonutrition has been introduced and proposed to have a positive modulatory effect on inflammatory and immune responses and gut function for surgical patients, especially for patients undergoing gastrointestinal cancer resection. We conducted this parallel-group, randomized and double-blind clinical controlled trial to investigate the efficacy of perioperative enteral immunonutrition (EIN) on clinical and immunological outcomes of patients undergoing esophageal resection. Methods: A randomized, parallel-group, double-blind, clinical trial was conducted between December 1, 2017 and March 1, 2018. This study enrolled 120 patients with esophageal cancer. And 112 patients were divided into two groups randomly: EIN group and enteral nutrition (EN) group. The EIN contained extra immunonutritional substrates, including a consistent combination of arginine, RNA and the omega-3 fatty acids compared with EN. Immune indicators were measured at preoperative day 7, postoperative day (POD) 1, 3, 7 and post-discharge day (PDD) 30. Results: There were 56 participants randomized to each group. Finally, 53 patients in EIN and 50 patients in EN were analyzed. Immune indicator was the primary outcome in this study. EIN yielded a significantly lower rate of CD8/CD3 (%) at POD 3 compared with EN group (P=0.005). The rate of CD4/CD8 (%) in EIN group was higher than that in EN group at POD3 (P=0.004). The serum levels of IgM at POD 3 and 7 were significantly higher in EN group compared with EIN group (P=0.025 and P=0.009, respectively). The rate of NK (%) and the serum level of IgA were significantly higher in EIN group compared with EN group at PDD 30 (P=0.022 and P=0.041, respectively). No significant differences were found in 2-year progression-free survival and overall survival. Conclusions: Immunonutrition is a safe and feasible nutritional treatment, which has a positive modulatory impact on immune responses after esophagectomy. Although no significant difference was found in clinical and survival outcomes between EIN and EN groups, immunonutrition could still have a positive effect on immunological function of patients undergoing esophagectomy.

Molecular characteristics of multifocal esophageal squamous cell carcinomas to discriminate multicentric origin from intramural metastasis

Summary: This study aimed to differentiate between multicentric origin (MO) and intramural metastasis (IMM) in multifocal esophageal squamous cell carcinomas (ESCCs) and explore the tumor immune microenvironment. The research identified complex clonal evolution patterns in IMM cases and found that tumor cells disseminated from primary tumors to IMM tumors were independent of lymph node metastasis. The study also revealed higher immune cell infiltrates and immune pathway activation in large tumors compared to small ones in MO cases. These findings provide comprehensive characterization of the heterogeneity and clonal relationship of multifocal ESCCs, which can aid in distinguishing MO from IMM and interpreting immunotherapy responses.

Summary: This study depicted the mutational profiles of NSCLC patients with RET fusions at baseline or after resistance to EGFR-TKIs. The results suggest that RET fusions may mediate secondary resistance to third-generation EGFR-TKIs and could be associated with poor prognosis in patients with NSCLC.

Summary: Selective dorsal neurotomy (SDN) is a surgical treatment for primary premature ejaculation (PE) that lacks a standard surgical procedure for selecting the branches of the dorsal penile nerves. This study aimed to explore the value of intraoperative neurophysiological monitoring (IONM) of penile sensory-evoked potential (PSEP) for standardizing SDN procedures. The results showed that IONM may improve short-term therapeutic effectiveness and reduce complications in SDN surgeries.

A novel NGS-based diagnostic algorithm for classifying multifocal lung adenocarcinomas in pN0M0 patients

Summary: In this study, NGS analysis was used to diagnose MLA patients with MPLAs and IPMs. By incorporating a predefined NGS criterion, accurate classification of IPMs was achieved. A refined algorithm was developed that combined NGS and histologic results to correctly diagnose MPLAs and IPMs. This NGS-based algorithm showed promising potential in complementing traditional CHA-based MLA diagnosis and helping determine patient staging and treatment.

Summary: This retrospective study analyzed next-generation sequencing (NGS) data from 1025 non-small cell lung cancer (NSCLC) patients with compound epidermal growth factor receptor (EGFR) mutations. Different subtypes of compound EGFR mutations were associated with distinct clinical features, genetic architectures, and prognostic outcomes. Rare mutation-dominant subtypes with variations of uncertain significance (VUSs) residing in the kinase domain had poor clinical outcomes and implied disruptions in kinase domain regulation/function.

Comprehensive profiling of EGFR mutation subtypes reveals genomic-clinical associations in non-small-cell lung cancer patients on first-generation EGFR inhibitors

Summary: In this study, the genomic profiles of NSCLC patients with cEGFR and uEGFR mutations were analyzed using targeted next-generation sequencing. The results showed that uEGFR-positive patients were more likely to have co-occurring genetic alterations in the Hippo pathway and a higher TMB. Smoking-related mutations were also enriched in uEGFR-positive patients. Subgroup analyses revealed potential prognostic biomarkers and significant differences in mutational profiles among patients with different EGFR sub-type mutations.

The relationship between the distribution of facial erythema and skin type in rosacea patients: a cross-sectional analysis

Summary: This study found that individuals with rosacea have different facial erythema distribution patterns, which may be related to skin types. The erythema distribution pattern of rosacea was divided into peace signs, wing shapes, and neither of the two patterns. The peace sign pattern was more common in individuals with darker skin tones, while the wing shape pattern was associated with the lateral parts of the cheeks. This has important implications for understanding the pathogenesis and treatment of rosacea.

Mutations in the MET tyrosine kinase domain and resistance to tyrosine kinase inhibitors in non-small-cell lung cancer

Summary: This study found that MET tyrosine kinase domain (TKD) mutations were detected in both baseline and patients treated with TKIs in non-small-cell lung cancer (NSCLC) patients. MET-H1094Y may be associated with acquired resistance after recurrent EGFR-TKIs treatment. Preliminary data suggests that EGFR-mutated NSCLC patients who acquired MET-V1092I or MET-H1094Y may benefit from combinatorial therapy with EGFR-TKI and MET-TKI, providing new insights into personalized medical treatment.
