Implementation and effectiveness of a multi-domain program for older adults at risk of cognitive impairment at neighborhood senior centres

The study implemented a multi-domain dementia prevention program for at-risk older adults in neighborhood senior centres, reaching nearly 50% of eligible participants with positive feedback. Despite no between-group differences in cognition, quality of life, and blood parameters, the program had a positive impact on physical function.
To address the paucity of research investigating the implementation of multi-domain dementia prevention interventions, we implemented and evaluated a 24-week, bi-weekly multi-domain program for older adults at risk of cognitive impairment at neighborhood senior centres (SCs). It comprised dual-task exercises, cognitive training, and mobile application-based nutritional guidance. An RCT design informed by the Reach, Effectiveness, Adoption, Implementation, Maintenance framework was adopted. Outcome measures include cognition, quality of life, blood parameters, and physical performance. Implementation was evaluated through questionnaires administered to participants, implementers, SC managers, attendance lists, and observations. The program reached almost 50% of eligible participants, had an attrition rate of 22%, and was adopted by 8.7% of the SCs approached. It was implemented as intended; only the nutritional component was re-designed due to participants' unfamiliarity with the mobile application. While there were no between-group differences in cognition, quality of life, and blood parameters, quality of life reduced in the control group and physical function improved in the intervention group after 24 weeks. The program was well-received by participants and SCs. Our findings show that a multi-domain program for at-risk older adults has benefits and can be implemented through neighborhood SCs. Areas of improvement are discussed.Trial registration: ClinicalTrials.gov NCT04440969 retrospectively registered on 22 June 2020.

Functional health index of intrinsic capacity: multi-domain operationalisation and validation in the Singapore Longitudinal Ageing Study (SLAS2)

Summary: Background ad hoc approaches are used to create composite indexes of intrinsic capacity (IC) based on five domains recommended by the World Health Organization for healthy ageing. We examined how combinations of domain-specific measures determine measurement performances of composite IC indexes. Results showed that among Singaporean older adults, cognition, sensory, and locomotion are predominant IC domains. A multi-domain IC index performs better with more domain measures, but a minimalist 3-domain index performs just as robustly as a 4- or 5-domain index.

Associations of low handgrip strength and hand laterality with cognitive function and functional mobility - the Yishun Study

Summary: The study found that low hand grip strength and asymmetry were associated with declining cognitive function and functional mobility in older adults. Low hand grip strength was independently associated with lower functional mobility and cognitive function, as well as immediate and delayed memory. The combination of low hand grip strength and asymmetry was associated with poorer language scores. However, the associations with cognitive and physical functions were primarily driven by grip strength rather than asymmetry.

The study found significant correlations between these parameters and age, making the reference data valuable in clinics for geriatric, obesity, and oncology patients. This study also contributes to the limited reference database on Southeast Asian body composition.

Summary: This study examines the protective effect of happiness on all-cause mortality risk and investigates the factors that contribute to this effect, as well as the potential gender differences. The results show that happiness is significantly associated with lower mortality, and this association remains even after considering other risk factors. Psychological health and functioning factors play a crucial role in the association between happiness and increased life expectancy.

Perceptions of Senior Center and Psychosocial Wellbeing During COVID-19

Risk Factors of Progression to Cognitive Frailty: Singapore Longitudinal Ageing Study 2

Summary: Cognitive frailty is associated with increased risk of dementia and adverse health outcomes. This study aims to investigate the risk factors for incident cognitive frailty. The results of the analysis using LASSO multivariable logistic regression models showed that eye problems and low HDL-cholesterol were risk factors for cognitive frailty transition, while higher levels of education and participation in cognitive stimulating activities were protective factors.
