Summary: Third-space endoscopy, especially peroral endoscopic myotomy (POEM), plays a significant role in therapeutic endoscopy. It not only provides a treatment option for achalasia but also shows promise in other esophageal motility disorders and as a salvage treatment. The prevention and management of gastroesophageal reflux after POEM have also seen major advancements. Moreover, POEM has led to the development of other submucosal tunneling procedures, expanding its application in various conditions.
