Vulvovaginal candidosis is a common condition in women, diagnosed based on clinical symptoms and microscopic detection. Confirmation of diagnosis is essential before treatment, with acute cases treated locally or orally, and chronic cases requiring maintenance therapy.
Approximately 70-75% of women will have vulvovaginal candidosis (VVC) at least once in their lifetime. In premenopausal, pregnant, asymptomatic and healthy women and women with acute VVC, Candida albicans is the predominant species. The diagnosis of VVC should be based on clinical symptoms and microscopic detection of pseudohyphae. Symptoms alone do not allow reliable differentiation of the causes of vaginitis. In recurrent or complicated cases, diagnostics should involve fungal culture with species identification. Serological determination of antibody titres has no role in VVC. Before the induction of therapy, VVC should always be medically confirmed. Acute VVC can be treated with local imidazoles, polyenes or ciclopirox olamine, using vaginal tablets, ovules or creams. Triazoles can also be prescribed orally, together with antifungal creams, for the treatment of the vulva. Commonly available antimycotics are generally well tolerated, and the different regimens show similarly good results. Antiseptics are potentially effective but act against the physiological vaginal flora. Neither a woman with asymptomatic colonisation nor an asymptomatic sexual partner should be treated. Women with chronic recurrent Candida albicans vulvovaginitis should undergo dose-reducing maintenance therapy with oral triazoles. Unnecessary antimycotic therapies should always be avoided, and non-albicans vaginitis should be treated with alternative antifungal agents. In the last 6 weeks of pregnancy, women should receive antifungal treatment to reduce the risk of vertical transmission, oral thrush and diaper dermatitis of the newborn. Local treatment is preferred during pregnancy.

Summary: This study aimed to treat pregnant women in the first trimester with intermediate bacterial microbiota imbalance by using a vaginally applied lactobacilli preparation to restore the normal microbiota and delay preterm delivery. The results showed that among women without lactobacilli, those who received treatment had a significant decrease in Nugent score and higher gestational age at delivery and neonatal birthweight compared to the untreated group.

Trends in caesarean section rates in Europe from 2015 to 2019 using Robson's Ten Group Classification System: A Euro-Peristat study

Summary: The study assessed changes in caesarean section (CS) rates in Europe from 2015 to 2019, and used the Robson Ten Group Classification System (TGCS) to evaluate the contribution of different obstetric populations to overall CS rates and trends. The findings showed that CS rates and trends varied among the 28 European countries. The TGCS analysis revealed that labour induction increased in most countries, while multiple pregnancies decreased. Countries with decreasing CS rates tended to see decreases across all TGCS groups, whereas countries with increasing rates tended to see increases in most groups. In countries with the greatest increase in CS rates, the absolute contribution of certain TGCS groups to the overall CS rate tended to increase.

Polymicrogyria in a patient after twin-twin transfusion syndrome

Summary: This case report presents a patient with a monochorionic twin pregnancy, development of twin-twin transfusion-syndrome (TTTS) and polymicrogyria (PMG) of one fetus. The patient underwent fetoscopic laser ablation at gestational week 16+1 due to TTTS grade 3. Sonographic follow-up revealed a cortical malformation in the former donor, identified as PMG through MRI scans. The report describes the course of the pregnancy and the clinical development of the child over 3 years, highlighting the power of neuroplasticity for a good neurological outcome despite extensive cortical malformation. Additionally, it stresses the importance of thorough prenatal imaging characterization for optimal prenatal counseling in cases of cortical developmental malformations.
