Summary: This paper explores the expectations and reported outcomes of patients undergoing nonsurgical root canal treatment (NS-RCT), nonsurgical root canal retreatment (NS-ReTx), and endodontic microsurgery (EMS). The study finds that patients value outcomes such as tooth survival, resolution of symptoms, aesthetics, and radiographic healing. Process-related factors are as important as treatment outcomes for patients. Effective communication and education during treatment can increase patient satisfaction and reduce stress. Dissatisfaction is linked to the lack of a comprehensive treatment and follow-up plan. Thorough planning ensures that patients are fully informed and have a structured approach to achieve their desired outcomes.

Summary: The study aimed to develop and validate machine learning models for predicting case difficulty in endodontic microsurgery, assisting clinicians in preoperative analysis. Four models were established using linear regression, support vector regression, and extreme gradient boosting algorithms. The XGBoost model outperformed the LR and SVR models on all evaluation metrics, providing important references for predicting surgical difficulty. Machine learning models can efficiently and accurately assist clinicians in preoperative analysis in endodontic microsurgery.

Summary: Paresthesia can occur as a consequence of unsuccessful root canal treatment, especially when it leads to apical periodontitis involving the mental neurovascular bundle. This case report presents a patient who experienced discomfort on mastication and persistent numbness of the lower left lip due to incomplete primary endodontic treatment. Cone-beam computed tomography imaging confirmed the involvement of the mental neurovascular bundle. Non-surgical root canal retreatment was performed and resolved the paresthesia after a 3-year follow-up. This suggests that nonsurgical endodontic retreatment may be effective in healing large periapical lesions and resolving complications like paresthesia. (J Endod 2023;49:920-924)

Summary: The present study aimed to evaluate the significance of the American Association of Endodontics (AAE) Case Difficulty Assessment on the occurrence of endodontic mishaps in an undergraduate student clinic. The results showed that cases with moderate-to-high difficulty experienced more mishaps and a greater number of treatment visits, while the type of instrumentation technique was not associated with mishap occurrence. Therefore, undergraduate students should use the AAE case classification assessment tool to reduce treatment mishaps and the number of visits.

Summary: This study evaluated the utility of an endodontic case difficulty assessment form (ECAF) for dental students and its association with RCT outcomes evaluated by a specialist. The results showed differences in assessments between students and supervisors in 55% of cases, with students often perceiving cases as easier than the teacher. Students found the ECAF user-friendly, although it did not demonstrate their competence in RCTs.

The study showed that hysteresis energy density can be used as a predictor of low cycle fatigue failure. Summary: The preparation of severely curved S-shaped double-curved root canals can be technically challenging. A novel root canal instrumentation technique is proposed, which modifies 2 techniques: the McSpadden crown-down technique and the tactile controlled activation technique. The proposed technique recommends 3 apical strokes after the initial engagement before withdrawal to minimize instrumentation time. This technique presents a new method of crown-down instrumentation that prevents overflaring and avoids instrument separation.
