The results suggest that AIEC colonization is associated with postoperative ileal lesions and endoscopic recurrence in CD patients. Additionally, AIEC colonization is linked to a specific microbiota signature.

New drugs before, during, and after hematopoietic stem cell transplantation for patients with acute myeloid leukemia

Summary: The treatment of acute myeloid leukemia (AML) has improved with the use of next-generation sequencing and targeted therapies. Allogeneic stem cell transplantation (allo-HCT) remains important for consolidating treatment and reducing relapse risk. Maintenance therapy following allo-HCT can further enhance the anti-leukemia effect without increasing the risk of complications. This paper summarizes novel therapies for AML before, during, and after allo-HCT and reviews ongoing studies.

Summary: Using single-cell genomics, it was found that high-risk events in multiple myeloma (MM) patients are often missed at diagnosis and only selected at relapse. This suggests the need for more aggressive treatment at diagnosis to prevent these aggressive cells from becoming dominant clones.

Summary: In this registry-based study, the outcomes of allogeneic hematopoietic cell transplantation (allo-HCT) in adult patients with acute lymphoblastic leukemia (ALL) were compared. The study found that the total dose of total body irradiation (TBI) did not influence the survival outcomes for patients, regardless of whether they received a standard 12-Gray or lower 8-Gray dose.

Summary: Systemic steroids are the first-line treatment for acute graft-versus-host disease, but about 50% of patients become steroid-refractory or dependent. Ruxolitinib is the only approved therapy for these patients and has shown superior efficacy compared with the best available therapy in a clinical trial.

Summary: This study evaluated the safety and efficacy of a novel recombinant human interleukin-22 (IL-22) dimer in combination with systemic corticosteroids for the treatment of newly diagnosed gastrointestinal acute graft-versus-host disease (GVHD). The results showed that the overall response rate of the treatment group at day 28 was 70%.

Summary: In this study, the impact of measurable residual disease (MRD) on the outcomes of post-transplant cyclophosphamide (PTCy)-based allo-HCT in acute myeloid leukemia (AML) patients in first complete remission (CR1) was assessed. The results showed that MRD positive (MRD+) patients had higher relapse rates and lower survival rates compared to MRD negative (MRD-) patients.

The results show that outcomes of allo-HCT in CR2 have significantly improved over time, with the most favorable outcomes achieved with a matched sibling donor (MSD).

Summary: A study on the association between the occurrence of GVHD and the relapse of AML in patients undergoing Haplo-HCT with PTCy-based GVHD prophylaxis found that while grade I acute GVHD was associated with better LFS, grade III-IV acute and extensive chronic GVHD were correlated with higher nonrelapse mortality and lower LFS.

Summary: This study aims to evaluate the effect of fecal microbiota transplantation (FMT) on toxicity in patients undergoing myeloablative allo-HSCT. The study will include 60 male and female patients, randomly assigned to two groups, one receiving FMT and one as a control group. The primary endpoint is the GvHD-free relapse-free survival rate at 1 year after allo-HSCT. The results of this study will be disseminated through peer-reviewed publications and conferences.

Summary: This study analyzed the gut microbiota of FMF patients and found differences compared to healthy controls, with increased pro-inflammatory bacteria in severe FMF patients. The severity of FMF and resistance to colchicine correlated with specific microbiota alterations. Colchicine treatment was associated with an expansion of anti-inflammatory taxa.

Summary: Previous studies have suggested a role for the CARD9 pathway in atherosclerosis, but the underlying mechanisms are still unclear. This study shows that the pro-atherogenic effects of Card9 deficiency are mediated by CD36-dependent defective autophagy, which can be reversed by rapamycin and metformin. Therefore, the CARD9 pathway is an important protective pathway in atherosclerosis.

Summary: This multicentre cohort study investigated the clinical outcome and course of inflammatory bowel disease (IBD) patients with recurrent Clostridioides difficile infection (CDI) who received fecal microbiota transplantation (FMT). The study found that FMT is safe and effective for treating rCDI in IBD patients, and IBD exacerbation after FMT is infrequent.
