Summary: This study evaluated the maternal and perinatal outcomes of pregnancies affected by SARS-CoV-2 infection, finding a higher risk of ICU admission for infected mothers but a negligible risk of vertical transmission.

Transversus abdominis plane block versus wound infiltration for post-cesarean section analgesia: A systematic review and meta-analysis of randomized controlled trials

Gaetano Riemma, Antonio Schiattarella, Stefano Cianci, Marco La Verde, Maddalena Morlando, Giovanni Sisti, Irene Esposito, Luigi Della Corte, Pasquale Sansone, Pasquale De Franciscis

Summary: There may be no significant advantages in choosing TAP block over WI for post-cesarean section analgesia, as there were no significant differences in terms of opioid consumption, adverse effects, and other outcomes between the two interventions.

Neutrophil-to-Lymphocyte Ratio and Platelet-to-Lymphocyte Ratio in Twins Compared with Singletons

Alexander Sabre, Giovanni Sisti, Kecia Gaither

Summary: This study compared NLR/PLR markers and blood cell components in twin pregnancies versus singleton pregnancies. The findings showed that NLR was higher in twins in the first trimester, while platelet levels were lower in twins compared to singletons in the second trimester. These results suggest that the unique physiological characteristics of twins may impact hematologic parameters.

Timing of delivery and blood pressure cut-off in chronic hypertension during pregnancy: State of art and new proposals

Giovanni Sisti, Antonio Schiattarella, Maddalena Morlando, Andrew Corwin

Summary: This study compared elective delivery and expectant management in pregnant women with chronic hypertension, finding randomized trials favored expectant management while observational studies favored induction of labor. Retrospective cohort studies on pregnant women with blood pressure lower than 140/90 mm Hg consistently showed increased risk of hypertensive disorders of pregnancy, gestational diabetes mellitus, and small-for-gestational-age neonates.

Summary: A meta-analysis of 5 trials found that Phloroglucinol favorably impacts on the total duration of the labor in primiparae and multiparae women with a singleton pregnancy.

Hispanic race is a risk factor for COVID-19 during pregnancy: data from an urban New York City hospital

Deena Elkafrawi, Giovanni Sisti, Felipe Mercado, Brian Rodriguez, Julie Joseph, Chandra Jones, Antonio Schiattarella, Ruchi Upadhyay

Summary: Limited studies have been conducted on the factors that predispose asymptomatic pregnant women to COVID-19 positivity. The results of a retrospective study on 263 asymptomatic pregnant women revealed that Hispanic race was a significant risk factor for COVID-19 infection during pregnancy. However, no variables were found to be significantly associated with COVID-19 positivity in the multivariate analysis. This study suggests that the cultural and socioeconomic factors of Hispanic women living in the community may contribute to a higher infection rate.

Preeclampsia: state of art and future perspectives. A special focus on possible preventions

Ozge Kahramanoglu, Antonio Schiattarella, Oya Demirci, Giovanni Sisti, Franco Pietro Ammaturo, Carlo Trotta, Federico Ferrari, Agnese Maria Chiara Rapisarda

Summary: Preeclampsia is a disease characterized by the development of hypertension after the 20th week of pregnancy, often accompanied by proteinuria or organ dysfunction. As delivery is the only definitive treatment, preeclampsia remains a leading cause of preterm birth and perinatal mortality. Strategies to reduce adverse outcomes focus on early prevention, surveillance, and intervention. Predictive models combining various factors have been developed to accurately identify women at higher risk. This review discusses the current and future perspectives on preeclampsia, including its pathogenesis, risk factors, clinical features, screening and prevention strategies, novel therapies, and healthcare management.

Is blood pressure 120-139/80-89 mmHg before 20 weeks a risk factor for hypertensive disorders of pregnancy? A meta-analysis

Giovanni Sisti, Cecilia Fochesato, Deena Elkafrawi, Brooke Marcus, Antonio Schiattarella

Summary: Hypertensive disorders of pregnancy affect a significant number of pregnant women globally and has serious implications for fetal and maternal health. This study found evidence of poor pregnancy outcomes in patients with blood pressure below the accepted cut-off of 140/90 mmHg, indicating the need for interventions to reduce the risk of hypertensive disorders in pregnancies with blood pressures ranging from 120-139/80-89 mmHg.

Neutrophil gelatinase-associated lipocalin as a predictor of pre-eclampsia: A systematic review and meta-analysis

Giovanni Sisti, Gal Rubin, Chi Zhou, Teresa Orth, Antonio Schiattarella

Summary: Several studies have found that protein neutrophil gelatinase-associated lipocalin (NGAL) in maternal blood is associated with the development of pre-eclampsia, suggesting that it could be used as a potential predictor.

Maternal Outcomes Following Active vs. Expectant Management of Previable Preterm Pre-Labor Rupture of Membranes: A Meta-Analysis

Megan A. Sylvester, Gabrielle Mintz, Giovanni Sisti

Summary: The diagnosis of previable preterm pre-labor rupture of membranes (PROM) is associated with poor outcomes for both the mother and the fetus. A meta-analysis was conducted to compare the effects of active management and expectant management on maternal risks. The results showed no significant differences in maternal risks between active management and expectant management for previable preterm PROM at <24 weeks.

Association between first trimester platelet to lymphocyte ratio and stillbirth

Giovanni Sisti, Debra Guinn, Melissa Magenta, Jessica Silva, Julie T. Joseph, Ruchi Upadhyay, Antonio Schiattarella

Summary: This study explored the association between neutrophil to lymphocyte ratio (NLR) and platelet to lymphocyte ratio (PLR) measured during pregnancy, and the occurrence of stillbirth (SB). The study found that a PLR higher than 156.4 in the first trimester can reliably exclude the possibility of SB later during pregnancy. Lower platelet and higher lymphocyte levels may be related to an early inflammatory process. Larger studies are needed to confirm the results.
