Genomic and Resistance Epidemiology of Gram-Negative Bacteria in Africa: a Systematic Review and Phylogenomic Analyses from a One Health Perspective

Antibiotic resistance (AR) remains a major threat to public and animal health globally. However, AR ramifications in developing countries are worsened by limited molecular diagnostics, expensive therapeutics, inadequate numbers of skilled clinicians and scientists, and unsanitary environments. The epidemiology of Gramnegative bacteria, their AR genes, and geographical distribution in Africa are described here. Data were extracted and analyzed from English-language articles published between 2015 and December 2019. The genomes and AR genes of the various species, obtained from the Pathosystems Resource Integration Center (PATRIC) and NCBI were analyzed phylogenetically using Randomized Axelerated Maximum Likelihood (RAxML) and annotated with Figtree. The geographic location of resistant clones/clades was mapped manually. Thirty species from 31 countries and 24 genera from 41 countries were analyzed from 146 articles and 3,028 genomes, respectively. Genes mediating resistance to beta-lactams (including bla(TEM-1), bla(CTX-M), bla(NDM), bla(IMP), bla(VIM), and bla(OXA)(-48/181)), fluoroquinolones (oqxAB, qnrA/B/D/S, gyrA/B, and parCE mutations, etc.), aminoglycosides (including armA and rmtC/F), sulfonamides (sul1/2/3), trimethoprim (dfrA), tetracycline (ter(A/B/C/D/G/O/M/39)], colistin (rncr-1), phenicols (catA/B, crnIA), and fosfomycin (fosA) were mostly found in Enterobacter spp. and Klebsiella pneumoniae, and also in Serratia marcescens, Escherichia coil, Salmonella enterica, Pseudomonas, Acinetobacter baumannii, etc., on mostly IncF-type, IncX(3/4), CoIRNAI, and IncR plasmids, within lnt/1 gene cassettes, insertion sequences, and transposons. Clonal and multiclonal outbreaks and dissemination of resistance genes across species and countries and between humans, animals, plants, and the environment were observed; Escherichia coil ST103, K. pneumoniae ST101, S. enterica ST1/2, and Vibrio cholerae ST69/515 were common strains. Most pathogens were of human origin, and zoonotic transmissions were relatively limited. IMPORTANCE Antibiotic resistance (AR) is one of the major public health threats and challenges to effective containment and treatment of infectious bacterial diseases worldwide. Here, we used different methods to map out the geographical hot spots, sources, and evolutionary epidemiology of AR. Escherichia coli, Klebsiella pneumoniae, Salmonella enterica, Acinetobacter baumannii, Pseudomonas aeruginosa, Enterobacter spp., Neisseria meningitis/gonorrhoeae, Vibrio cholerae, Campylobacter jejuni, etc., were common pathogens shuttling AR genes in Africa. Transmission of the same clones/strains across countries and between animals, humans, plants, and the environment was observed. We recommend Enterobacter spp. or K. pneumoniae as better sentinel species for AR surveillance.

Summary: This study investigated cephalosporin-resistant clinical strains of Escherichia coli in Mozambique, revealing a high prevalence of MDR pAmpC- and/or ESBL-producing isolates with FOX/MOX and CTX-Ms as the major beta-lactamase types. Genetic analysis showed diversity among strains with some clusters indicating within-hospital spread. These findings emphasize the urgent need for accurate diagnostics and infection control measures.

Global evolutionary epidemiology and resistome dynamics of Citrobacter species, Enterobacter hormaechei, Klebsiella variicola, and Proteeae clones

Summary: Citrobacter spp., Enterobacter hormaechei subsp., Klebsiella variicola and Proteae tribe members are rare Enterobacterales increasingly implicated in nosocomial infections. These species, containing multiple antibiotic resistance genes, are widely distributed globally and can easily transmit these genes to more pathogenic species. Surveillance studies are necessary to identify and contain these antibiotic-resistant species.

Summary: Despite the global recommendation of the safe and effective measles vaccine since 1974, there were over 17 million cases of measles in 2017, with the majority occurring in low- and middle-income countries. A study from 2000 to 2019 in 101 LMICs showed that after gains in measles vaccine coverage from 2000 to 2010, there was regression in coverage in over half of the districts from 2010 to 2019, with rural areas having lower coverage than urban areas.

Current and emerging polymyxin resistance diagnostics: A systematic review of established and novel detection methods

Summary: The emergence of polymyxin resistance poses a threat to public and animal health due to transferable mcr genes. Current and emerging polymyxin resistance diagnostics include phenotypic and molecular methods, with Rapid ResaPolymyxin NP test and Fastinov (R) recommended for initial screening due to their fast turnaround time, high sensitivity, and specificity.

Prevalence of drug resistance-conferring mutations associated with isoniazid- and rifampicin-resistant Mycobacterium tuberculosis in Ethiopia: a systematic review and meta-analysis

Summary: In Ethiopia, high prevalence of drug-resistant gene mutations in Mycobacterium tuberculosis (Mtb) is observed, especially in genes related to rifampicin (RIF) and isoniazid (INH) resistance. Tracking specific gene mutations can enhance the diagnostic and epidemiological value of tuberculosis control.

Phylogenomics, epigenomics, virulome and mobilome of Gram-negative bacteria co-resistant to carbapenems and polymyxins: a One Health systematic review and meta-analyses

Summary: This study analyzed Gram-negative bacteria with co-resistance to carbapenems and polymyxins from a One Health perspective. The results showed that these bacteria were widely reported in Asian countries and isolated from clinical, animal, and environmental sources. Colistin resistance was mainly mediated by mgrB inactivation and mcr-1 gene, while OXA-48/181 was the most common carbapenemase. The study also identified bacteria co-resistant to other antibiotics and detected virulence genes and capsule typing.

Prevalence of multidrug-resistant tuberculosis in East Africa: A systematic review and meta-analysis

Summary: The study aimed to estimate the pooled prevalence of multidrug-resistant tuberculosis among newly diagnosed and previously treated pulmonary tuberculosis cases in East African countries. The results showed a significant prevalence of multidrug-resistant tuberculosis in the region, with 4% among newly diagnosed cases and 21% among previously treated cases. Living conditions, lifestyles, previous medical history, diabetes history, and HIV infection were identified as risk factors contributing to the higher prevalence of multidrug-resistant tuberculosis in East Africa.

The results showed a high prevalence of carbapenemase genes and a low prevalence of mcr genes in Klebsiella pneumoniae, highlighting the need for careful use of antibiotics to prevent further escalation and outbreak of multi-drug resistant infections.

Global epidemiology, genetic environment, risk factors and therapeutic prospects of mcr genes: A current and emerging update

Summary: This research found that mcr genes are widely distributed in Enterobacteriaceae species and pose a threat to public health as they are mainly transmitted to humans through food animals. The dissemination of mcr genes is mediated by mobile genetic elements and there are compensatory adaptation mechanisms. Combination therapy is a promising option for managing and treating colistin-resistant Enterobacteriaceae infections.

Profile and Frequency of Mutations Conferring Drug-Resistant Tuberculosis in the Central Southeastern and Eastern Ethiopia

Summary: This study aimed to determine the frequency and type of mutations responsible for resistance to rifampicin, isoniazid, fluoroquinolones, and second-line injectable drugs in tuberculosis patients. The results showed that mutations conferring drug resistance were common among the isolates tested. Unknown mutations were also observed, indicating the need for further research to understand the complete spectrum of mutations.

Species-Specific Analysis of Bacterial Vaginosis-Associated Bacteria

Summary: Since 2005, the pathogen BVAB has been identified in the female vaginal tract using PCR, but its actual species was unknown. Without the full genome of these pathogens, we cannot fully understand their pathogenicity, treatment, resistance, and diagnostics. Through sequence homology and phylogenetics analysis, we have identified the actual species of BVAB-1, -2, and -3 and found them to be anaerobic and uncultured species that can only be identified through metagenomics.
