Herpes Zoster Radiculomyelitis With Aquaporin-4 Antibodies: A Case Report and Literature Review

Background: The relationship between varicella-zoster virus (VZV)-associated myelitis and aquaporin-4 immunoglobulin-G (AQP4-IgG) remains unknown. Case Report: We report a case of acute radiculomyelitis with longitudinal extensive hyperintensity signals traversing the brainstem until the upper thoracic cord in a 55-year-old healthy woman following herpes zoster infection in the left C4-T3 dermatome. VZV-specific IgG in the cerebrospinal fluid (CSF) and AQP4-IgG positivity on enzyme-linked immunosorbent assay (ELISA) were undetectable. Thus, she was diagnosed with immune-competent VZV radiculomyelitis. Forty-two months later, she experienced a relapse, and AQP4-IgG positivity was detected on ELISA. A cell-based assay (CBA) showed AQP4-IgG positivity not only at the time of recurrence but also retrospectively at 1 month after the initial symptoms. We concluded that AQP4-IgG-positive neuromyelitis optica spectrum disorder (NMOSD) was concurrent with VZV myelitis. After the second attack, she was treated with azathioprine and has had no relapse since then. Conclusion: We reported a case of VZV radiculomyelitis with confirmed concurrent AQP4-IgG positivity. NMOSD induced by herpes zoster has been recently identified, but distinguishing it from VZV myelitis can be difficult and whether these two diseases aggravate each other is unknown. Awareness of the potentially varied presentation of VZV myelitis can enable earlier recognition and proper treatment.

Cerebral artery dissection secondary to antiphospholipid syndrome: A report of two cases and a literature review

Summary: Antiphospholipid syndrome (APS) is an autoimmune disorder characterized by thromboembolic events, including ischemic stroke or complications in pregnancy. Cervical artery dissection (CAD) is a common cause of stroke in young adults, and the concomitant presence of APS and CAD is extremely rare. Two rare cases of ischemic stroke caused by CAD secondary to APS were described in this report, along with a review of the literature indicating specific mechanisms for cerebral artery disorders secondary to APS.

White matter alterations in Parkinson's disease with levodopa-induced dyskinesia

Summary: This study aimed to investigate white matter alterations in Parkinson's disease patients with and without levodopa-induced dyskinesia using advanced diffusion magnetic resonance imaging techniques. The results showed significant differences in white matter microstructural disruption, especially in temporal lobe fibers, between patients with and without levodopa-induced dyskinesia. The tracts in temporal lobe fibers had a more than 2-fold odds ratio for the presence of levodopa-induced dyskinesia, suggesting a potential association with the pathogenesis of the sequela.

Non-invasive diagnostic tool for Parkinson's disease by sebum RNA profile with machine learning

Summary: This study utilized RNA in skin surface lipids for transcriptome analysis and demonstrated the potential of these expression profiles with machine learning as diagnostic biomarkers for Parkinson's disease. Differential gene expression analysis identified more than 100 differentially expressed genes between PD patients and healthy controls, with several genes related to oxidative phosphorylation being upregulated. Machine learning using gene expression information from skin surface lipids could efficiently discriminate PD patients from healthy controls, with an area under the receiver operating characteristic curve of 0.806.

Summary: This study investigated the association between serum omega-3 polyunsaturated fatty acids and intracerebral hemorrhage during the COVID-19 pandemic. The results showed that patients with intracerebral hemorrhage during the pandemic had significantly lower levels of serum eicosapentaenoic acid compared to before. Low levels of eicosapentaenoic acid might serve as an endogenous surrogate marker for intracerebral hemorrhage during the COVID-19 pandemic.
