Delirium: A Frequent Manifestation in COVID-19 Older Patients

The authors report a high prevalence of delirium in COVID-19 old patients admitted in an academic hospital. During the recent COVID-19 period, delirium was present in 38% of old patients admitted with delirium at the COVID ward of the Erasmus MC University Medical Center of Rotterdam. We do not know in which patients COVID-19 can cause delirium; however, considering the high prevalence of delirium in COVID-19 old patients and the potential serious consequences, attention is needed in order to reduce disability and mortality in this vulnerable category of patients.

Anticholinergic Drug Use on Admission and the Risk of In-Hospital Falls in Older Hospitalized Patients

Summary: The study investigated the association between anticholinergic drug exposure on admission and in-hospital falls. The results showed that each additional anticholinergic drug in use was associated with an increased odds of experiencing a fall. Other measures of anticholinergic drug use were not associated with in-hospital falls.

Summary: The study explores the impact of functional impairments on the stage at which mortality increases in older individuals with chronic kidney disease (CKD). The results show that physical performance, such as handgrip strength, can help identify patients at a higher risk of mortality, even with mild to moderate impairment in kidney function. These findings improve the predictive accuracy of individual measures of kidney function.

Summary: This study compared baseline characteristics, anti-inflammatory drug use, and DMARD use patterns among RA patients in Southern Italy and the United States. The findings showed that most RA patients, especially elderly patients, were treated with anti-inflammatory drugs and csDMARDs rather than bDMARDs/tsDMARDs during the first year post-diagnosis, highlighting the need for better management of barriers in the use of these agents.

Short-term and long-term increased mortality in elderly patients with burn injury: a national longitudinal cohort study

This suggests that elderly patients are at higher risk of death following burn injuries.

The Effect of Age on Non-Invasive Hemodynamics in Chronic Heart Failure Patients on Left-Ventricular Assist Device Support: A Pilot Study

Summary: This study aimed to investigate the differences in non-invasive hemodynamics in LVAD patients of different age categories and their association with adverse events. The results showed that older LVAD patients had higher heart rate, heart rate-adjusted augmentation index, and pulse wave velocity, and lower stroke volume compared to younger participants. Patients with adverse events had lower central pulse pressure compared to those without any adverse event.

Summary: Cognitive impairment is common in patients with chronic kidney disease (CKD). This article reviews interventions for the complications of CKD and prevention of vascular events, which may potentially also be protective against cognitive impairment. Nonpharmacological and pharmacological methods to prevent cognitive impairment and/or minimize its impact on CKD patients' daily lives are discussed. It is necessary to conduct studies assessing the effect of interventions on the cognitive function of patients with CKD.

Summary: This study evaluated the effectiveness of the Describe, Investigate, Create, Evaluate (DICE) method in improving neuropsychiatric symptoms (NPS) in Alzheimer's disease (AD) at the memory clinic. The results showed that the method did not provide significant benefits at the group level, but individuals with high levels of NPS and NPS-related distress may benefit from this intervention.

Summary: This study investigated the age and sex trajectories, regional differences, and global reference values of arterial stiffness. The results showed that the global age-standardized means of pulse wave velocity were 12.5 m/s for brachial-ankle PWV and 7.45 m/s for carotid-femoral PWV. Males had higher levels than females, and there were regional differences across different countries.

Quality of Life and Kidney Function in Older Adults: Prospective Data of the SCOPE Study

Summary: This study investigated the longitudinal alteration in health-related quality of life (HRQoL) over a two-year period among older adults with early-stage chronic kidney disease (CKD) and its association with CKD progression. The results showed that 41% of the participants experienced a decline in HRQoL, and 16.3% showed kidney function decline. Participants with HRQoL decline had higher levels of depression and greater decline in physical performance. The decline in kidney function did not contribute to the decline in HRQoL, but higher levels of depression and better physical performance were associated with less HRQoL decline.

New horizons in cognitive and functional impairment as a consequence of cerebral small vessel disease

Summary: Cerebral small vessel disease (cSVD) is commonly seen in brain imaging of older adults, especially those with cardiovascular disease risk factors. The link between cSVD and cognitive impairment is established, but how and when these vascular changes lead to cognitive decline is still uncertain. Technological advances allow for more accurate identification and quantification of cSVD, which can aid in diagnosing vascular cognitive impairment and identifying individuals at higher risk of cognitive and functional deterioration. This study explores how improved assessment of cSVD through neuropsychological testing, comprehensive geriatric assessment, and modern imaging techniques can enhance diagnosis and prognosis of cognitive impairment. The study also discusses new treatment approaches for cSVD and how these insights can optimize screening and treatment strategies for older adults with cognitive impairment and multiple morbidities.

Atypical presentation of COVID-19 in older patients is associated with frailty but not with adverse outcomes

Summary: The aim of this study was to investigate the association between atypical presentation of COVID-19, frailty, and adverse outcomes, as well as the incidence of atypical presentation. The results showed that atypical presentation of COVID-19 was significantly associated with frailty, but not with worse disease outcomes. Therefore, physicians need to remain vigilant for COVID-19 in frail older patients, as they may not present with typical symptoms.
