What is the risk of prostate cancer mortality following negative systematic TRUS-guided biopsies? A systematic review

Objective To investigate the risk of prostate cancer-specific mortality (PCSM) following initial negative systematic transrectal ultrasound-guided (TRUS) prostate biopsies. Design Systematic review. Data sources PubMed and Embase were searched using a string combination with keywords/Medical Subject Headings terms and free text in the search builder. Date of search was 13 April 2020. Study selection Studies addressing PCSM following initial negative TRUS biopsies. Randomised controlled trials and population-based studies including men with initial negative TRUS biopsies reported in English from 1990 until present were included. Data extraction Data extraction was done using a predefined form by two authors independently and compared with confirm data; risk of bias was assessed using the Newcastle-Ottawa Scale for cohort studies when applicable. Results Four eligible studies were identified. Outcomes were reported differently in the studies as both cumulative incidence and Kaplan-Meier estimates have been used. Regardless of the study differences, all studies reported low estimated incidence of PCSM of 1.8%-5.2% in men with negative TRUS biopsies during the following 10-20 years. Main limitation in all studies was limited follow-up. Conclusion Only a few studies have investigated the risk of PCSM following initial negative biopsies and all studies included patients before the era of MRI of the prostate. However, the studies point to the fact that the risk of PCSM is low following initial negative TRUS biopsies, and that the level of prostate-specific antigen before biopsies holds prognostic information. This may be considered when advising patients about the need for further diagnostic evaluation. PROSPERO registration number CRD42019134548.

Lower urinary tract injuries in patients with pelvic fractures at a level 1 trauma center - an 11-year experience

Summary: This retrospective study aimed to describe the incidence, diagnosis, treatment, and morbidity of lower urinary tract injuries in patients with pelvic fractures. The results showed inconsistent management and severe complications in urethral injuries, while long-term complications mainly occurred in patients with complete urethral ruptures. The treatment and follow-up of urethral injuries were found to be unsystematic even in an experienced medical center.

Pituitary-testis axis dysfunction following adjuvant androgen deprivation therapy

Summary: Men with high-risk, non-metastatic prostate cancer receive adjuvant androgen deprivation therapy (ADT) for at least 2 years according to Danish guidelines. It remains unclarified if patients regain the function of the pituitary-testis axis after cessation of ADT. Our study investigated the function of the pituitary-testis axis following adjuvant ADT and found that persistent testosterone deficiency (TD) is a common long-term consequence, equally distributed between pituitary and testicular dysfunction. The study emphasizes the necessity for systematic follow-up of full pituitary-testis axis function in patients receiving adjuvant ADT.

The primary objective is to study the rate of unblinding after surgery and the number of participants who cannot guess the surgical technique. Summary: This study aimed to investigate the risk of new-onset cardiovascular events for patients with stage I-III colorectal cancer who received elective surgery compared to a matched population. The study found that patients operated on for colorectal cancer had an increased risk of heart failure compared to cancer-free individuals. Several potential risk factors for cardiovascular events were also identified.

Summary: This study compared the effects of two different blood pressure management strategies, hypotension-avoidance and hypertension-avoidance, on major vascular complications after noncardiac surgery. The results showed that both strategies had similar incidence of major vascular complications.

Standardized prostate cancer incidence and mortality rates following initial non-malignant biopsy result

Summary: A study in Denmark involving 37,231 men who had an initial non-malignant transrectal ultrasonography (TRUS) biopsy showed that these men had a higher incidence of subsequent prostate cancer but a lower risk of prostate cancer death compared to the general population. It suggests that the oncological risk of cancers missed in the initial TRUS biopsy is low, and attempts to increase biopsy sensitivity are unwarranted. Current follow-up after a non-malignant biopsy may be overaggressive, especially for men over the age of 60.

Danish Prostate Registry (DanProst) - an Updated Version of the Danish Prostate Cancer Registry, Methodology, and Early Results

Summary: This article introduces the Danish Prostate Cancer Registry (DaPCaR) and the Danish Prostate Registry (DanProst), the latter being a comprehensive centralized registry that includes all men who have undergone histological evaluation of the prostate tissue. DanProst is built on the assessment data from the Danish National Registry for Pathology and integrates data from multiple national registries.

Incidence of and Risk Factors for Recurrent Urinary Tract Infections in Renal Transplant Recipients

Summary: This study investigates the incidence and risk factors for recurrent urinary tract infections (rUTIs) in renal transplant recipients and evaluates the impact on graft and patient survival. The results show that one in six patients experience rUTIs after renal transplantation, and multiple factors such as age, gender, history of lower urinary tract symptoms, and UTI within 30 days of surgery contribute to rUTIs. However, rUTIs do not affect graft function and patient survival.

Impact of previous depression on the risk of suicide among prostate cancer patients

Summary: The study examined the risk of suicide among prostate cancer patients compared to cancer-free men, taking into consideration clinical stage, socioeconomic position, somatic comorbidity, and previous depression. The risk of suicide was found to be similar between prostate cancer patients and cancer-free men, but it was highest within the first year after diagnosis. Prostate cancer patients with a history of depression had a higher risk of suicide. Healthcare professionals should pay attention to the risk of suicide among men with previous depression following a prostate cancer diagnosis.

First experience with the Hugo™ robot-assisted surgery system for endometriosis: A descriptive study

The advent of new robotic systems is welcomed to advance surgical care for patients worldwide.
