Summary: The aim of this study was to evaluate the relationship between comorbidities, molecular landscape, and outcome in acute myeloid leukemia (AML) patients aged 60 years or older who underwent intensive induction therapy. Next-generation sequencing identified 323 mutations in 77 genes among 91 patients, with NPM1, FLT3, TET2, and DNMT3A being the most frequently mutated genes. Different mutations were associated with achieving complete remission, primary refractory disease, and mortality after relapse. Short-term outcome was best predicted by Ferrara unfitness criteria and performance status, while genomic classifications were better predictors of long-term outcome. It is suggested that specific prognostic classifications should be designed for AML patients over 60 years, taking into consideration performance status, comorbidities, and cytogenetic/molecular data.
