Defining and managing COVID-19-associated pulmonary aspergillosis: the 2020 ECMM/ISHAM consensus criteria for research and clinical guidance

Severe acute respiratory syndrome coronavirus 2 causes direct damage to the airway epithelium, enabling aspergillus invasion. Reports of COVID-19-associated pulmonary aspergillosis have raised concerns about it worsening the disease course of COVID-19 and increasing mortality. Additionally, the first cases of COVID-19-associated pulmonary aspergillosis caused by azole-resistant aspergillus have been reported. This article constitutes a consensus statement on defining and managing COVID-19-associated pulmonary aspergillosis, prepared by experts and endorsed by medical mycology societies. COVID-19-associated pulmonary aspergillosis is proposed to be defined as possible, probable, or proven on the basis of sample validity and thus diagnostic certainty. Recommended first-line therapy is either voriconazole or isavuconazole. If azole resistance is a concern, then liposomal amphotericin B is the drug of choice. Our aim is to provide definitions for clinical research and up-to-date recommendations for clinical management of the diagnosis and treatment of COVID-19-associated pulmonary aspergillosis.

Summary: This study assessed the infection prevention and control practices in hospitals participating in the nationally representative HAI surveillance network in India. The results indicated that the scores for programmed IPC activities, implementation of health care workers immunization program, and monitoring and evaluation component were relatively low in the participating hospitals. Therefore, improvement is needed in terms of human resources, surveillance of HAIs, and monitoring and evaluation components.

Summary: The study aimed to determine the epidemiological cut-off values (ECVs) of ten antifungal agents in a wide range of yeasts and Aspergillus spp. using gradient concentration strips. Retrospective collection of minimum inhibitory concentrations for various antifungal agents was done from 35 French microbiology laboratories between 2002 and 2020. The calculated ECVs can aid in interpreting minimum inhibitory concentrations and identifying non-wild type isolates.

Summary: The study used data from the Mycosands survey to investigate the factors affecting the distribution of yeasts and molds along European shores. Environmental factors such as climate, soil properties, and water conditions were analyzed using species distribution modelling. The results showed that yeasts and molds have different distributions along European shores, with yeasts being more tolerant of low temperatures and molds showing affinity with specific soil compositions and water conditions.

Short Tandem Repeat Genotyping and Antifungal Susceptibility Testing of Latin American Candida tropicalis Isolates

Summary: Candida tropicalis is a common opportunistic pathogen causing infections in Latin America, and the rise of antifungal-resistant isolates is becoming a concern. This study utilized short tandem repeat (STR) genotyping and antifungal susceptibility testing (AFST) to investigate the population genomics and resistance patterns of 230 C. tropicalis isolates. The results revealed outbreak events, the presence of anidulafungin-resistant isolates, and azole resistance-associated substitutions. The findings highlight the importance of this STR genotyping scheme for identifying outbreaks and understanding the spread of antifungal-resistant C. tropicalis isolates.

Increasing influenza vaccination coverage in healthcare workers: analysis of an intensified on-site vaccination campaign during the COVID-19 pandemic

Summary: This study investigates the impact of an intensified influenza vaccination campaign in a maximum-care hospital on influenza vaccination coverage in healthcare workers during the COVID-19 pandemic in 2020/21. The campaign, which involved a mobile vaccination team and a central vaccination site, led to a significant increase in vaccination coverage among healthcare workers, especially among nurses.

Summary: Patients with haematological malignancies are at high risk of invasive fungal disease. The German Society of Haematology and Medical Oncology has updated their antifungal prophylaxis recommendations, recommending continued antifungal prophylaxis for patients with long-lasting neutropenia and posaconazole as the drug of choice. There is insufficient data to give general recommendations for antifungal prophylaxis in patients receiving novel treatments for HM, such as CAR-T-cell therapy or novel targeted therapies for AML.

Metagenomic Next-Generation Sequencing of Plasma for Diagnosis of COVID-19-Associated Pulmonary Aspergillosis

Summary: Timely diagnosis remains an unmet need in non-neutropenic patients at risk for aspergillosis, including those with COVID-19-associated pulmonary aspergillosis (CAPA). Currently available mycological tests show limited sensitivity when testing blood specimens. Metagenomic next-generation sequencing (mNGS) to detect microbial cell-free DNA (mcfDNA) in plasma might overcome some of the limitations of conventional diagnostics.

Summary: This study demonstrated that low concentrations of Candida haemulonii EVs are not recognized by the classical pathway of the oxidative burst generated by macrophages, allowing them to transport virulence factors and evade host immune system. However, high concentrations of EVs activated microbicidal actions in macrophages. Therefore, EVs may contribute to the virulence of Candida haemulonii and could be potential therapeutic targets.

The Influence of Medium Composition on EUCAST and Etest Antifungal Susceptibility Testing

Summary: There is an ongoing effort to optimize and revise antifungal susceptibility testing methods due to the rising number of fungal infections and drug-resistant fungi. This study found that different media could affect the antifungal susceptibility results of Candida and Aspergillus species, with Candida species exhibiting higher variability. Additionally, the EUCAST method was more sensitive in detecting MIC variations compared to the Etest method.

Aspergillus terreus Antibody Serosurveillance in Tyrol: A Population-Based, Cross-Sectional Study of a Healthy Population

Summary: The rare and emerging mold Aspergillus terreus is an important pathogen in certain geographical areas, such as Tyrol (Austria) and Houston (Texas). This study aimed to evaluate the trends in A. terreus-specific IgG antibodies in different regions of Tyrol and compare them with the environmental spread of A. terreus. The results showed significant differences between the Tyrolean Upland and Lowland, with higher antibody levels in females. These findings support that A. terreus IgG antibody levels reflect the environmental epidemiology of A. terreus in Tyrol.
