Ulcerative colitis endoscopic index of severity is associated with long-term prognosis in ulcerative colitis patients treated with infliximab

Background and Aim: Ulcerative colitis (UC) is a chronic inflammatory bowel disease that is characterized by periods of remission and episodes of relapse. Mucosal healing is an emerging therapeutic target in UC and various scoring systems have been used. The UC endoscopic index of severity (UCEIS) is the only validated endoscopic index at present, with minimum interobserver variation. Correlation of UCEIS scores after treatment and clinical outcomes of UC has not been examined. In the present study, we aimed to evaluate the usefulness of UCEIS after treatment with infliximab. Methods: The medical records of 82 UC patients, treated with infliximab at Keio University Hospital between October 2010 and July 2013, were reviewed retrospectively. Endoscopic findings were evaluated based on the UCEIS. Results: Mean pre-therapeutic UCEIS score was 5.1. Pre-therapeutic UCEIS scores were not associated with short-term-outcomes. Forty-five patients underwent colonoscopy at 3-12 months after starting treatment; mean post-therapeutic UCEIS score was 2.4, with a score of 0-1 in 16 (35.6%) patients, 2-4 in 19 (42.2%) patients, and 5-8 in 10 (22.2%) patients. Importantly, a post-therapeutic UCEIS score of 0 or 1 after treatment was associated with a favorable long-term outcome. Conclusion: UCEIS score is a useful instrument for evaluating endoscopic improvement in UC patients treated with infliximab, and mucosal healing may be defined with a UCEIS score of 0 or 1.

Summary: A variant near the DOCK2 gene on chromosome 5 is associated with severe COVID-19 in patients under 65 years old, with this risk allele being more prevalent in East Asian individuals and rare in Europeans. Decreased expression of DOCK2 is observed in younger patients with severe cases of COVID-19, particularly in non-classical monocytes. Suppressed DOCK2 expression is also found in lung specimens from severe COVID-19 pneumonia patients. Inhibition of DOCK2 function exacerbates the severity of pneumonia in a hamster model of SARS-CoV-2 infection. These findings suggest that DOCK2 plays an important role in the host immune response to SARS-CoV-2 infection and the development of severe COVID-19.

Usefulness of texture and color enhancement imaging in assessing mucosal healing in patients with ulcerative colitis

Summary: This study aimed to evaluate the usefulness of texture and color enhancement imaging (TXI) in assessing the degree of inflammation in patients with ulcerative colitis (UC). The results showed that patients with a TXI score of 2 had significantly lower UC relapse rates compared to those with TXI scores of 0-1. TXI score was significantly associated with pathologic scores, and a TXI score of 2 was identified as the only risk factor for UC relapse. Therefore, TXI can be used to identify patients with poor prognosis in MES 1 and provide evidence for controversial treatment intensification.

Summary: This study evaluated the clinical characteristics of perianal lesions in newly diagnosed patients with Crohn's disease (CD) and investigated the impact of perianal lesions on the quality of life in Japanese patients with CD. The results showed that nearly half of the newly diagnosed CD patients had perianal lesions, with perianal fistulas and abscesses being the most common. Young age, male sex, disease location, and behavior were significantly associated with the presence of perianal lesions, while stricturing behavior and alcohol intake were associated with a lower prevalence. Perianal lesions were associated with fatigue and impairment of daily activities.

Etrasimod for ulcerative colitis: evaluating phase III results

Summary: This study aimed to determine the risk of venous thromboembolism (VTE) with central venous catheter (CVC) placement in hospitalized Japanese patients with inflammatory bowel disease (IBD) without thromboprophylaxis. The results showed that CVC placement is a major risk factor for VTE among hospitalized Japanese patients with IBD.

Vaccination in patients with inflammatory bowel disease-Asian perspectives: results of a multinational web-based survey in the 8th Asian Organization for Crohn's and Colitis meeting

Summary: This study investigated the current approaches and practices of physicians regarding vaccinations for inflammatory bowel disease (IBD) patients in different Asian countries/regions. The results showed similarities among countries/regions in terms of recognizing the importance of vaccination, but there were differences in the selection and recommendation of certain vaccines, reflecting each country's vaccination guidelines and health insurance. Therefore, more communication among doctors and a consensus regarding IBD vaccination differences in the Asian region are needed.
