Targeting the Four Pillars of Enterohepatic Bile Salt Cycling; Lessons From Genetics and Pharmacology

Bile salts are essential for lipid homeostasis and can be both culprits and potential remedies in gut or liver disorders. Efficient transporters mediate the uptake and efflux of bile salts, ensuring their enterohepatic circulation. Deficiencies in key transporters can lead to pathological conditions, and targeting these pathways may offer new therapeutic opportunities.
Bile salts play a pivotal role in lipid homeostasis, are sensed by specialized receptors, and have been implicated in various disorders affecting the gut or liver. They may play a role either as culprit or as potential panacea. Four very efficient transporters mediate most of the hepatic and intestinal bile salt uptake and efflux, and are each essential for the efficient enterohepatic circulation of bile salts. Starting from the intestinal lumen, conjugated bile salts cross the otherwise impermeable lipid bilayer of (primarily terminal ileal) enterocytes through the apical sodium-dependent bile acid transporter (gene SLC10A2) and leave the enterocyte through the basolateral heteromeric organic solute transporter, which consists of an alpha and beta subunit (encoded by SLC51A and SLC51B). The Na+-taurocholate cotransporting polypeptide (gene SLC10A1) efficiently clears the portal circulation of bile salts, and the apical bile salt export pump (gene ABCB11) pumps the bile salts out of the hepatocyte into primary bile, against a very steep concentration gradient. Recently, individuals lacking either functional Na+-taurocholate cotransporting polypeptide or organic solute transporter have been described, completing the quartet of bile acid transport deficiencies, as apical sodium-dependent bile acid transporter and bile salt export pump deficiencies were already known for years. Novel pathophysiological insights have been obtained from knockout mice lacking functional expression of these genes and from pharmacological transporter inhibition in mice or humans. Conclusion: We provide a concise overview of the four main bile salt transport pathways and of their status as possible targets of interventions in cholestatic or metabolic disorders.

Waitlist mortality of young patients with biliary atresia: Impact of allocation policy and living donor liver transplantation

Summary: The Eurotransplant (ET) allocation prioritization for patients with biliary atresia (BA) younger than 2 years has significantly reduced the waitlist mortality by decreasing the proportion of deceased donor liver transplantations (DDLT) and increasing the proportion of living donor liver transplantations (LDLT). The waitlist mortality decreased from 6.7% to 2.3% after the implementation of the new allocation rule, and the median waitlist duration also decreased.

Longitudinal relationship between albuminuria in infancy and childhood

Summary: Mildly increased albuminuria is common in adults and 2-year-old children. This study aims to evaluate the longitudinal persistence of albuminuria from infancy to school age, and found that albuminuria at 2 years of age does not largely persist until the age of 12.

Summary: The aim of this study was to assess neurodevelopment in young patients with biliary atresia (BA) and determine the predictive value of General Movement Assessment (GMA) at infant age for neurodevelopmental impairments at toddler age. The results showed that one-third of toddlers with BA exhibited impaired motor skills. GMA after Kasai porto-enterostomy (KPE) had a high predictive value for identifying BA infants at risk of neurodevelopmental impairments.

Summary: This study aimed to evaluate the relationship between genetic mutations related to BSEP deficiency and clinical features. The results showed that the combination of one relatively mild mutation and one severe mutation resulted in a clinical presentation similar to patients with two severe mutations, and they had a poor response to surgical interruption of the enterohepatic circulation.

Albuminuria and markers for cardiovascular risk in 12-year-olds from the general Dutch population: a cross-sectional study

Summary: This study aims to evaluate albuminuria in a population-based pediatric cohort and investigate its association with cardiovascular risk factors. The results show a lower prevalence of albuminuria in 12-year-olds compared to previous studies, and it is negatively associated with body mass index. Additionally, there is no significant association between albuminuria and antenatal factors.
