Chest CT-derived pulmonary artery enlargement at the admission predicts overall survival in COVID-19 patients: insight from 1461 consecutive patients in Italy

Enlarged main pulmonary artery diameter (MPAD) is an independent predictor of mortality in COVID-19 patients.
Objectives Enlarged main pulmonary artery diameter (MPAD) resulted to be associated with pulmonary hypertension and mortality in a non-COVID-19 setting. The aim was to investigate and validate the association between MPAD enlargement and overall survival in COVID-19 patients. Methods This is a cohort study on 1469 consecutive COVID-19 patients submitted to chest CT within 72 h from admission in seven tertiary level hospitals in Northern Italy, between March 1 and April 20, 2020. Derivation cohort (n = 761) included patients from the first three participating hospitals; validation cohort (n = 633) included patients from the remaining hospitals. CT images were centrally analyzed in a core-lab blinded to clinical data. The prognostic value of MPAD on overall survival was evaluated at adjusted and multivariable Cox's regression analysis on the derivation cohort. The final multivariable model was tested on the validation cohort. Results In the derivation cohort, the median age was 69 (IQR, 58-77) years and 537 (70.6%) were males. In the validation cohort, the median age was 69 (IQR, 59-77) years with 421 (66.5%) males. Enlarged MPAD (>= 31 mm) was a predictor of mortality at adjusted (hazard ratio, HR [95%CI]: 1.741 [1.253-2.418], p < 0.001) and multivariable regression analysis (HR [95%CI]: 1.592 [1.154-2.196], p = 0.005), together with male gender, old age, high creatinine, low well-aerated lung volume, and high pneumonia extension (c-index [95%CI] = 0.826 [0.796-0.851]). Model discrimination was confirmed on the validation cohort (c-index [95%CI] = 0.789 [0.758-0.823]), also using CT measurements from a second reader (c-index [95%CI] = 0.790 [0.753;0.825]). Conclusion Enlarged MPAD (>= 31 mm) at admitting chest CT is an independent predictor of mortality in COVID-19.

Summary: This study investigated the incidence, clinical characteristics, and outcomes of GBS in COVID-19 patients. The results showed an increased incidence of GBS in COVID-19 patients, suggesting a higher risk of GBS in individuals with COVID-19. Additionally, the study found a reduction in GBS events during the pandemic, which may be attributed to a decrease in the spread of more common respiratory infections.

Summary: In this study, CMR findings in patients with myocarditis-like syndrome during the acute and post-acute phase of COVID-19 were reported. The majority of cases were confirmed as myocarditis by CMR, with mild extent of lesions. Additionally, some patients had myocardial infarction and cardiomyopathies. CMR can improve diagnostic accuracy and evaluate subclinical cardiomyopathies.

Summary: This systematic review and meta-analysis aimed to evaluate the effect of full-dose heparin-based thromboprophylaxis on survival in hospitalized non-critically ill COVID-19 patients. The analysis showed that full-dose anticoagulation reduced all-cause mortality compared to prophylactic or intermediate dose anticoagulation. Therefore, full-dose anticoagulation can decrease all-cause mortality in hospitalized non-critically ill COVID-19 patients.

Summary: This study aimed to evaluate the diagnostic value of optical coherence tomography (OCT) for pulmonary microvascular thromboinflammatory syndrome in patients with COVID-19 pneumonia. The results showed that OCT assessment of the distal pulmonary arteries in hospitalized COVID-19 patients is safe and accurate, enabling the first in vivo documentation of distal pulmonary arterial thrombosis in patients with elevated thromboinflammatory markers, even when their CT angiogram was negative for pulmonary thrombosis.

Left ventricular assist devices promote changes in the expression levels of platelet microRNAs

Summary: This study found significant differences in the expression levels of platelet miR-126, miR-374b, miR-223, and miR-320a between LVAD patients and healthy controls. The expression levels of platelet miR-25, miR-144, miR-320, and miR-451a changed significantly during LVAD support, and these miRs were implicated in cardiac- and coagulation-associated pathways. Patients who experienced bleeding had significantly higher expression levels of platelet miR-151a and miR-454 before LVAD implantation, and these miRs were also differentially expressed before the clinical manifestation of bleeding events. This study provides evidence of significant modulation of platelet miRs expression driven by LVADs and suggests the potential predictive value of platelet miRs in bleeding events.

Summary: This study aims to investigate the efficacy of high dose esomeprazole in reducing organ dysfunction in patients with sepsis or septic shock. Through a randomized trial, the study will compare the mean daily Sequential Organ Failure Assessment (SOFA) score over 10 days between the high dose esomeprazole group and the placebo group, as well as secondary outcomes such as antibiotic-free days, single organ failure severity, intensive care unit-free days at day 28, and mortality. The results of this study are expected to provide new insights for the treatment of sepsis or septic shock.

Propofol and survival: an updated meta-analysis of randomized clinical trials

Summary: This study conducted a systematic review and meta-analysis, and found that propofol may reduce survival in perioperative and critically ill patients compared to other hypnotic agents. Therefore, a careful assessment of the risk versus benefit of propofol compared to other agents is needed when planning for large, pragmatic multicentric randomized controlled trials to provide a definitive answer.

Summary: This double-blind, randomized clinical trial investigated the impact of continuous vs. intermittent administration of meropenem on the composite outcome of mortality and emergence of pandrug-resistant or extensively drug-resistant bacteria in critically ill patients. The results showed that continuous administration did not improve the primary outcome at day 28, and none of the secondary outcomes were statistically significant.
