Gender incongruence and gender dysphoria in childhood and adolescence-current insights in diagnostics, management, and follow-up

Gender incongruence (GI) occurs when a person's gender identity does not align with the gender assigned at birth, requiring complex medical and psychological treatment within a multidisciplinary team. Long-term follow-up and additional studies are needed to optimize treatment regimens.
Gender incongruence (GI) is defined as a condition in which the gender identity of a person does not align with the gender assigned at birth. Awareness and more social acceptance have paved the way for early medical intervention about two decades ago and are now part of good clinical practice although much robust data is lacking. Medical and mental treatment in adolescents with GI is complex and is recommended to take place within a team of mental health professionals, psychiatrists, endocrinologists, and other healthcare providers. The somatic treatment generally consists of the use of GnRH analogues to prevent the progression of biological puberty and subsequently gender-affirming hormonal treatment to develop sex characteristics of the self-identified gender and surgical procedures. However to optimize treatment regimens, long-term follow-up and additional studies are still needed. What is known The prevalence of gender dysphoria increased significantly in the past years and can lead to significant complaints and burdens especially during puberty. Pubertal suppression and gender-affirmed treatment can be effectively used in adolescence with gender dysphoria. What is new Transgender mental and medical healthcare is a long-lasting process during which not only the child/adolescent with GI but also their parents/family have to be counseled in making choices about their social, medical, and legal transitions. There are an increasing number of transgender persons defining as nonbinary. Therefore, an individualized approach by an experienced team is necessary.

Measuring steroids in hair opens up possibilities to identify congenital adrenal hyperplasia in developing countries

Summary: This study developed a method to measure steroids in hair and found that the concentration of 17OHP, androstenedione, and testosterone in hair can be used to confirm CAH in patients and monitor treatment effectiveness. These findings provide opportunities for using a simple noninvasive technique to diagnose and monitor CAH in developing countries.

Summary: A survey conducted in Europe revealed a high degree of variability and discrepancy in the use of prenatal dexamethasone (Pdex) treatment for congenital adrenal hyperplasia (CAH). Only 36% of the surveyed medical centres provide Pdex treatment, and the starting point and dosage of the treatment vary among different centres. Limited availability of long-term follow-up registries was also observed. This highlights the importance of a European cooperation initiative to establish evidence-based guidelines for prenatal diagnostics, treatment, and follow-up of pregnancies at risk for CAH.

Summary: This study analyzed the use of salt supplementation, medication dosage, and growth indicators in children with CAH aged 0-3 years. The results showed that in children receiving additional salt supplementation, medication dosages were lower during the first months of life but there were no differences in weight, height, and blood pressure compared to those not receiving salt supplementation. All children showed an increase in BMI-SDS and a high proportion of blood pressure readings exceeding the normal range.

Management of Acute Adrenal Insufficiency-Related Adverse Events in Children with Congenital Adrenal Hyperplasia: Results of an International Survey of Specialist Centres

Summary: While there is variation in the definition and management of AI-related adverse events in children among centres, there is a good level of consensus on specific aspects that can lead to greater benchmarking of care. Over 90% of centres in high-income countries provided steroid management plans, one-to-one patient education, and contact details of health care staff. Additionally, the majority of centres recommended glucocorticoid stress dosing in the event of fever, and vomiting was the most common indication for intramuscular hydrocortisone injection.

Interpretation of Steroid Biomarkers in 21-Hydroxylase Deficiency and Their Use in Disease Management

Summary: The most common form of congenital adrenal hyperplasia is 21-hydroxylase deficiency (21OHD), occurring in 1:16 000 newborns worldwide. Lifelong treatment aims to replace deficiencies and suppress androgen production, but balancing between androgen excess and chronic high glucocorticoid exposure is challenging. Clinical assessment and biochemical parameters guide therapy, and understanding adrenal biomarker values is important. The pathophysiologic and pharmacologic principles are utilized to manage complications and consequences of 21OHD.

Elevated IGF-1 concentrations in children with low grade glioma: A descriptive analysis in a retrospective national cohort

Summary: To evaluate the prevalence and clinical effects of elevated insulin-like growth factor 1 (IGF-1) concentrations in children with low grade glioma (LGG), a retrospective study collected tumor, treatment, endocrine, and auxological data from 358 children. The study found that 18.4% of children with available IGF-1 measurements had elevated IGF-1 concentrations, but no association was found between elevated IGF-1 concentrations and tumor behavior or height SDS at last follow-up. Elevated IGF-1 concentrations were associated with worsening hypothalamic dysfunction over time.

Novel likely pathogenic variant in NR5A1 gene in a Tanzanian child with 46,XY differences of sex development, inherited from the mosaic father

Summary: Pathogenic variants in the NR5A1 gene can cause differences in sex development. We report the clinical, endocrinological, and genetic characteristics of a patient with a novel likely pathogenic missense variant in the NR5A1 gene. This variant was inherited from the apparently unaffected mosaic father, which might have implications for the recurrence risk in this family.

Summary: Testicular adrenal rest tumors (TARTs) may arise from chronic stimulation of adrenocorticotropic hormone (ACTH)-sensitive cells in the testes. However, this study found that the human fetal testis (HFT) is unresponsive to ACTH, suggesting that TARTs do not arise from fetal testicular cells chronically exposed to ACTH in utero.
