Analysis of a child who developed abnormal neuropsychiatric symptoms after administration of oseltamivir: a case report

Background: Neuropsychiatric side effects of oseltamivir occur occasionally, especially in infants and young patients, but nothing is known about possible contributory factors. Case presentation: We report a case of a 15-year-old Japanese female with influenza infection who developed abnormal psychiatric symptoms after administration of standard doses of oseltamivir. She had no history of neurological illness, had never previously taken oseltamivir, and had not developed psychiatric reactions during previous influenza infection. Her delirium-like symptoms, including insomnia, visual hallucinations, and a long-term memory deficit, disappeared after cessation of oseltamivir and administration of benzodiazepine. Detailed assessment was performed, including neurological examination (electroencephalogram, brain magnetic resonance imaging, single photon emission computed tomography with 99m Tc-ethyl cysteinate dimer and with I-123-iomazenil, cerebrospinal fluid analysis and glutamate receptor autoantibodies), drug level determination and simulation, and genetic assessment (OAT1, OAT3, CES1, Neu2). Conclusions: Abnormal slowing in the electroencephalogram, which is characteristic of influenza-associated encephalopathy, was not observed in repeated recordings. The serum level determination of active metabolite Ro 64-0802 determined at 154 h after final dosing of oseltamivir was higher than the expected value, suggesting delayed elimination of Ro 64-0802. Thus, abnormal exposure to Ro 64-0802 might have contributed, at least in part, to the development of neuropsychiatric symptoms in this patient. The score on Naranjo's adverse drug reaction probability scale was 6. Mutation of c.122G > A (R41Q) in the sialidase Neu2 gene, increased CSF glutamate receptor autoantibodies, and limbic GABAergic dysfunction indicated by SPECT with I-123-iomazenil were found as possible contributory factors to the CNS side effects.

Summary: The study reveals the challenging neurological, physical and developmental aspects, as well as intractable seizures, in patients with West syndrome. More than a half of the children showed developmental delay after onset, even though seizures were reduced during the course of the disease.

Summary: This study examined the medical and psychosocial status of patients with epilepsy in Japan to assess the application of the Intractable/Rare Diseases Act. The majority of epilepsy patients met the severity criteria defined by the Act, but some groups had lower proportions due to successful surgical therapy. The study suggests that further research is needed to update the list of designated syndromes/diseases in the Act.

Aphasia and a Dual-Stream Language Model in a 4-Year-Old Female with Landau-Kleffner Syndrome

Summary: Landau-Kleffner syndrome is a rare neurological disorder characterized by acquired aphasia with distinctive EEG findings. In a pediatric case where LKS was diagnosed, imaging findings suggested involvement of the right hemisphere in language comprehension. Although little is known about the mechanisms of aphasia and its recovery, disparities between expressive and receptive aphasia recovery were observed.

The analysis of 34 cases and a new case revealed distinct seizure types and EEG abnormalities in MOG-CCE.

Therapeutic Drug Monitoring for Rufinamide in Japanese Patients With Epilepsy: Focus on Drug Interactions, Tolerability, and Clinical Effectiveness

Summary: This study assessed the drug interactions between rufinamide and concomitant antiepileptic drugs (AEDs), and identified the therapeutic window for rufinamide. The results showed that rufinamide exhibited linear pharmacokinetics and its concentrations were affected by the concomitant use of enzyme-inducing AEDs and valproate. Clinical response was observed in approximately 23.0% of patients, with higher rufinamide concentrations in patients with tonic/atonic seizures. Adverse events were reported in 35.8% of patients, and patients with concentrations greater than 20 mcg/mL had a higher incidence of adverse events.

Impact of the coronavirus pandemic on families of patients with congenital insensitivity to pain with anhidrosis

Summary: COVID-19 outbreaks have had various effects on the physical and mental health of families and children with congenital insensitivity to pain with anhidrosis (CIPA). A questionnaire survey was conducted to identify the specific issues and needs of CIPA patients and their families during the pandemic. The survey found that families faced difficulties in predicting the future, household and work concerns, and fear of infection. Increased time together during the pandemic led to both stress and better understanding among families. Sleep disturbances and behavioral changes were observed in a significant number of CIPA patients. Each family had its own coping mechanisms, but the importance of peer support, connections with experts, and prompt responses for resolution were emphasized.

Clinical value of therapeutic drug monitoring for levetiracetam in pediatric patients with epilepsy

Summary: Limbic encephalitis (LE) is characterized by acute or subacute impairment of short-term memory, seizures, and psychiatric symptoms. This report describes a 46-year-old Japanese female with SLE who was diagnosed with severe NPSLE presenting as LE-SLE and showed rapid response to treatment with methylprednisolone pulses and high-dose prednisolone. The patient had elevated anti-GluRs levels in serum and CSF on admission, which decreased to normal range at one-year follow up, indicating potential prognostic value.
