An overview of systematic reviews on upper extremity outcome measures after stroke

Background: Although use of standardized and scientifically sound outcome measures is highly encouraged in clinical practice and research, there is still no clear recommendation on which tools should be preferred for upper extremity assessment after stroke. As the aims, objectives and methodology of the existing reviews of the upper extremity outcome measures can vary, there is a need to bring together the evidence from existing multiple reviews. The purpose of this review was to provide an overview of evidence of the psychometric properties and clinical utility of upper extremity outcome measures for use in stroke, by systematically evaluating and summarizing findings from systematic reviews. Methods: A comprehensive systematic search was performed including systematic reviews from 2004 to February 2014. A methodological quality appraisal of the reviews was performed using the AMSTAR-tool. Results: From 13 included systematic reviews, 53 measures were identified of which 13 met the standardized criteria set for the psychometric properties. The strongest level of measurement quality and clinical utility was demonstrated for Fugl-Meyer Assessment, Action Research Arm Test, Box and Block Test, Chedoke Arm and Hand Activity Inventory, Wolf Motor Function Test and ABILHAND. Conclusions: This overview of systematic reviews provides a comprehensive systematic synthesis of evidence on which outcome measures demonstrate a high level of measurement quality and clinical utility and which can be considered as most suitable for upper extremity assessment after stroke. This overview can provide a valuable resource to assist clinicians, researchers and policy makers in selection of appropriate outcome measures.

Urdu translation and cross-cultural validation of the Fugl-Meyer assessment in people with stroke

Summary: The study found that the Urdu FMA is reliable and valid for assessing sensorimotor function in people with chronic stroke, with acceptable internal consistency and reliability. It also showed moderate correlations with Functional Independence Measure, Modified Rankin Scale, and National Institute of Health Stroke Severity. Factor analysis revealed that two factors (upper and lower extremity) explained 67.8% of the variance.

Embodied learning in multiple sclerosis using melodic, sound, and visual feedback: a potential rehabilitation approach

Upper Limb Stroke Rehabilitation Using Surface Electromyography: A Systematic Review and Meta-Analysis

Summary: This review and meta-analysis evaluated the effectiveness of different types of sEMG-driven interventions on upper limb function in stroke patients. The results did not show significant differences between sEMG interventions compared to non-sEMG interventions or between different types of sEMG interventions in improving upper limb function. Further research with larger sample sizes is needed to determine the specific effects of sEMG interventions on upper limb function in stroke populations.

Intra- and inter-rater reliability of the Italian Fugl-Meyer assessment of upper and lower extremity

Summary: The purpose of this study was to assess the reliability of the Italian version of the Fugl-Meyer Assessment in patients with sub-acute stroke. The results showed that the FMA had satisfactory intra- and inter-rater reliability at the item-level, and reliability levels above 70% were achieved at the subscale and total score level when considering one- or two-points difference. Systematic disagreements were found for some items of the FMA-UE, but not for FMA-LE. The study concludes that the Italian version of the FMA is a reliable instrument for clinical and research purposes.

Early prediction of upper limb functioning after stroke using clinical bedside assessments: a prospective longitudinal study

Summary: This study aimed to develop a prediction algorithm using a set of simple early clinical bedside measures to predict upper limb capacity at 3-months post-stroke. The algorithm achieved high sensitivity, specificity, and accuracy in differentiating between poor, limited, and good outcomes. Simple bedside assessments have advantages and can be integrated into routine clinical practice and used in data registries to aid therapy decisions and guide patient selection in clinical trials.

Patient-Reported Outcome Measures for Assessing Dual-Task Performance in Daily Life: A Review of Current Instruments, Use, and Measurement Properties

Exercise dosage to facilitate the recovery of balance, walking, and quality of life after stroke

Summary: This study analyzed the optimal dosage and effectiveness of aerobic training (AT) and resistance training (RT) on balance, walking capacity, and quality of life (QoL) in stroke survivors. The results showed that AT was the most effective in improving walking capacity, while combining AT and RT was beneficial for improving QoL. High-dose aerobic exercise conducted in rehabilitation facilities was more effective in improving walking capacity in chronic stroke patients.

The evaluation includes clinical assessments, neuroimaging, and blood sample analysis, and the study also records clinical variables that may influence recovery outcomes. The results of this study will provide novel data on stroke recovery and biomarkers.

Effects of Peripheral Cooling on Upper Limb Tremor Severity and Functional Capacity in Persons with MS

Summary: This study investigated the effect of peripheral upper limb cooling on tremor severity and functional performance in multiple sclerosis patients with intention tremor. The results showed that cooling the forearm significantly reduced tremor severity and improved functional performance, and cooling the whole upper limb had a noticeable effect on tremor severity during the first half-hour after cooling.

Individual interviews were conducted with 26 participants and their next-of-kin, and the transcripts were analyzed using qualitative content analysis and International Classification of Functioning, Disability, and Health (ICF) linking methods. The results showed that next-of-kin perceived higher levels of disability among individuals with perceived cognitive impairment or dementia compared to the participants themselves.
