Intraperitoneal Administration of Paclitaxel Combined with S-1 Plus Oxaliplatin as Induction Therapy for Patients with Advanced Gastric Cancer with Peritoneal Metastases

Shin Saito, Hironori Yamaguchi, Hideyuki Ohzawa, Hideyo Miyato, Rihito Kanamaru, Kentaro Kurashina, Yoshinori Hosoya, Alan Kawarai Lefor, Naohiro Sata, Joji Kitayama

Summary: The combination chemotherapy using SOX + IP-PTX regimen showed high efficacy in patients with peritoneal metastases from gastric cancer, with a 1-year overall survival rate of 79.5%. Common toxicities included neutropenia, leukopenia, and anemia, but there were no treatment-related deaths.

Flow cytometry-based analysis of tumor-leukocyte ratios in peritoneal fluid from patients with advanced gastric cancer

Kazuya Takahashi, Kentaro Kurashina, Shin Saito, Rihito Kanamaru, Hideyuki Ohzawa, Hironori Yamaguchi, Hideyo Miyato, Yoshinori Hosoya, Alan Kawarai Lefor, Naohiro Sata, Joji Kitayama

Summary: The study showed that the detection of tumor cells in ascites or peritoneal lavages using flow cytometry is correlated with the progression of peritoneal metastases. Tumor-leukocyte ratio (TLR) is associated with peritoneal cancer index scores and the ratios of different cell types.

MiR-29b may suppresses peritoneal metastases through inhibition of the mesothelial-mesenchymal transition (MMT) of human peritoneal mesothelial cells

Yuki Kimura, Hideyuki Ohzawa, Hideyo Miyato, Yuki Kaneko, Akira Saito, Kazuya Takahashi, Mineyuki Tojo, Hironori Yamaguchi, Kentaro Kurashina, Shin Saito, Yoshinori Hosoya, Alan Kawarai Lefor, Naohiro Sata, Joji Kitayama

Summary: Peritoneal dissemination, a major metastatic pathway for gastrointestinal and ovarian malignancies, is influenced by the downregulation of miR-29b in peritoneal fluids. Through in vitro experiments, it was found that miR-29b inhibits TGF-beta 1 induced mesothelial-mesenchymal transition (MMT) and has a regulatory effect on mesothelial cells. MiR-29b may be a potential therapeutic target for preventing the development of peritoneal metastases.

Preoperative Osteopenia Is Associated with Significantly Shorter Survival in Patients with Perihilar Cholangiocarcinoma

Jun Watanabe, Atsushi Miki, Yasunaru Sakuma, Kentaro Shimodaira, Yuichi Aoki, Yoshiyuki Meguro, Kazue Morishima, Kazuhiro Endo, Hideki Sasanuma, Alan Kawarai Lefor, Takumi Teratani, Noriyoshi Fukushima, Joji Kitayama, Naohiro Sata

Summary: This study aimed to evaluate whether osteopenia is a prognostic factor in patients with perihilar cholangiocarcinoma. The study found that osteopenia is associated with significantly shorter survival in patients with perihilar cholangiocarcinoma, and preoperative osteopenia may be a useful tool for predicting prognosis in these patients.

Effect of Oral Administration of Lactiplantibacillus plantarum SNK12 on Temporary Stress in Adults: A Randomized, Placebo-Controlled, Double-Blind, Parallel-Group Study

Takumi Watanabe, Kyoko Hayashi, Tsuyoshi Takara, Takumi Teratani, Joji Kitayama, Toshio Kawahara

Summary: Studies in mice have shown that Lactiplantibacillus plantarum SNK12 (SNK) has anti-stress effects, increasing neurotrophic factor and gamma-aminobutyric acid receptor levels in the hippocampus and improving depressive behavior. In this study, SNK ingestion was found to reduce stress levels, negative feelings, and stress-related markers in healthy adults.

Novel Drug Delivery Method Targeting Para-Aortic Lymph Nodes by Retrograde Infusion of Paclitaxel into Pigs' Thoracic Duct

Akira Saito, Natsuka Kimura, Yuji Kaneda, Hideyuki Ohzawa, Hideyo Miyato, Hironori Yamaguchi, Alan Kawarai Lefor, Ryozo Nagai, Naohiro Sata, Joji Kitayama, Kenichi Aizawa

Summary: This study investigated the efficiency of delivering anticancer drugs directly to intra-abdominal lymph nodes by retrograde administration through the thoracic duct. The results showed that thoracic infusion achieved higher drug doses in the lymph nodes compared to systemic administration. This method may have clinical benefits for patients with extensive lymphatic metastases in abdominal malignancies.

Simulator Fidelity Does Not Affect Training for Robot-Assisted Minimally Invasive Surgery

Shin Saito, Kazuhiro Endo, Yasunaru Sakuma, Naohiro Sata, Alan Kawarai Lefor

Summary: This study compared performance using a surgical robot after training with different fidelity simulators. The results showed significant differences in time and GEARS scores among the different simulators. The conclusion is that training with simulators of varying fidelity can achieve similar performance using the da Vinci robot.

Anatomical Targeting of Anticancer Drugs to Solid Tumors Using Specific Administration Routes: Review

Akira Saito, Joji Kitayama, Ryozo Nagai, Kenichi Aizawa

Summary: Despite progress in developing anti-cancer agents, systemic chemotherapy for solid tumors leads to insufficient drug uptake by targeted tumor tissue and often damages healthy organs. Local administration of anti-cancer agents offers a strategy to increase drug concentrations in tumor tissue while reducing systemic toxicity. However, the survival benefits of this approach are limited. This review analyzes clinical results and discusses future directions for regional cancer therapy with local administration of chemotherapy.
