Summary: Multiple myeloma patients undergoing anti-myeloma therapy have an increased risk of venous thromboembolism (VTE). Current guidelines recommend low-dose direct oral anticoagulants (DOACs) as an alternative to aspirin for thromboprophylaxis. A systematic review and meta-analysis comparing DOACs with aspirin showed that DOACs had a significantly lower incidence of VTE. Bleeding event rates did not differ significantly between the two groups. This suggests that DOACs may be a preferable option for thromboprophylaxis in multiple myeloma patients, but further research is needed to evaluate the optimal strategy.

The findings highlight the importance of optimizing blood pressure control and screening for target organ damage in children with ambulatory hypertension.

Summary: This systematic review and meta-analysis evaluated the efficacy of direct oral anticoagulants (DOACs) compared with enoxaparin in an Asian population after total knee arthroplasty (TKA). The results showed that DOACs significantly reduced the rate of overall venous thromboembolism (VTE) compared with enoxaparin. However, there were no significant differences in deep vein thrombosis, pulmonary embolism, and rates of bleeding complications between the two.

Summary: This study applied covariate-specific receiver operating characteristic (ROC) curve analysis to individual patient data on D-dimer testing for excluding pulmonary embolism. The results showed significant differences in D-dimer concentration and performance among age groups, while the distribution of results was similar for sex. Estimating conditional ROC curves can improve the ability to select thresholds with greater applicability.

Summary: In the past, patients with pancreatic ductal adenocarcinoma often underwent immediate surgery for tumor resection. Now, more and more patients receive chemotherapy (radiotherapy) before surgery. It is known that pancreatic cancer patients have a higher risk of developing thrombosis. However, the incidence of thrombosis in patients with pancreatic cancer during neoadjuvant chemotherapy is not well-studied. Few studies have investigated VTE incidence in this population, and it is unclear if these patients should use perioperative thromboprophylaxis. This narrative review summarizes the current evidence.

Summary: Antiphospholipid syndrome (APS) is an autoimmune disease characterized by the presence of antiphospholipid antibodies (aPL) that cause thrombotic and obstetric complications. APS serves as a model for studying the mechanisms of thromboinflammation and the relationship between innate immune cells and thrombosis. Monocytes are activated by aPL to produce proinflammatory cytokines and tissue factor, while neutrophils generate neutrophil extracellular traps and interact with endothelial cells, leading to thrombosis. Platelets become procoagulant upon activation by aPL and increase interactions with leukocytes. Understanding these mechanisms provides potential therapeutic targets for APS and other thromboinflammatory diseases.

Summary: Imaging tests are commonly used in healthcare, but concerns about their overuse have overshadowed their benefits in clinical decision-making. This study demonstrates the importance of considering patient-centered outcomes (PCOs) when evaluating the value of imaging tests. The PROD framework categorizes PCOs into four main domains and highlights the interplay between them and the factors that can modify them.
